LEMBAR PENJELASAN KEPADA CALON SUBJEK PENELITIAN

Selamat pagi/siang Bapak/Ibu, pada hari ini saya, dr.Brama Ihsan Sazli,

peserta Pendidikan Pasca Sarjana Ilmu Penyakit Dalam / Magister Klinik FK

USU Medan

akan melakukan penelitian yang berjudul ”

Perbandingan faal

hemostasis pada pasien penyakit jantung koroner dengan dan tanpa DM tipe 2

dengan menggunakan tromboelastografi

”. Dimana faal hemostasis ini merupakan

tingkat proses pembekuan darah bapak.

Anda bebas memilih keikutsertaan dalam penelitian ini tanpa paksaan dari

pihak manapun. Bila Anda sudah memutuskan untuk ikut, Anda juga bebas untuk

mengundurkan diri/ berubah pikiran setiap saat tanpa dikenakan denda atau pun

sanksi apapun.

Apabila Anda bersedia berpartisipasi dalam penelitian ini, Anda diminta

menandatangani lembar persetujuan untuk peneliti. Prosedur selanjutnya adalah:

1.

Anda akan diwawancarai oleh peneliti untuk menanyakan identitas subjek,

riwayat penyakit, riwayat obat-obatan, dan riwayat kesehatan lain yang

berhubungan dengan penelitian ini.

2.

Anda akan diminta untuk dilakukan pengambilan darah sebanyak 1 kali.

Pengambilan darah dilakukan sehari setelah dilakukan prosedur.

3.

Pengambilan darah akan dilakukan oleh perawat yang berkompeten dan

telah terbiasa melakukan tindakan tersebut.

4.

Sampel darah akan dibawa untuk dilakukan pemeriksaan.

Sebagai subjek penelitian, Anda berkewajiban mengikuti aturan atau

petunjuk penelitian seperti yang tertulis di atas. Bila ada yang belum jelas, Anda

dapat bertanya kepada peneliti.

Pada dasarnya, pengambilan darah hanya akan menyebabkan risiko

minimal. Risiko yang mungkin terjadi adalah lebam/kulit kebiruan atau sedikit

perdarahan pada daerah tempat pengambilan darah. Anda dapat menekan daerah

suntikan jika darah masih mengalir dan mengompres daerah suntikan jika terjadi

lebam/kulit kebiruan.

Segala biaya pemeriksaan laboratorium dan penyediaan obat menjadi

tanggung jawab peneliti. Bila masih terdapat pertanyaan atau keluhan sewaktu

penelitian ini berjalan, maka Bapak/Ibu dapat menghubungi saya pada:
