Saya yang bernama Halima Eriyani Hrp adalah mahasiswa Program Studi Ilmu
Keperawatan Universitas Sumatera Utara. Saat ini saya sedang melakukan
penelitian tentang “Karakteristik pasien fraktur memilih pengobatan tradisional
didukun patah Suliah di kelurahan Titipapan”. Penelitian ini bertujuan untuk
mengetahuiKarakteristik pasien berdasarkan demografi yang meliputi Usia, Suku,
Agama, Pendidikan, Pekerjaan, penghasilan, sumber informasi, jenis fraktur/patah
tulang yang dialami, alasan memilih pengobatan tradisional patah tulang didukun
patah suliah kelurahan titipapan
Untuk keperluan tersebut saya mohon kesedian anda untuk menjadi menjadi
responden dalam penelitian ini. Jika anda bersedia silahkan menandatangani
lembar persetujuan ini dengan sukarela dan saya persilahkan untuk mengisi
kuesioner dengan jujur dan sesuai dengan apa yang anda alami.
