1.0 PURPOSE / TUJUAN
The purpose is establishing the requirements for Subcontractor for working at PTMI Batam Facility.
Tujuan dari instruksi kerja ini adalah menetapkan persyaratan bagi Subkontraktor untuk bekerja di fasilitas PTMI Batam.
2.0 SCOPE / RUANG LINGKUP
This work instruction applies to Local and Non Local Subcontractor who work on the premises of PTMI Batam Facility.
Instruksi kerja ini berlaku untuk Subkontraktor Lokal dan Non Lokal yang bekerja di fasilitas PTMI Batam.

Subcontract - a subcontract is an agreement between JRM and a third party to provide a specific item of work which includes performing actual labor at our facilities, the owner’s facilities, offshore or other sites.
Subkontrak - subkontrak adalah perjanjian antara JRM dan pihak ketiga untuk menyediakan spesifikasi pekerjaan yang meliputi penyediaan tenaga kerja yang ada di fasilitas kita, fasilitas pemilik, offshore atau tempat-tempat lain.
6.0 WORK INSTRUCTION DETAILS / RINCIAN INSTRUKSI KERJA
6.1. Subcontractor shall ensure that their personnel are aware of and comply with all HSES aspects defined within their contract with JRM. This may include, but is not limited to :
• JRM Global Safe Work Practices Manual, HSE 003
• Hazard Controls identified in the Subcontractor Risk Assessment • Subcontractor HSES Requirements
• Subcontractor’s Bridging Plan
Subkontraktor harus memastikan bahwa personil mereka menyadari dan memenuhi semua aspek HSES yang ditetapkan dalam kontrak mereka dengan JRM. Ini meliputi, tetapi tidak terbatas pada :
• Manual JRM Global ; Praktek Kerja Aman, HSE 003
• Kontrol Bahaya yang diidentifikasi pada Penilaian Resiko bagi Subkontraktor • Persyaratan HSES bagi Subkontraktor
• Rencana Penghubung bagi Subkontraktor
6.2. Subcontractor shall provide a safe workplace and shall otherwise take all precautions for the HSES of Subordinate Parties and persons and property in or near the premises where work is being performed.
Subkontraktor harus menyediakan tempat kerja yang aman dan sebaliknya harus mengambil tindakan pencegahan untuk HSES dari bawahannya dan orang-orang dan barang-barang yang ada atau yang berdekatan dengan lingkungan dimana pekerjaan tersebut dilakukan.
6.3. Subcontractor shall self-inspect its areas of control to assure compliance with the HSES requirements.
Subkontraktor harus mengadakan kontrol dan inspeksi sendiri di areanya untuk memastikan pelaksanaannya sesuai dengan persyaratan HSES.
6.4. Subcontractor shall implement the JRM policies and procedures during working in JRM facility.
Subkontraktor harus mengimplementasikan kebijakan-kebijakan dan prosedur-prosedur JRM selama bekerja di fasilitas JRM.
6.5. Subcontractor shall meet JRM medical requirements :
Subkontraktor harus memenuhi semua persyaratan medis JRM :
6.5.1. Subcontractor shall comply with all relevant local laws and regulations with respect to occupational safety of and medical services for its employees. Subkontraktor harus mematuhi semua hukum dan peraturan lokal yang terkait dengan keselamatan dan kesehatan kerja terhadap karyawannya. 6.5.2. Pursuant to the above, Subcontractor shall ensure that all its employees have
been examined by a registered medical practitioner, clinic or hospital and found fit for work in accordance with the relevant regulations. If so requested by JRM, Subcontractor shall submit a medical certificate evidencing the same to JRM’s Subcontract Administrator or his designee.
Terkait dengan poin di atas, Subkontraktor harus memastikan bahwa semua karyawannya telah menjalani pemeriksaan medis oleh dokter yang terdaftar, oleh klinik atau rumah sakit dan dinyatakan sehat untuk bekerja sesuai dengan peraturan yang berlaku. Jika diminta oleh JRM, Subkontraktor harus menyerahkan sertifikat medis sebagai bukti yang sama kepada JRM Subkontraktor Administrator atau orang yang menerima delegasinya.
6.6. Subcontractor shall meet JRM Security requirements :
6.6.1. SKCK/SKKB (Good Attitude Police Clearance) / Surat Keterangan Catatan Kepolisian/Surat Keterangan Kelakuan Baik
6.6.1.1. SKCK/SKKB (Good Attitude Police Clearance) is a reference letter issued by Police which stated that the personnel have a good attitude and is not related with criminal cases. Every personnel must have the SKCK as a requirement for working at PTMI Facility.
SKCK/SKKB adalah surat keterangan yang dikeluarkan oleh Kepolisian yang menyatakan bahwa personil yang bersangkutan berkelakuan baik dan tidak tersangkut dengan masalah kriminal.

Setiap personil harus memiliki SKCK sebagai persyaratan untuk bekerja di fasilitas PTMI
6.6.1.2. Subcontractor’s Manager/Coordinator to submit the original SKCK/SKKB of their personnel which the copy is submitted to the Security along with the safety induction attendance list as requirement to get PTMI ID.
Manajer/Koordinator Subkontraktor menyerahkan SKCK/SKKB yang asli dari para personil mereka dan fotokopinya di serahkan kepada Security bersamaan dengan daftar hadir safety induction sebagai persyaratan untuk mendapatkan kartu identitas PTMI. 6.6.2. Personal Identity / ID card / Personil Identitas/Kartu Identitas
6.6.2.1. Subcontractor’s employees shall posses PTMI identity card prior to entering and commencing the task in the PTMI Facility.
Para karyawan subkontraktor harus memiliki kartu identitas PTMI sebelum memasuki dan memulai tugasnya di fasilitas PTMI. 6.6.2.2. Subcontractor’s Manager/Coordinator should request in writing for
ID card issue for their personnel and agree to be reimbursed or invoice by PTMI if the card lost or not return. PTMI Department Head or Project Management representative who sponsors the subcontractor working in the facility has to sign on the ID request issue form.
Manajer/Koordinator Subkontraktor harus menyampaikan permintaan tertulis untuk mengeluarkan kartu identitas bagi personil mereka dan setuju untuk di claim atau diberi faktur tagihan oleh PTMI jika kartu hilang atau belum dikembalikan. Departemen Head PTMI atau perwakilan Project Management yang mensponsori subkontraktor yang bekerja di fasilitas ini harus menandatangani formulir permintaan untuk mengeluarkan kartu identitas.
6.6.2.3. All ID cards issued to subcontractor’s employees shall be returned to Security office at least a day before they leave the facility or contract completion.
Semua kartu identitas yang dikeluarkan kepada para karyawan subkontraktor harus dikembalikan ke kantor Security paling lambat satu hari sebelum mereka meninggalkan fasilitas atau kontrak selesai.
6.6.2.4. Any misuse or violation of the PTMI ID card policy will be the responsibility of subcontractor Manager. The involved person (s) will be subject for disciplinary action until termination or banned for entering the PTMI Facility.
Penyalahgunaan atau pelanggaran kebijakan kartu identitas PTMI akan menjadi tanggung jawab Manajer Subkontraktor. Mereka yang terlibat akan dikenakan sanksi disiplin sampai dengan pemberhentian atau dilarang memasuki fasilitas PTMI.
6.7. Personnel Safety Orientation / Induction / Personil Safety Orientasi/Induksi
6.7.1. The personnel must attend the safety induction conducted by L&OD Department prior to work in the PTMI Facility. The duration and contents of the induction will be in accordance with PTMI existing procedures and requirements. Request to shorten or reduce the duration would not be considered. Participants should come at the specified time and late comers would not be allowed to join the class.
Para personil harus menghadiri safety induction yang diadakan oleh L&OD Department sebelum bekerja di fasilitas PTMI. Jangka waktu dan isi dari induksi akan disesuaikan dengan prosedur dan persyaratan PTMI yang berlaku. Permintaan untuk memperpendek atau mengurangi jangka waktunya tidak dianjurkan. Para peserta harus datang tepat waktu dan bagi yang terlambat datang tidak diizinkan untuk mengikuti kelas tersebut.
6.7.2. Notification or request for safety induction should be submitted to HSE School two days in advance by department head or project management representative who sponsors the use of the subcontractor.
Pemberitahuan atau permintaan untuk safety induction harus diserahkan kepada HSE School dua hari sebelumnya oleh departemen head atau perwakilan project management yang mensponsori pemakaian subkontraktor.
6.7.3. Safety Briefing can be given for short term visit and should be given by the Host such as for visitor, vendor or subcontractor who conducting the work in office only or non critical base work less than a week. In addition of the safety briefing form, any relevant hazard and risk may exposure to the vendor/subcontractor during performing the work should be stressed in the briefing.
Penjelasan singkat tentang safety dapat diberikan untuk kunjungan singkat dan harus diberikan oleh tuan rumah kepada tamu, vendor atau subkontraktor yang melaksanakan pekerjaan di kantor saja atau pekerjaan tidak terlalu mendesak kurang dari satu minggu. Sebagai tambahan pada formulir safety briefing, segala hal yang berhubungan dengan bahaya dan resiko yang bisa terpapar kepada vendor/subkontraktor selama menjalankan pekerjaan harus ditekankan dalam penjelasan singkat tersebut.
6.8. PPE (Personal Protection Equipment) /Perlengkapan Alat Pelindungan Diri
6.8.1 Subcontractor company shall provide PPE to their personnel working in the PTMI Facility and the PPE must meet safety standard.
Perusahaan subkontraktor harus menyediakan APD kepada para personil mereka yang bekerja di fasilitas PTMI dan APD tersebut harus memenuhi standar keselamatan.
6.8.2. Subcontractor’s employees shall wear PPE and comply with the PTMI PPE requirements.
Para karyawan subkontraktor harus memakai APD dan harus sesuai dengan persyaratan APD PTMI.
