Final submission of Forms R6 and associated materials

Purpose

To inform schools about arrangements for the return of certification materials: Forms R6 and Forms R7.

Background

At the end of each year, schools provide the QCAA with data for the preparation of:

• Queensland Certificate of Education (QCE)

• Queensland Certificates of Individual Achievement (QCIA)

• Senior Statements

• Tertiary Entrance Statements.

Schools are required to:

• send Forms R6 for all Authority subjects with an exiting cohort, and any associated Forms R7

• submit the files via the QCAA’s SDCS website www.qcaa.qld.edu.au/senior/sdcs.

Schools are not required to send Forms R12 for Authority-registered subjects, but must retain copies of the Forms R12 for internal and external review processes.

What you need to do

1. Send hard copy materials to the QCAA in the supplied Express Post envelope as outlined in the table on the next page.

2. Upload SDCS data submission 4 and 5 via the QCAA SDCS website at www.qcaa.qld.edu.au/senior/sdcs by the due dates.
