Summary of change:

Change the "SPATIAL_REFERENCE_SYSTEMS" table name to

"SPATIAL_REF_SYS" everywhere in 12-128r1 to match the table

name specified in OGC 06-104r4 instead of the table name specified in OGC 99-049.
