LEMBAR PENJELASAN KEPADA CALON SUBJEK PENELITIAN

Selamat pagi adik - adik, pada hari ini, saya dr.Vera Avliwani akan

melakukan penelitian yang berjudul “ Perbandingan Derajat Miopia pada

Murid SMA di Kota dengan Murid SMA di Desa Tahun 2014

”. Penelitian

ini untuk menilai perbandingan derajat miopia pada murid SMA

berdasarkan jenis kelamin, riwayat anggota keluarga yang berkaca mata,

mulai berkaca mata, waktu lamanya membaca, menonton TV,

menggunakan komputer dan menggunakan handpone.Untuk menegakkan

derajat miopia dengan menggunakan alat snellen chart.

Pada adik – adik yang bersedia mengikuti penelitian ini nantinya akan

diharuskan mengisi surat persetujuan ikut dalam penelitian dan mengisi

kuisioner.

Segala biaya pemeriksaan ditanggung oleh peneliti dan tidak dibebankan

pada pasien. Bila masih terdapat pertanyaan, maka adik - adik dapat
