The only normative SOAP reference (cl. 3) is for W3C SOAP, "Simple Object Access Protocol (SOAP) 1.2". Indeed, this is the only version standardized by the W3C.

Clause D.4 specifies the incorrect namespace: it should be "http://www.w3.org/2003/05/soap-envelope".
