V.11 LAMPIRAN APPENDIX APPENDIX
1. Isi Lampiran (jika ada) antara lain kutipan-kutipan panjang, lembaran-lembaran (data sheet), penurunan rumus yang relevan atau list program yang kalau dimasukkan dalam bagian Inti terlalu berlebihan, tetapi jika dibuang sayang. Dokumen asli, foto-foto, formulir-formulir dan semacamnya dapat juga dimasukkan dalam Lampiran.
The contents of the Appendix (if any) include long quotations, data sheets, derivative formulas that are relevant or list of programs which if included in the Core section is too excessive, but if discarded, it would be a shame. Original documents, photographs, forms and similar contents can also be included in the Appendix.
2. Dokumen yang hendak dimasukkan dalam Lampiran tidak boleh ditempelkan begitu saja, melainkan harus difotocopi pada kertas yang seukuran dengan kertas laporan KP, kecuali jika dokumen itu telah sama formatnya dengan ukuran kerta laporan KP.
Documents to be included in the Appendix may not be simply stitched but must be photocopied on paper the same size as the Ppractical workk report paper, unless the document is in the same format as the Practical Work report paper size.
3. Agar Lampiran tampak menonjol sebagai bab yang berdiri sendiri, di depan halaman pertama dari Lampiran dapat disisipi satu halaman kosong yang ditulis kata Lampiran.
In order for the Appendix to stand out as a standalone chapter, in front of the first page of the Appendix, a blank page with the words Appendix can be inserted.
4. Masing-masing jenis bahan yang dilampirkan dapat dilampirkan secara terpisah. Masing-masing Lampiran itu kemudian diberi nomor urut secara Alfabetik (A, B, C ……) dan diberi judul sesuai dengan maksudnya.
Each type of document attached in the Appendiz can be attached separately. Each Appendix is then numbered alphabetically (A, B, C ……) and given a title according to its purpose.
5. Jika Lampiran cukup banyak, dapat dibuatkan Daftar Lampiran terdiri sebagaimana penulisan untuk Daftar Gambar ataupun Daftar Tabel.

Daftar Lampiran tersebut kemudian diisikan di belakang Daftar Tabel.
If there are a lot of Appendix, an Appendix List can be made consisting of the writing for the List of Figures or the List of Tables. The Attachment List is then filled in the end of the Table List.

CHAPTER VI. EVALUATION
Untuk mendapatkan hasil evaluasi yang obyektif maka penilaian mata kuliah KP terdiri dari:
Nilai Pembimbing Perusahaan (40%) Nilai Dosen Pembimbing (30 %) Nilai Seminar (30 %)
To get an objective evaluation result, the practical work course assessment consists of:
Score of Company Advisors (40%)
Supervisor Score (30%) Seminar Score (30%)
Aspek-aspek yang perlu dinilai oleh perusahaan/instansi tempat KP telah ditentukan seperti pada formulir penilaian yang disediakan Departemen Teknik Elektro atau menurut persepsi perusahaan/instansi tempat KP itu sendiri.
Aspects that need to be assessed by the company/agency where the practical workhas been determined are as included on the assessment form provided by the Department of Electrical Engineering or according to the perception of the company / agency where the practical work is itself.
Formulir nilai Dosen Pembimbing diberikan setelah mahasiswa menyerahkan buku laporan KP. Penilaian ini mencakup aspek kesungguhan pelaksanaan KP, penguasaan materi dan tata tulis buku laporan.
The Supervisor score form is given after students submit the practical work report book. This assessment includes aspects of seriousness in the implementation of practical work, mastery of the material and the writing of the report book.
Nilai Seminar KP diberikan jika mahasiswa yang bersangkutan telah melaksanakan seminar KP. Dalam penilaian ini lebih ditekankan pada penguasaan materi KP. Seminar KP dilaksanakan sekali dalam setiap semester (setelah UAS). Pendaftaran seminar KP dilaksanakan di bag. Administrasi KP JTE, sedang jadwal pelaksanaannya akan diatur sendiri oleh seksi KP Bidang Studinya masing-masing.
The score of the practical work seminar is given if the student has finished a practical work seminar. In this assessment, the understanding of

practical work material is more emphasized. practical work seminars are held once in every semester (after final term test). The registration for the practical work seminar can be done through Administration of JTE KP, while the schedule will be regulated by each practical work section of the field of study.
Nilai akhir mata kuliah KP akan diumumkan pada tiap akhir semester setelah ketiga unsur nilai lengkap.
The final score for the practical work will be announced at the end of each semester after the three scores are complete.
Dalam kondisi khusus mahasiswa yang hendak yudisium dan tidak berkesempatan melakukan seminar KP, maka diijinkan menggunakan hanya dua komponen nilai saja. Dalam hal ini nilai akhir maksimal yang mungkin adalah 70 (Nilai Angka) atau B (Nilai Huruf).
In special conditions, students who want to graduate and do not have the opportunity to do practical work seminars, are allowed to use only two scores. In this case the maximum possible final score is 70 (Number Value) or B (Letter Value).

BAB VII. SANKSI-SANKSI
CHAPTER VII. PENALTIES
Berikut adalah sangsi sangsi yang akan diberikan jika,
Here are the sanctions that will be given if,
1. Mahasiswa yang tidak melaksanakan KP padahal perusahaan/ instansi yang dituju telah mengirim jawaban penerimaan, tidak boleh mengajukan permohonan KP baru dalam semester yang sama. Ketentuan ini tidak berlaku apabila mahasiswa tersebut telah memenuhi persyaratan pembatalan yang disetujui Sekretaris Program Studi Sarjana bidang Tugas Akhir, Kerja Praktek, dan Kerjasama DTE.
Students who do not done their practical workeven though the intended company / agency has sent an acceptance response, may not apply for a new practical work in the same semester. This does not apply if the student has met the cancellation requirements approved by the Secretary of the Undergraduate Study Program in the field of Final Project, Job Training, and DTE Collaboration.
2. Surat balasan penerimaan dari perusahaan/instansi tempat KP yang terlambat sampai di JTE melewati batas akhir tanggal pelaksanaan KP (berdasarkan Surat Pengajuan) tidak dibenarkan. Dalam hal ini pelaksanaan KP dianggap batal. Mahasiswa yang bersangkutan dipersilahkan mengajukan KP baru.
If the response letter from the company / agency where the practical work is arrived late at JTE and past the deadline of the practical workdate (based on the submission letter), then it is not justified. In this case the implementation of the practical work is canceled. The student is welcome to apply for a new practical work.
3. Mahasiswa yang memiliki kasus dalam aktifitas KP sebelumnya, tidak diijinkan mengajukan usulan KP baru, sebelum menuntaskan kasus tersebut.
Students who have issues in previous practical work activities are not allowed to submit proposals for new practical works, before solving the issues.

4. Penyerahan buku laporan yang melebihi 3 bulan (setelah KP berakhir) dengan alasan apapun tak bisa diterima (ditolak), dan dipersilahkan mengajukan usulan KP baru dengan topik yang berbeda.
Submission of report books that are more than 3 months (after the practical work ends) for any reason cannot be accepted (rejected), and students are welcome to submit new practical work proposals with different topics.
5. Mahasiswa yang belum mengikuti seminar KP dalam semester yang sama dengan pelaksanaan KP, diberi kesempatan paling lambat pada semester berikutnya.
Students who have not attended the practical work seminar in the same semester as the practical work implementation, are given the opportunity no later than the next semester.
6. Lama waktu yang diperlukan untuk melakukan seluruh aktifitas sebuah KP maksimal 2 semester. Jika setelah 2 semester proses KP belum selesai, mahasiswa dianggap tidak lulus mata kuliah KP. Mahasiswa dipersilahkan mengajukan usulan KP baru dengan topik yang berbeda.
The duration of time to complete all practical work activities is a maximum of 2 semesters. If after 2 semesters the practical work process has not been completed, the student is deemed not to have passed the practical work course. Students are welcome to submit proposals for new practical work with different topics.
7. Mahasiswa yang terbukti melakukan kecurangan dalam bentuk apapun ketika melakukan aktifitas KP, maka KPnya dianggal batal. Mahasiswa yang bersangkutan dipersilahkan mengajukan usulan KP baru dengan topik yang berbeda.
Students who are proven to have cheated in any form during the Practical Work activities, their practical works are canceled. The student is welcome to submit a proposal for a new practical work with a different topic.
8. Nilai mata kuliah KP untuk mahasiswa yang akan yudisium dapat dikeluarkan tanpa harus menuggu berakhirnya semester tersebut. Dalam hal ini tidak diadakan seminar susulan bagi mahasiswa yang akan lulus (Yudisium).
The value of practical work courses for students who will be graduated can be issued without having to wait for the end of the semester. In this

case there is no follow-up seminar for students who will graduate (Yudisium).
9. Segala hal yang belum tertuang dalam peraturan ini akan diserahkan keputusannya pada Pimpinan Departemen Teknik Elektro FTEIC-ITS dengan tetap mempertimbangkan masukan-masukan dari pihak terkait.
Everything that has not been stated in this regulation will be decided by the Head of the Electrical Engineering Department of FTEIC-ITS while still considering input from related parties.
