At the filing medical record unit in Dental Hospital For Education At Jember University occurs some problems such as many folder of medical record are broken, incorrect order. Therefore, it needs an evaluation process. The purpose of this research is to evaluate activity of filing medical record in Dental Hospital For Education At Jember University using fishbone diagram method. The data collection in this research is by semi structured interview and observation. The results of this research is man variable that causes the problem is not attended training and workshop and have never received coaching from the director. On material variable, the cause of problem is that a medical record folder condition, on method variable, the cause of problem is that no renewal of standard operational procedure. On money variable, the cause of problem is that never conducted a budget arrangement. Therefore, it needs an activity which can improve human resource quality such as joining in training, workshop and renewal of standard operational procedure.
