PERAN VAKSIN PADA PENCEGAHAN INFEKSI VIRUS

Infeksi virus dapat terjadi setelah virus berhasil merusak barier pertahanan tubuh dan menginvasi sistem imun inang. Respon imun inang terhadap virus terbagi menjadi 2 kategori utama: tidak spesifik (terutama interferon) dan spesifik (termasuk respon imun humoral dan cell-mediated immunity). Gambaran klinik infeksi virus tergantung pada interaksi antara virus-inang dan respon imun inang terhadap infeksinya. Berat ringannya penyakit yang timbul dapat ditentukan oleh beberapa parameter yang dimiliki virus dan inang, termasuk strain virusnya, jumlah virus yang masuk dan keadaan umum individu yang terinfeksi.
Terapi untuk infeksi virus termasuk penggunaan obat-obat anti virus yang terbukti kurang efektif karena virus menggunakan sistem metabolik inangnya dan karenanya obat antivirus akan bersifat anti-sel.
Pencegahan terhadap infeksi virus dapat dicapai dengan imunisasi menggunakan vaksin. Dibandingkan penggunaan obat-obat anti virus, vaksinasi adalah cara yang paling efektif dan ekonomis untuk mencegah infeksi virus yang serius. Terbukti bahwa beberapa vaksin berhasil memberikan imunitas protektif terhadap beberapa patogen dan sangat efektif dalam mengurangi insidensi infeksi virus yang terjadi setiap tahun. Vaksin adalah molekul-molekul, biasanya berbentuk protein, yang dapat menstimulasi respon imun. Pemahaman yang baik mengenai respon imun terhadap infeksi virus akan memfasilitasi penggunaan vaksin dengan lebih baik.

Viral infections may occur after the breakage of the natural protective barriers of the body and invasion of the host immune system. The host's immune respons against viruses fall into two major categories: nonspesifik (which the most important are interferons) and specific (including both humoral and cell-mediated immunity). The outcome of a viral infection is determined by the nature of the virus-host interaction and the host's response to the infection. The severity of disease may be determined by many viral and host parameters, including the strain of virus, the inoculum size and the general health of the infected individual.
Treatments for viral infections include the usage of antiviral drugs which have proved of little use therapeutically since the virus uses host-cell metabolic reactions and thus, the most part, anti-cell agents.
Prevention of viral infection can be achieved by immunizations using vaccines. Compare to the treatments using antiviral drugs, vaccination is the most cost-effective method of prevention of serious viral infections. This was shown as several vaccines have proved to provide protective immunity against several potential pathogens and remarkably effective at reducing the annual incidence of viral disease. Vaccine are molecules, usually but not necessarily proteins, that can elicit immune response. Good understanding of the immune response against viral infection will facilitate a better vaccine management.

PENDAHULUAN
1.1 Latar Belakang
Pen yak it yang disebakan oleh VIruS dewasa ini semakin bertambah. Sebagai
contoh: virus adalah penyebab utama gastroenteritis di negara-negara industri, dan
dari 36,8 juta kasus yang dilaporkan di Amerika Serikat; 30,8 juta (80%) kasus
disebabkan oleh virus seperti Rotavirus, Astrovirus, adenovirus, dan Sapovirus (Lopman, Reacher, van Duijnhoven, Hanon, Brown, Koopmans, 2003). Di Amerika
Serikat dan negara berkembang lainnya, infeksi rabies dilaporkan masih terdapat
dalam populasi manusia dan menurut data dari Centers for Disease Control and
Prevention (CDC) pada tahun 2000, terdapat 93% kasus rabies yang disebabkan oleh
hewan liar (Centers for Disease Control and Prevention, 2003). Tingginya angka
morbiditas dan mortalitas infeksi virus ini salah satunya disebabkan karena kesulitan
dalam memberikan terapi. Selain itu, gejala klinik infeksi virus pada stadium awal
sangat tidak spesifik sehingga diagnosis sebelum terjadinya infeksi stadium lanjut
menjadi sulit. Alasan lainnya adalah karena ban yak infeksi virus yang bersifat lethal
dan ada beberapa yang dapat menginfeksi secara laten serta menimbulkan kerusakan
jaringan yang terus berlanjut (misalnya: virus Hepatitis B dapat menyebabkan
cirrhosis hepatis).
Pada infeksi oleh bakteri, terapi yang diberikan lebih sederhana dibandingkan
dengan terapi infeksi oleh virus, yaitu dengan pemberian antibiotika. Pada infeksi
virus, terapi yang dapat diberikan sering kali lebih kompleks. Beberapa obat anti
virus memiliki efek samping yang bersifat sitotoksik (toksik terhadap sel) dengan

tersedia obat anti virusnya. Oleh karenanya, penggunaan vaksin menjadi langkah
yang lebih baik untuk diterapkan sebagai strategi pencegahan infeksi virus.
Beberapa jenis vaksin yang sudah tersedia, misalnya : vaksin Polio, vaksin MMR
(Measles Mumps Rubella), vaksin Varicella, dsb. Seiring dengan berkembangnya ilmu pengetahuan, telah ditemukan beberapa virus baru yang dapat menimbulkan
infeksi dan belum tersedia atau sedang dikembangkan pengobatannya. Pemahaman
lebih lanjut mengenai cara kerja vaksin menjadi sangat penting dalam upaya
meningkatkan effisiensi vaksin yang sudah tersedia serta usaha pengembangan vaksin
yang baru.
1.2 Identifikasi Masalah

Vaksin apa saja yang telah tersedia untuk penanggulangan infeksi ViruS ?Bagaimana peranannya dalam mencegah infeksi virus yang sesungguhnya ?
1.3 Maksud dan Tujuan
Maksud: mengetahui peran vaksin dalam pencegahan infeksi virus.
Tujuan:
1. Ingin mendalami patogenesa infeksi virus dan interaksi an tara virus dengan
sistem kekebalan tubuh inang
2. Mendapatkan manfaat yang optimal dari pemberian vaksin (khususnya
terhadap infeksi virus tertentu)

4.1 Kesimpulan
Meningkatnya angka kesakitan (morbiditas) dan angka kematian (mortalitas)
akibat infeksi virus banyak disebabkan karena terlambatnya diagnosis, meluasnya
penularan diantara individu dan belum tersedianya obat antivirus yang efektif dalam
pengobatan infeksi virus. Infeksi virus dapat terjadi setelah virus menempel pada
reseplor spesifik sel inang lalu terjadi invasi ke sel inang. Pada umumnya virus dapat
masuk ke dalam tubuh melalui: saluran pemafasan, saluran pencemaan, saluran
urogenital, mata dan kulit. Respon imun inang terhadap infeksi virus terbagi menjadi
respon imun innate dan respon imun adaptive. Salah satu usaha untuk mencegah
infeksi virus adalah dengan vaksinasi/imunisasi. Dengan vaksinasi, maka individu
akan terpapar oleh antigen virus yang terdapat dalam vaksin, sehingga timbul respon
imun dan dibentuknya antibodi terhadap virus-virus tertentu. Antibodi yang dibentuk
diharapkan dapat memberikan proteksi terhadap infeksi yang sesungguhnya atau
walaupun sakit, gejala yang timbul tidak akan berat dan membahayakan jiwa.
Beberapa jenis vaksin yang telah tersedia diantaranya: vaksin live attenuated, vaksin
killed/inactivated, vaksin sub-unit dan vaksin rekombinan. Pemahaman mengenai infeksi virus serta respon imun yang terjadi akan dapat meningkatkan efisiensi
penggunaan vaksin.
4.2 Saran
Vaksinasi merupakan cara yang tepat untuk mencegah, menurunkan angka
morbiditas dan mortalitas akibat infeksi virus. Dengan vaksin, beberapa infeksi virus
sudah berhasil dieliminasi dari populasi manusia. Dengan pemahaman yang lebih

baik mengenm respon lmun yang terjadi dan pemahaman mengenai vaksin,
diharapkan berbagai infeksi virus dapat dicegah, perlu dikembangkan penelitian
terhadap berbagai vaksin baru dan cara produksinya untuk memperkecil/mengurangi
