Final submission of Forms R6 and associated materials
Purpose
To inform schools about arrangements for the return of certification materials: Forms R6 and R7.
Background
At the end of each year, schools provide QSA with data for the preparation of Queensland Certificates of Education, Queensland Certificates of Individual Achievement, Senior Statements and Tertiary Entrance Statements.
This memorandum establishes in what form the various pieces of data must be sent to the QSA and how that transmission is to occur.
Issues
Schools should submit the files via the QSA’s SDCS website (www.qsa.qld.edu.au/647.html).
Schools are required to send Forms R6 for all Authority subjects for which there is an exiting cohort and any associated Forms R7.
Schools are no longer required to send the Forms R12 for Authority-registered subjects. All Forms R12 are electronically stored at the QSA following Moderation meetings. Schools are to retain these forms for internal and external review processes.

What you need to do
1. Upload SDCS data Submission 4 and 5 via the website
2. Send hard copy materials to the QSA using Express Post satchels
Data submission 4 To the QSA not later than 12 noon, Monday 18 November 2013
Send the following in the Express Post provided:
• subject summary reports
• class list reports for each small and intermediate Authority subject-group
• Forms R6 and any relevant Forms R7 for small and intermediate Authority subject- groups with class lists attached
Data submission 5 To the QSA not later than 10 am, Thursday 21 November 2013
Send the following in the Express Post provided:
• subject summary reports
• batch validation
• Forms R6 and any relevant Forms R7 for large Authority subject-groups

To ensure that all materials (Forms R6 and R7) are returned by the deadline, all schools will receive Australia Post Express Post satchels. These satchels are prepaid at no cost to schools.
How to post Express Post satchels:
• simply post over the counter or
• place in the special gold Express Post street boxes (do not post in red street posting boxes).
The timelines for data processing are extremely tight and we ask schools to ensure that they comply with the due dates. Schools that fail to meet the due dates may not have their students’ results processed by the dates advertised on the QSA’s Senior Education Profile Calendar.
The QSA reception is located on Floor 7 at 154 Melbourne Street, South Brisbane and accepts personal deliveries between 8:30 am and 4:30 pm.
