Dokumentasi rekam medis di Rumah Sakit Baptis Batu terdapat beberapa jenis formulir, salah satunya yaitu formulir Asuhan Gizi. Formulir asuhan gizi dibuat untuk memecahkan masalah gizi yang sistematis dengan berpikir kritis dan membuat putusan memberikan kualitas asuhan gizi yang aman dan efektif.
Rancangan penelitian yang digunakan survei deskriptif pendekatan kuantitatif.
Variabel penelitian ini menggunakan independent variable. Tujuan penelitian ini yaitu menganalisis kelengkapan formulir asuhan gizi rawat inap RS Baptis Batu menggunakan tabel analisis kuantitatif dengan 7 kriteria pendokumentasian klinis.
Populasi dalam penelitian ini sebanyak 40 dokumen dari bulan September- Desember 2018 dan menggunakan total sampling yaitu 40 dokumen dari bulan September-Desember 2018. hasil penelitian ini menunjukkan bahwa keakuratan identifikasi pasien sebesar 86%, keakuratan laporan/catatan penting sebesar 86%, keakuratan autentifikasi sebesar 0%, keakuratan pencatatan yang baik sebesar 58%.
Kesimpulannya saat dilakukannya analisis kuantitatif adalah, peneliti menemukan ketidaklengkapan formulir asuhan gizi di Rumah Sakit Baptis Batu mencapai 100%. Mengembangkan Desain Formulir Asuhan Gizi formulir basis web jika bagian pada formulir belum terisi tidak akan bisa ke halaman berikutnya.

Nutrition care forms are made to solve nutritional problems systematically by thinking critically and making decisions providing safe and effective quality of nutrition care. The research design used descriptive quantitative approach survey. This research variable uses independent variable. The purpose of this study was to analyze the completeness of the inpatient nutrition care form at Baptist Stone Hospital using a quantitative analysis table with 7 clinical documentation criteria. The population in this study is 40 documents from September-December 2018 and uses total sampling of 40 documents from September-December 2018. the results of this study indicate that the accuracy of patient identification is 86%, the accuracy of the report / important note is 86%, the accuracy of authentication is 0%, the accuracy of recording is good at 58%. In conclusion, when the quantitative analysis was conducted, the researchers found that the incompleteness of the nutrition care form at Batu Baptist Hospital reached 100%. Develop a Care Form Design Nutrition web base form if the part on the form has not been filled will not be able to the next page .
