Abstrak : Dalam pelayanan kesehatan ketepatan dan keakuratan penulisan diagnosis merupakan faktor penting untuk mendukung pelayanan kesehatan. Ketidaktepatan dan ketidakakuratan penulisan kode diagnosis dapat mengakibatkan terganggunya pelayanan kesehatan, karena dalam era asuransi kesehatan yang masif ini maka klaim biaya rumah sakit dapat terhambat diterima oleh rumah sakit. Penelitian ini bertujuan untuk mengetahui ketepatan penulisan diagnosis dengan keakuratan kode diagnosis supaya bisa mengetahui ketepatan penulisan diagnosis dengan keakuratan kode diagnosis Pregnancy, Childbirth And The Puerperium pada rumah Sakit Ibu dan Anak Puri Malang. Pada penelitian ini sampel yang digunakan adalah 68 dokumen rekam medis dan menggunakan Teknik Sampling Jenuh dengan pengambilan populasi sebagai sampel, diperoleh hasil ketepatan penulisan diagnosis sebesar 35.3% dan ketidaktepatan penulisan diagnosis sebesar 64.7% dan diperoleh hasil pada keakuratan kode diagnosis sebesar 44.1% dan ketidakakuratan sebesar 55.9%. Analisis data menggunakan analisis kuantitatif dengan menggunkan metode uji Chi-Square pada Hubungan Ketepatan Penulisan Diagnosis dengan Keakuratan Kode Diagnosis Pregnancy, Childbirth And The Puerperium. Hasil dari analisis uji Chi-Square diketahui nilai p=0.03<0.05 dan didapat hasil H1 diterima dan H0 ditolak yang artinya ada Hubungan antara Ketepatan Penulisan Diagnosis dengan Keakuratan Kode Diagnosis Pregnancy, Childbirth And The Puerperium di Rumah Sakit Ibu dan Anak Puri Malang.

Abstract : Accuracy in writing a diagnosis and accuracy of the diagnosis code is one of the most important supporting factors in health services. In the system of clinical classification and codification of medical recorders the disease is required to carry out clinical classification appropriately for the accuracy of the codification of the disease. This study aims to determine the accuracy of writing a diagnosis with the accuracy of the diagnosis code so that it can know the accuracy of writing a diagnosis with the accuracy of the diagnosis code Pregnancy, Childbirth And The Puerperium at Mother and Child Hospital Puri Malang. In this study the sample used was 68 medical record documents and using the Saturated Sampling Technique with population sampling as a sample, the accuracy of diagnosis writing was 35.3% and the diagnosis writing accuracy was 64.7% and the accuracy of the diagnosis code was 44.1% and the inaccuracy was 55.9%. Data analysis used quantitative analysis using the Chi-Square test method on the Correlation of Accuracy in Writing a Diagnosis with the Accuracy of Pregnancy, Childbirth And The Puerperium Diagnosis Codes. The results of the Chi-Square test analysis revealed that the value of p = 0.03
<0.05 and obtained H1 results were accepted and H0 was rejected, which means there is a relationship between the Accuracy of Writing the Diagnosis with the Accuracy of the Pregnancy, Childbirth And The Puerperium Diagnosis at Puri Malang Mother and Child Hospital.
