Final submission of Forms R6 and associated materials
Purpose
To inform schools about arrangements for the return of certification materials: Forms R6 and R7.
Background
At the end of each year, schools provide the Queensland Curriculum and Assessment Authority (QCAA) with data for the preparation of:
• Queensland Certificates of Education (QCE)
• Queensland Certificates of Individual Achievement (QCIA)
• Senior Statements
• Tertiary Entrance Statements.
This memorandum establishes in what form the various pieces of data must be sent to the QCAA and how that transmission is to occur.
Issues
Schools should submit the files via the QCAA’s SDCS website: www.qcaa.qld.edu.au/647.html.
Schools are required to send Forms R6 for all Authority subjects with an exiting cohort and any associated Forms R7.
Schools are not required to send the Forms R12 for Authority-registered subjects. All Forms R12 are electronically stored at the QCAA following moderation meetings. Schools are to retain copies of their Forms R12 for internal and external review processes.

What you need to do
1. Upload SDCS data submission 4 and 5 via the website: www.qcaa.qld.edu.au/647.html.
2. Send hard copy materials to the QCAA using the QCAA-supplied Express Post envelope as outlined in the table below.

Data submission 4 12 pm, Monday 23 November 2015 • subject summary reports
• class list reports for each small and intermediate Authority subject-group
• Forms R6 and any relevant Forms R7 for small and intermediate Authority subject-groups with class lists attached.
Data submission 5 10 am, Thursday 26 November 2015 • subject summary reports
• batch validation
• Forms R6 and any relevant Forms R7 for large Authority subject-groups.
Express Post envelopes can be posted:
• over the counter at a post office or
• in the gold Express Post street boxes (i.e. do not post in the red street postboxes).
Alternatively, the QCAA reception accepts personal deliveries between 8:30 am and 4:30 pm and is located on Level 7 at 154 Melbourne Street, South Brisbane.
Note: The timelines for data processing are extremely tight and we ask that schools comply with the due dates. Schools that fail to meet the due dates may not have their students’ results processed by the dates advertised on the QCAA’s Senior Education Profile (SEP) calendar:
