ADMISSION REQUIREMENTS

1. Original and one (1) photocopy of Form 138 (High School Card);

2. Original and one (1) photocopy of MSU-SASE Report of Rating;

3. Original and one (1) photocopy of Certificate of Good Moral Character issued by an official of the school previously attended;

4. Authenticated Copy of Birth Certificate from NSO (to be returned), and 2 photocopies of it;

5. Two (2) copies of ID pictures 1”x1” and two (2) 2”x2” size bust picture (recent and un- retouched); and

6. Two (2) long brown envelopes

1. Original and one (1) photocopy of Report of Rating (MSU SASE or MCAT);

2. Original and one (1) photocopy of Transfer Credential/ Honorable Dismissal;

3. Original and one (1) photocopy of Transcript of Records (TOR);

4. Original and one (1) photocopy of Certificate of Good Moral Character issued by an official of the school previously attended;

5. Authenticated Copy of Birth Certificate from NSO (to be returned), and 2 photocopies of it;

6. Two (2) copies of ID pictures 1”x1” and two (2) 2”x2” size bust picture (recent and un- retouched); and

7. Two (2) long brown envelopes
