Trong dd cd 6 yeu 16' cd gia trj tien lupng

cao hPn la: Tien sl!r dot quy, huyel ap lam thu khi_vao vien > 200mmHg, diem Glasgow < 9, dudi cirng mat nao, rd'i loan than kinh thuc vat (sdt > 40°, va md hdi lanh, tang tiet ddm dai, gian ddng tii, mat phan xa ddng t i j vdi ang sang); dudng kinh d ton liiupng > 2cm. Cac trieu chimg dudi cUng mat nao, co giat chie'm ty le thap nhUng la nhUng trieu chifng tien lUdng nang va gap 6 nhdm tCr vong.

SUMMARY

The result of NPC with stage I I I , w patients that treated by concurrent radiochencotherapy The result of study 56 NPC with stage Hi i v patients that treated by concurrent radiochemotherapy (linac 3D-CRT) at K hospital during from April 2007 to April 2009 as follows:

The rate of tumoral complete response; partial response is 91,1%; 8,9%, respectively. The rate of nodal complete response; partial response is 92,8%;

7,2%, respectively. Overall rate of complete response;

partial response is 89,3%; 10,7% respectively Rate of 3 year overall survival; 4 year overall sun/ival is 85,1%; 72,2% respectively. The average sun/ival time is 46,6 months.

Rate of 3 year disease free survival; 4 year disease free survival is 81,3% and 65,2%. Rate of recurrence found at nasopharynx; local lymph node is 7,1%; 3,6%, respectively. Rate of long distance metastasis is 16,1% (oss metastasis is 8,9%; liver is 3,6%; lung metastasis is 1,8%; Brain metastase is 1,8%) among them.

2.3. Xa trj la phirdng phap dieu tri chinh va chu yeu trong UTVH. Cisplatin: Benh nhan dupc truyen 2 lit Glucose 5 % va 40mEq KCI/L va 12,5gm mannitol trudc truyen Cisplatin. Cisplatin dupe truyen trong 20-30 phiil sau dd truyen 1 lit

Glucose 5%, 40mEq KCI, 25gm manitol trong 4 gid va tiep sau dd Hit Glucose 5%, 40mEq KCI, MgS04 trong 8 gid.

Benh nhan dupc dung Ondanxetron va Dexamethasone trudc, sau truyen hda chat de d u phong ndn va dj ifng thud'c.

5FU: Lieu 1000mg/m^/24 gid truyen lien tuc trong 96 gid vao vao cac tuan 11,15.19 sau dpi truyen manitol thU 2 dupc hoan thanh.

So do dieu tri tie'n hanii nhu trong bang 1.

Budc 1: hoa xa dong thdi Xa tri 3D theo hinh dang khoi u bang may gia toe.

2.7. Xtir ly so' lieu: Sau khi thu thap, dUpc xCr ly bang phan mem tin hpc SPSS 16,0 va Stada 10.0. Phan tich thdi gian sd'ng them theo phuong phap Kaplan-Meier. So sanh thdi gian sd'ng them giua cac nhdm bang test Log Rank va md hinh hoi qui Cox (trong phan tich da bien).

III. KET QUA VA BAN LUAN

Trong nghien ciflj cOa chiing tdi toan bp benh nhan IhUc hien dung qui trinh xa trj gia toe dat ra.

100% sd benh nhan dieu tri dO 6 dpi hda tri, dat lieu xa lai u nguyen phat va hach di can 70Gy, trong dd cd 5 benh nhan phai keo dai thdi gian xa trj 2-3 tuan do ton thUPng da va niem mac do tia.

Trong nghien ciru ciia chung tdi vdi thdi gian theo doi tU 6 de'n 53 thang t^ le song them 3 nam, 4 nam toan bp la 85,1% va 72,2%, lan lUPl. Thdi gian sd'ng them trung binh 46,6 thang. Ty le song them 3 nam, 4 nam khdng benh la 81,3% va 65,2% lan lupt.

3.3. Tai phat va di can

T^ le tai phat tai vom la 7,1%, tai hach la

Ke't qua ve tai phat tai chd tai vimg trong nghien ciru cCia chung toi phii hpp vdi cac lac gia trong va ngoai nudc ty le tai phat tai u la 7,1% (4 BN) va t9 le tai phat lai^hach 3,6% (2 BN).

Ngd Thanh Tiing nghien cUu 367 benh nhan UTVH dupc dieu trj tU nSm 1993-1995 cho thay t^ le di can xa chiem 17,4% sau thdi gian theo ddi 36 thang, xuong bj di can nhieu nhat (25%) sau dd la gan (19,4%), phoi (13,9%). Thdi gian di can chil ye'u dien ra trong 2 nam dau sau dieu trj (63,9%). T ^ le tai phat va di can sau dieu trj trong nghien ciru nay thap hon hau he't cac nghien ciru trong va ngoai nUdc trudc day chimg td vai trd quan trpng ciia phac do hda xa tri dong thdi giup giam t^ le tai phat va di can.

IV. KET LU/kN 1. Dap umg dieu trj:

T9 le dap img hoan toan lai u 9 1 , 1 % , mot phan tai u la 8,9%.

SUMMARY

ASSESSMENT ON RESULT OF MALIGNANT LYMPHO ADENOMA NON - HODGKIN TREATMENT BY CHOP AND DHAP DIAGRAM

Through the study of 80 patients treated under CHOP and DHAP diagram. We found that: the clinical charactenstics in examination of 2 distinctive study groups have no significant result. To meet the complete treatment and commonly meet, the DHAP diagram is higher than that of CHOP one.
