Electronic

Medieal Record

Medical record

is

files that coatained document

patient identity, health record, and the other [4]. Formedy,

medical record

md

often failed to

flrlfill

as

caused

many

hardcopy documents, ineffective operetional procedures, md many more[5]. Now, medical data record designed with elecfronics technology such as web or desktop based which make it easier and more effective. It was

Eloofionics Medioal Reoords (EMR).

In

the EMR,

patie,nt data stored as some data type, such as searchable text

some case

it

stored as dosument imag€s [6]. Although

it

reserch explained that generally, information in patient medical record must be contained:

l)

Who is the patient, and who was the person who give medical service. For exmple Made Sukarsa (who), Dr. Arya Sasmita (who give medioal service)

2)

What when, why, and how oedical service applied. For

Database,

E-R Model,

and

Nomalization

This

research was conducted

within the

of

detail design

md

dwolopmeat

of

a

database. Database is collection ofrelaional data [8]. The basic concept

of

the database

is

a

oolleption

of

records,

or

pieces

of

knowledge.

A

sfiuotured database has a description

of

the kinds of fac8 that are stored

in

it

an explmation

is

called schema. Scheme describe

fte

object, representod a database, and the relationships betwwn objects. There

re

mmy ways to organize the schemo, or modeling &e database s*ucture: these

are known as the model databas€ or data model. Models ane

commonly used today

is

the

reldional model.

which

in

laymm terms represent

all &e

information

in

the form

of

tables that are interconnected with each table coosists of rows

ard

columns

(the frue

ddinition

uses

m*hemdical

In

this model, the rclationship betwen tables represenled premises using the same values betweetr tsbles. Other models such as the hie, ruohical model and the network model use

a

more

erylicit

wly

to

repres€nt relsionships between tables. Utilization databases for d*,a management also has other purposes as follows

[9]:

speed and ease, the efficiency

of

storage space (space), accurrcy, availability, completeness, security, usage togethemess (shre ability)

Data models arc commonly used in the

desip

of informcion systems first level, for orample to describe the

ilforrdion

needs aad the type of informatiou to be stored in the database

for analysis. Entity Reldionship Model or E-R Model, knoum as Entity Relationship Diagram @RD)

is

a data model or diagrau for high-level descriptions of conceptual data model.

ERD

providcs

a

graphical nolation

for

rcprcscnting conceptual daa models in the form of an Entity Relationship Diagram. By desiped The E-R nodet, systeo decigner able

real-world enterprises onto a oonceparal schema [6].ERD uses a number notation ard symbols to dessribe ttre stucture and relationship batween dat4 including:

b)

In ERD, there is important phase.

it

is mapping cardinalities

of

entities could be associated via relationship set by mapping

[0].

For example, sets

The process to make a new entity from existing attributes called nonualization. For oxample, {rgtre 2

With sme

normalization process used

ERD,
