ADVISORY GUIDE

Introduction:

Applying for the creation or amendment of an academic program is an accurate process that requires providing multiple and valid information. Therefore, kindly read the form items carefully and fill out the form accurately. It is also hoped that you will avoid unjustified redundancy or summaries that may affect information quality.

Before you start filling out the form, please gather necessary information that will help you to complete this application smoothly and accurately taking into account the following important items:

1. Information related to the goals, vision and future plan of the program.

2. The components and infrastructure of the program.

3. The teaching and administrative staff.

4. The course plan and its components.

5. Feasibility study on creating a new department (justifications of creation, its economic impacts on community and areas of work for department’s graduates).

Application Terms & Conditions:

1. Fill out all attached forms. In case it is not possible to provide a certain detail, the University Rectorate for Educational Affairs (Planning Committee) shall be contacted 2. All required documents and data must be attached to the application.

3. The completed information should be accurate and clear.

Attachments:

When submitting this application, the following are to be attached:

1. The Program Description Form according to the National Commission for Academic Accreditation and Assessment, and according to the approved teaching language in the department.

2. Course Description Form of the study plan according to the approved teaching language in the department.

3. Minutes of the meetings held by the department’s plan committee, and college’s plan committee if related to the amendment or creation of the study plan.

4. Minutes of the department and college meetings including their approval and recommendation to create or amend the study plan.

5. Evidence indicating procedures taken prior to the preparation of the study plan (addressing the labor market, holding workshops or meetings with the community, graduates and faculty members)

6. Evidence indicates the mechanism of selecting references and relevant adoption and actions accordingly.

7. Formal communications and mechanisms taken to validate the study plan.

8. Any evidence indicating the quality of the study plan and its conformity with national and international standards.

Note: The information in the attachments is to be provided in the application and is supposed to be prepared earlier in order to help you fill out the application form.

How to complete an application:

1. The application must be completed electronically.

2. The application must be clearly printed out on one-sided paper.

3. Three copies of the application and one copy of attachments are to be submitted.

4. The attachments are to be enclosed to the last page of the application and must be referred to during filling the submitted application in the assigned area.

5. In case there are other attachments, they must be attached and also referred to during filling the submitted application in the assigned area.
