Several points may call for discussion — (i.) the time of getting the patient out of bed, (ii.) the use of caustic sublimate in ophthalmia neonatorum. Room. - The patient should be placed in a good-sized room that is light and airy, in the quietest part of the house, preferably with a southern aspect. If the patient complains that her feet are cold, a hot-water bottle well covered with a flannel wrapper may be placed in bed.

Mushroom. - Clean the patient in the morning and in the evening with a sponge, rub the back and dependent parts well with methyl alcohol or cologne. No utensils for eating or drinking should be left in the room, but should be taken away on a tray as soon as the patient eats. The utmost cleanliness must be exercised; the vulva and surrounding areas should be thoroughly cleansed of lochial discharge.

Breasts.-The breasts should be thoroughly washed with soap and water twice a day when sponging the patient, and before and after feeding with sterile water and boracic lotion. Temperature and Pulse.-The temperature and pulse should be taken morning and evening just before the patient is sponged and at the same time every day.

Intestines. The intestines should be opened by a specialist towards the end of the second day after birth. Urine. – A few hours after birth, the mother should place the baby on a heated pan and encourage her to urinate. The height of the fundus above the symphysis should be measured from the third day.

If involution is slow, it may be beneficial to raise the head of the bed, and this may dislodge and openings may he ordered, as this tends to contract the uterus and any fragments of the placenta or membrane which may be in the uterus can be felt, ejected. Bed.—The cot should be of the openwork variety, so that the fresh air can enter on all sides. A hot water bottle that is very well protected with a flannel cover and placed in the cot may be necessary for the warmth of the child.

If he has not passed urine within the first twenty-four hours, examine him thoroughly, as a plug of vernix caseoso may be in the urethral meatus. Bowels.—The bowels of a new-born infant should be open within 12 hours after birth; if they fail to do so, gentle massage of the anus may act as a stimulus, or castor oil 5i.

Stool - Abnormality. - The presence of curds in the stool of a nursing child indicates that the milk is too strong, and sterile water, a few drachms, or barley water, given directly before feeding, is indicated. Clean the baby by wiping the vernix caseosa with wool swabs dipped in warm olive oil, taking special care to wipe between the various skin folds where it is most likely to collect. The bath should be repeated with the exception of the anointing every morning and at the same time.

After using the sublimated abrasive solution, it is necessary to thoroughly wash off all traces of the solution with a 2 percent boric acid solution. Food.—The breast is the ideal food for the child, and in a normal mother no other should be considered. For the first three days a creamy fluid called colostrum is in the breast; it is considered to have a mild laxative effect, cleansing the intestine of rneconium.

Since the capacity of the child's stomach will be about 31V2, the nurse must consider - the amount probably received from the breast. Crying.-If baby cries, it is probably cold, hungry, needs to be changed, or is in pain.

In a recent number of the Speculum, objection was made to the neglect of Psychology by the Medical Faculty. You will find no mention of the no less important factor, the maintenance of the patients in good spirits and hope. Of the functions of the body, some are completely automatic, and do not at any time require the intervention of the higher cerebral centers.

On other functions, such as breathing, the action of the will is effective, but merely incidental. For example, an emotion can increase or decrease the rate of the heart beat, can constrict or dilate the arterioles of the skin, can cause vomiting or diarrhea, can suspend breathing for a moment. In some a disturbance of the mind may disturb the bodily functions, and this is true not only in an acute disturbance as above, but also in a chronic disturbance, — such as constant anxiety.

What a relief comes to the patient's mind when she hears that she is not liable to sudden death, and that she will live many years, and will be well informed of the onset of danger. You should inquire about his ideas about his illness, about his moral, social, and financial history, and note the effect of events on its symptoms.

To my astonishment, no mention was made of what is known to our French brethren as the false gastropath, viz. the psychoneurotic, whose symptoms can accurately imitate a peptic ulcer or any other organic stomach lesion. Experience has shown that even in the presence of such an exact copy of an organic syndrome, the greater probability in such a case is that the disorder is a pure neurosis and deserves treatment first on the basis of this theory. From my own experience, I would say that with a moderate estimate 25 per cent. of the patients are pure psychical neurotics, and the greater part of the rest have some psychical neurosis superimposed as an organic lesion.

An inquiry as to whether the onset of symptoms coincided with any significant or strongly felt change in the patient's moral, social, financial, spiritual or emotional existence. Considering the possibility of such a diagnosis should, in my opinion, be a routine method for every student and doctor. You will find that those who suffer most from it are often excellent people, who will become grateful and devoted patients to the man who makes an effort to understand them and show them sympathy and help.
