Objective: The aim of the study was to evaluate microleakage in inlay direct composite restorations bonded with two different type of resin cements and a self adhesive resin cement. Methods: 32 extracted human premolars and molars were selected and were prepared 2 standard II class MO and OD for each with mesio-cervical margin located in enamel and the other in dentine and restored with direct composite (CeramX, Dentsply) inlays (light-cured and post-cured). Specimens were divided into 4 groups (n=8): one cemented with RelyX Unicem (REL) (3M ESPE) applied according to manufacturer’s instruction, one with RelyX Unicem (3M ESPE) applied in combination with Scotchbond Multipurpose Bonding System (SBM) (3M ESPE), one cemented with RelyX ARC (ARC) (3M ESPE) and the last with Filtek Supreme (FIL) (3M ESPE). Specimens were then thermo-cycled (5°C-55°, 30s, 500 cycles), immersed in 2% blue methylene (24h, 37°C). and leakage assessed using standardized scale. Sections were evaluated with stereoscopic microscope and results statistical analysed (Modified Wilcoxon-Mann-Whitney, Kruskal-Wallis tests). Results: All cements applied in combination with a bonding agent revealed the highest leakage at the dentine margins. REL showed no statistically differences between enamel or dentin margins and developed the highest leakage score in both dentine and enamel compared to the other cements; only dye penetration in enamel was statistically different from other groups. The use of SBM improved marginal seal. Resin cements with high viscosity performed a better marginal seal than the low viscosity cements in both enamel and dentine. Conclusion: The self adhesive resin cement RelyX Unicem used for composite inlay placement revealed a poor marginal seal when compared to resin cements that require bonding system.
