SUMMARY
Objectives: Determine the rate and risk factors related to overweight - obesity in children aged 6 to 10 years in primary schools, town of Ninh Hoa, Khanh Hoa province.
Methods: Phase 1: 1950-level investigation of primary pupils from 6-10 years old In the town of Ninh Hoa, Khanh Hoa province, from 01/10/2010 to 30/11/2010 to detennine the percentage of overweight-obese. Phase 2: case-control studies (278 students) to detennine cause and effect associated with differential rates.

obesity was 7.13% (95% CI: 5.99 to 8.42%), of which 3.74% are obese ( 9 5 % CI: 2.93 to 4.71
% ) . The rate of overweight - obese high school boys than girls (8.96% versus 5.18%, p <0.001).
The rate of overweight - obesity is highest in 9 years (9.61%), lowest in 7 years (5.37%). The degree of ovenweight mostly mild (76.25%), moderate overweight and weight is 23.75%. The risk factors involved including children whose fathers and/or overweight mothers; trafficking parents do the job; parents do not recognize children's nutritional status. Children with eating habits and fast food, snack, eating snacks before bed night diet rich in energy, less physical activity (<10 hours/week) static activity > 20 hours/week.
Conclusion: Our Results show that the rate contribute to overweight - obesity among primary school children in Khanh Hoa and increasingly necessary interventions for the risk factors for child protection and public health contract.

Tai Khanh Hoa, nghien cuu d thanh phd Nha Trang nam 1997 ghi nhan ty le thira can - beo phi hpc sinh tieu hpc la 2,7%, den nam 2001 da tang len 5,8% [1], [13]. Tuy nhien, van de thira can - beo phi tai thi xa Ninh Hda cho den nay chua cd cdng trinh nghien cuu nao khao sat, danh gia. Vi vay, chiing tdi tien hanh nghien cmi de tai nay vdi muc tieu:
1. Xac dinh ty le thica cdn - beo phi d tre tu 6 den 10 tuoi tgi cdc truong tieu hoc, thi xd Ninh Hod, tinh Khanh Hoa;
2. Tim hieu mot so yeu to lien quan den tinh trang thica cdn - beo phi cua tre em tai cdc truong neu tren.
II. Do'l TUpNG VA PHUONG PHAP NGHIEN COU
2.1. Giai doan 1: Dieu tra ngang 1950 hpc sinh cac trudng tilu hpc, iiia tudi tir 6 -10, t^i thi xa Ninh Hoa, tmh Khanh Hda, theo phuong thiic chgn mau tang cho khdi trudng va khdi Idp, vdi cdng thiic tinh co mau:

- Nhdm thira can - beo phi: nhiing hoc sinh dugc kham d giai doan 1 ma cd can nang/ chi6u cao > +2 SD hoac cd BMI > 85 bach phan vi. Kk qua giai doan 1 cd 139 hoc sinh TC-BP nen giai doan 2 chung tdi kham toan bd 139 em.
- Nhdm chumg: 139 hoc sinh cd ciing tudi, gidi, dia du, dan tdc vdi nhdm thira can - beo phi va cd CN/ CC hoac BMI trong gidi ban binh thudng.
- Bd me cua hpc smh ca 2 nhdm neu tren.
2.3. Thoi diem nghien cmi Tir 01/10/2010 ddn 30/11/2010 2.4. Cac chi tieu de danh gia va phan loai thira can, beo phi
2.4.1. Danh gid thira cdn
Doi vdi tre diroi 9 tuoi, chieu cao < 145 cm (nam) hoSc < 137 cm (nii). Dua vao chi sd can nang theo chieu cao (CN/ CC), phan loai nhu sau:
> + 2 SD : Thira can
> + 2 SD dSn + 3 SD : Thira can nhe
> + 3SD dgn + 4 SD: Thira can trung binh
> + 4 SD : Thira can nang
* Doi vol tre tir 9 tuoi den 10 tu^i, hoac tre cd chieu cao vugt qua gidi han ap dung chi tieu CN/ CC cOa WHO thi dira vao bach phan vi BMI theo tudi va gidi cua tre nhu sau:
> 85 den < 95 bach phan vi: Thira can nhe
> 95 bach phan vi: Thira can trung binh vanang

2.4.3. Tieu chuan loai trir
Loai trir cac beo phi thii phat: beo phi do nguyen nhan ndi tiet, do khiSm khuydt di tmyen, do diing thudc ...Do vay, nhung hoc sinh cd chieu cao theo tudi thap hon 5 bach phan vj, chiing tdi khdng dira vao nghien cuu.
2.5 Xir ly so lieu: Bang phuong phap thdng ke Y hoc vdi sir ho tro cua phan mkm EXCEL, SPSS 16.0. Trong nghien cuu md ta, sir dung sd do ty le phan tram. Trong nghien ciiu benh chiing, su dung ty suat chenh (OR:
Odds Ratio); test khi binh phuong (test z') va miic y nghia ciia sir khac biet.

IV. BAN LUAN
4.1. Ty le thu-a can - beo phi ciia hoc sinh tieu hoc thi xa Ninh Hoa
Qua nghien cuu 1950 hoc sinh tieu hoc tir 6 - 10 tudi tai thi xa Ninh Hda, tinh Khanh Hda, chung tdi cd mpt sd ket qua va nhan xet sau:
4.1.1. Ty le thira cdn ~ beo pill Ty le thira can - beo phi d hpc sinh tieu hpc thi xa Ninh Hda la 7,13%, trong do ty le beo phi chiem 3,74%.
Theo ket qua nghien ciru cua cac tac gia trong nude, tir nam 2002 den nay tai cac dia phuong mien Bac, thay ty le TC-BP la 6,2%
[5]; 4,1% [7]; 9,9% [6]; 4,1% [3]. Tai khu vuc mien Trung, khao sat d hoc sinh tieu hpc thanh phd Nha Trang nam 1997 thay ty le TC-BP la 2,7% [13], nam 2001 tang len 5,88% (p<0,001) [1]; Nghien cim tai Hud (2005) thdy TC-BP chidm 6,4%, trong dd BP chiem 3,1% [11]. Tai tp Hd Chi Minh, nghien cim nam 1998 thky ty le thira can la 12,2% [9]
va den nam 2003 khao sat lai thay ty le TC-BP tang len 22,7% (gan gap ddi); Mpt nghien cuu khac Hoang Thi Mai nam 2008 thay ty le TC- BP la 26,1% (n= 1745) [10].
So sanh vdi cac ket qua nghien cuu trong nude tren day va d cung dp tudi, ty le TC-BP trong nghien cuu cua chiing tdi tuong ddng vdi ty le TC-BP ciia cac tac gia mien Bac, mien Trung va thap hon khu vuc phia Nam.
Tuy thdi gian va dia diem nghien cuu thuc hien tai nhimg vung cd dieu kien kinh te khac nhau, nhung deu toat len dupe y nghTa la ty le TC-BP ngay cang gia tang, ty le TC-BP d hpc sinh tieu hpc thanh phd Hd Chi Minh cao hon tre cung tudi d cac viing khac. DSc biet tai Khanh Hda, ty le TC-BP tang d4n tir 2,7%
nam 2001, tang len 5,88% nam 2001 (d tp Nha Trang) va tang din 7,13% nam 2011 (d thi xa Ninh Hda).
Theo WHO, tren toan the gidi tinh hinh beo phi da tang len gap ddi ke tir nam 1980.

Cho den nay, tai thi xa Ninh Hda tinh Khanh Hda chua cd nghien cihi nao danh gia tinh hinh thira can - beo phi. Mac dii nhieu nam qua, chinh quyen dia phuong va nganh y te thi x5 luon quan tam dieu tra, danh gia va cd ke hoach phdng chdng suy dinh dudng cho tre dudi 5 tudi, vdi ket qua khich le la giam tre suy dinh duong dudi 5 tuoi cdn 8,4% vao cudi nam 2010, thap hon ty le suy dinh dudng cua tinh (10%) va toan qudc.
Qua nghien cim cua chung tdi, hpc sinh tieu hoc cd ty le TC-BP la 7,13%, ben canh dd suy dinh duong van cdn ty le la 6,92%. 4.2.2. Yiu td ban than tre

benh giam ap, lam giam cac di chu'ng tai bien do lan gay ra.Tii thang 2/2009 den thang 2/2011, Benh vien Dieu du9ng - Phuc hoi chiic nang Khanh Hoa da Oiu dung dieu tn 53 benh nhan bj iiet do lan bang OXCA.
