Pada hari ini Jum’at tanggal Dua puluh satu bulan September tahun Dua ribu dua

belas, Kami Panitia Pengadaan Barang Pengadaan Alat Praktek Laboratorium Untuk

Mahasiswa pada Politeknik Kesehatan Tasikmalaya melaksanakan Perubahan Dokumen

dikarenakan adanya beberapa poin yang membingungkan para peserta pengadaan Alat

Praktek Laboratorium Untuk Mahasiswa pada Politeknik Kesehatan Tasikmalaya.

Perubahan tersebut adalah :

1. Untuk Spesifikasi bertambah poinnya dan isian kolom Quantity jumlahnya sama

dengan yang ada pada BoQ, dengan perubahan terlampir dalam Berita Acara

Addendum ini;

2. Untuk Bill of Quantity (BOQ) nama barang disamakan dengan yang ada pada

Spesifikasi Barang, dengan perubahan terlampir dalam Berita Acara Addendum ini;

3. Untuk Surat menyurat yang berhubungan dengan pekerjaan ini ditujukan kepada

Panitia Pengadaan Barang Politeknik Kesehatan Tasikmalaya.

Perubahan dokumen hanya pada poin yang disebutkan di atas.
