The next section is assess the health and safety aspect and impact. This section is similar with the previous, the differences occurs in the condition of the impact.
The first step of this section is listing the health and safety potential hazard on the process or activity. The next step is analyze the risk of the process to the health and safety. The additional step in this section is classify the hazard into safety or health aspect.
The next is determine the condition when the impact is happens. In this section, there are 2 impact condition; Routine or Non-routine. Routine condition is the condition if the hazard coming from routine activity or process. Non-routine is the condition if the hazard coming from non-routine activity or process.
The next is give the probability score and severity score to determine Initial Risk Factor Value. After that list the existing control measurement. Existing control measurement in the existing activity or process or tools to control the hazard.
After that give the residual risk score after the hazard was controlled. Residual risk score is the score after the hazard was controlled. The score is similar with the initial risk level which are probability and severity. From residual risk, the risk level could be determined. The risk level represent the priority of the improvement that must be made.
The table above shows the result of AI-HIRA health and safety assessment. There are 7 activities that resulting environmental impact. The first is manual rooting activity, there are 3 potential hazards related with health and safety.
The first is stitched by needle, it classifies as safety aspect. The impact from this hazard is cut, laceration, and puncture injury. It was a hazard from routine activity, so the condition is Routine with the probability score is 6 and the severity score is 2, so the total risk factor value is 12. The existing control is finger protection & work instruction. The controlling method does not affecting that much in reducing the probability score. The probability score only reduce to 4 and the severity score still 2. The residual risk factor value from this process is 8 or moderate.
The second is not ergonomic sitting position, it classifies as health aspect. The impact from this hazard is low-back pain injury. It was a hazard from routine activity, so the condition is Routine with the probability score is 2 and the severity
score is 4, so the total risk factor value is 8. The probability is small because the impact is happens in a long term period of working, but when it happens it has high severity. There is still no existing control measurement, so there is no changes in risk factor value. The probability score still 2 and the severity score still 4. The residual risk factor value from this process is 8 or moderate.
The third is Machine's oil spills, it classifies as safety aspect. The impact from this hazard is slippery floor. It was a hazard from non-routine activity, so the condition is non-routine with the probability score is 5 and the severity score is 1, so the total risk factor value is 5. There is cup for spills oil container as controlling measurement. The probability score reduce to 3 and the severity score still 1. The residual risk factor value from this process is 3 or acceptable.
The next is gluing head process. In this process there are 2 aspects related to safety and health aspects. The first is skin contact with hot surface/ machine, it classifies as safety aspect. The impact from this hazard is burnt skin. It was a hazard from routine activity, so the condition is routine with the probability score is 5 and the severity score is 2, so the total risk factor value is 10. There are two existing control measurements, the first is provide acrylic cover & warning sign, and the second is modify process by adding manual toggle to ensure glue properly spread inside rot head. The probability score reduce to 3 and the severity score still 2. The residual risk factor value from this process is 6 or moderate.
The second is hot melt adhesive glue contact with skin, it classifies as safety aspect. The impact from this hazard is also burnt skin. It was a hazard from routine activity, so the condition is routine with the probability score is 6 and the severity score is 2, so the total risk factor value is 12. There are two existing control measurements, the first is provide timer to limit amount of air pressure, temperature controller and platform, and the second is provide additional toggle create less hazard rather than manual squeezing by operator hand. The probability score reduce to 4 and the severity score still 2. The residual risk factor value from this process is 8 or moderate.
The next process is grooming process. In this process there are 4 aspects related to safety and health aspects. There is no existing measurement, so there is no change is probability score and severity score, so the risk factor value from this process is still 2 or trivial.
The last is repetitive contact between dog brush and hand, it classifies as safety aspect. The impact from this hazard is scratched by dog brush on blowing hair process. It was a hazard from routine activity, so the condition is routine with the probability score is 4 and the severity score is 2, so the total risk factor value is 8.
The existing measurement control is Use proper dog brush, and controlled by process method. The probability score reduce to 2 and the severity score still 2.
The residual risk factor value from this process is 4 or acceptable.
The next is gluing head process. In this process there are 4 aspects related to safety and health aspects, but overall all the aspects will resulting fire impact. The
aspects are; the first is overheating, it was a hazard from non-routine activity, so the condition is non-routine with probability score is 3 and severity score is 3, with MCB, timer, and alarm as measurement control, the probability score becomes 1 and the severity score is still 3 so the risk factor value becomes 3 or acceptable.
The second is broken temperature indicator. It was a hazard from non-routine activity, so the condition is non-routine with probability score is 3 and severity score is 2. With regular maintenance/ calibration as measurement control, the probability score becomes 1 and the severity score is still 2 so the risk factor becomes 2 or trivial.
The third is material left in the oven. It was a hazard from routine activity, so the condition is routine with probability score is 3 and severity score is 2. By controlling the method, the probability score becomes 1 and the severity score is still 2 so the risk factor becomes 2 or trivial.
The last is short circuit, it was a hazard from routine activity, so the condition is routine with probability score is 3 and severity score is 2. By providing MCB as the measurement control, the probability score becomes 1 and the severity score is still 2 so the risk factor becomes 2 or trivial.
The next is blister sealing process. The aspects related to safety and health aspects in this process is pinched by moving part. It was a hazard from routine activity, so the condition is routine with probability score is 3 and severity score is 2. The impact of this aspect is injured finger. By providing acrylic cover on machine as measurement control, the probability score becomes 1 and the severity score is still 2 so the risk factor value becomes 2 or trivial.
The next is oven conveyor process. The aspects related to safety and health aspects in this process is pinched by uncovered moving part. It was a hazard from routine activity, so the condition is routine with probability score is 2 and severity score is 2. The impact of this aspect is injured finger. By providing cover hazardous part on the conveyor as measurement control, the probability score
becomes 1 and the severity score is still 2 so the risk factor value becomes 2 or trivial.
The last is hot sealer machine. The aspects related to safety and health aspects in this process is heat exposure. There are two impacts of this aspect; burnt skin because of the heat, and injured finger because of the moving part. Both of impact is a hazard from routine activity, so the condition for both impacts is routine. The probability score for burnt skin is 2 and severity score is 2, and the probability score for injured finger is 5 and severity score is 2. By providing a sign as measurement control for both impacts. There is no change on probability score and severity score for burnt skin 2 so the risk factor value still 4 or acceptable.
The measurement control changes the probability factor of injured finger becomes 3, but the severity score is still 2, so the risk factor value is 6 or moderate.
