First of all we express our sincere gratitude and appreciation to almighty God for his divine blessing which enables us to complete our final year project/internship successfully. Syed Akhter Hossain, Head of CSE Department for his kind help in completing our project and other faculty members and staff of CSE Department of Daffodil International University. We would like to thank our entire course staff at Daffodil International University who participated in this discussion while completing the course.

Prompt patient service is very important for patient treatment and sometimes a matter of saving a critical patient. On the other hand, the attending physician or nurses also do not immediately identify critical patients. We provide an instant online patient service where a hospitalized patient can call the attending physician and nurses on duty anytime from a mobile, Tab or any Smart Screen provided by the hospital authority or the patient's personal device.

Doctors and nurses also immediately receive the call information and visit the patient's room and can take the necessary steps more quickly. In this way, our system can guarantee excellent service to the patient with very little effort.

When the doctor prescribes a patient, you can view the patient's prescription via the high-tech patient service. The doctor sees the patient and the doctor prescribes the diagnostic test for the diagnosis of the patient's disease. If you would like to see the Patient Diagnostic Report, you can easily view it through High-Tech Patient Services.

With the help of the Patient Service application, the doctor can be aware of the patient's condition from his room. The doctor can see the depression report about the patient's condition through this application. The nurse always monitors the patient and reports the progress of the patient's physical condition.

According to the patient's 24-hour observation list, the patient will be provided with the services. To diagnose the disease, the diagnostic test will be performed by practicing the patient's condition.

And there are 7 activities: call a doctor, call a nurse, view a prescription, view a diagnosis, view a report, view a nutrition plan, view bills and view patient observation. For example, a patient can call a doctor or nurses, but he/she can only view the prescription if a doctor prescribes something through the system. Likewise, a nurse can only view the prescription if the doctor writes something into the system.

ER modeling is a data modeling technique used in software engineering to produce a conceptual data model of an information system. Diagrams created using this ER modeling technique are called Entity-Relationship Diagrams or ER Diagrams or ERDs. His original paper on ER modeling is one of the most cited papers in the computer software field.

Currently, the ER model serves as the foundation of many system analysis and design methodologies, computer-aided software engineering (CASE) tools, and warehouse systems. Database server that consists of a computer program and provides database services to other computers; mainly depend on the client-server model. Database server performs tasks like data analysis, storage, data manipulation, archiving, backup and recovery as well.

First of all, if someone goes to our application, he/she can see the login page first. But in order to put any information into our application, he/she has to adjust. To complete this task, a general blank form must be filled out with user information, user ID and password.

A user can modify the entered data or information about it later with the help of administration. A doctor can view patient call list and can create prescription and diet plan for any patient. A nurse can view Nurse Call List and can create Diagnosis Report, Bills and Nurse Observation Report for any patient.

He/she can also make Recipes and Diet Plan. Figure 5.5.1 below was the Nurses Portal. A patient can call the doctor and nurse at any time and can view the prescription and diet plan, diagnosis report, bills and nurse observation report for each patient. We implement all interactions like Add a medicine information, Add patient problems, Add pharmacy information, Problem based medicine search, Select pharmacy, pharmacy based medicine search, Select medicine, apply for order , confirm the order via email to the patient accordingly.

In our system, the patient can: Call the doctor, Call the nurse, View the prescription, View the diagnosis report, View the nutrition table, View the bills and View the patient observation. In our system, the doctor can do: prescription, diet chart, view the call list and call the nurse, view the diagnosis report and view the patient observation report. In our system, the nurse can do: view the call list, patient observation report, enter the diagnosis report, enter the bill and call the doctor.
