DASAR MEKANISME APOPTOSIS
Arief Ismail Khalik, 2004, Pembimbing : Hanna Ratnawati. dr., M.Kes. Apoptosis merupakan komponen penting dalam perkembangan dan homeostasis dari organisme eukariotik multiseluler. Ia bertanggungjawab dalam beberapa proses fisiologis, adaptif, dan patologis. Kematian sel karena apoptosis dapat dibedakan dengan kematian sel akibat nekrosis berdasarkan beberapa kriteria, termasuk morfologi yang karakteristik dan tidak terjadinya reaksi inflamasi.
Sebuah sel yang mengalami apoptosis menunjukkan perubahan morfologi yang karakteristik. Dalam beberapa jam, sel akan mengecil; nukleus dan sitoplasmanya mengkerut; mikrovili akan menghilang; sitoplasma membentuk fragmen yang terbungkus oleh membran sitoplasma (blebbing); kromatin nukleus memadat dan endonuklease seluler memecah kromosom DNA menjadi segmen-segmen.
Apoptosis terdiri dari fase inisiasi, fase efektor, dan fase eksekusi atau degradasi dan fagositosis dari sel yang mengalami apoptosis tersebut. Keluarga protein Bcl-2 dan ICE-related protease memiliki peranan penting dalam mengendalikan apoptosis. Regulasi apoptosis bergantung pada keseimbangan antar anggota keluarga protein Bcl-2. Reseptor permukaan sel, termasuk keluarga tumor necrosis factor receptor (TNFR), mampu memodifikasi sensitifitas sel terhadap proses apoptosis. Anggota keluarga TNFR lainnya dapat mempromosikan atau menginhibisi apoptosis.
Onkogen seluler dan viral yang menstimulasi proliferasi merupakan penginduksi yang kuat dari apoptosis. Induksi tersebut kemungkinan bergantung pada checkpoints dari siklus sel (produk dari tumor supressor gene) yang mendeteksi sel-sel yang bereplikasi abnormal dan memicu apoptosis. Inhibisi dari apoptosis merupakan salah satu tahap penting dari onkogenesis.
Dengan mengetabui dasar mekanisme apoptosis, diharapkan dapat diterapkan sebagai salah satu terapi kanker dan menekan angka kematian penderita kanker.

Apoptosis is a critical component of development and homeostasis in multicellular eukaryotic organisms. It is responsible for numerous physiologic, adaptive, and pathologic events. Apoptotic cell death can be distinguished from necrotic cell death by several criteria, including the characteristics morphology and the absence of resulting inflammatory reaction.
A cell undergoing apoptosis shows characteristic morphologic changes. Over the course of several hours, it shrinks overall as its nucleus and cytoplasm contract; surface features such as microvili disappear; fragments of cytoplasm pinch of! from the surface (blebbing); the nuclear chromatin condenses and cellular endonuclease." cleave the chromosomal DNA into segments.
.lhere are the initiaton phase, the effector phase, and the execution phase or degradation phase, also the phagocytosis of the apoptotic cells. The Bcl-2 family of proteins and the ICE-related proteases plays a central role in apoptosis control. Apoptotic regulation is dependent on the relative balance of opposing Bcl-2 family members. Many cell surface receptors, including the tumor necrosis factor receptor (TNFR) family, have been shown to mod~fy the apoptotic sensitivity of cells. D~frerent memhers of the INFRfamily can promote or inhibit apoptosis.
Cellular and viral oncogenes that stimulate proliferation are strong inducers of apoptosis. This induction prohably is dependent on cell cycle checkpoints (tumor supressor gene products) that detect ahnormally replicating cells and trigger apoptosis. Inhihition of apoptosis therefore is frequently an essential step in the process o{oncogenesis. By knowing the mechanism ofapoptosis, this knowledge can
be used as a cancer therapy and supress the death numbers for the cancer patients.

PENDAHULUAN
1.1. Latar Belakang
Apoptosis berasal dari bahasa Yunani yang berarti "gugur" atau "rontok"
atau dikenal sebagai kematian sel terprogram (programmed cell death). -,,-ematian
sel
terprogram
1m terjadi
selama
masa
pertumbuhan
(embriogenesis,
organogenesis) dan pada sel-sel dewasa.
Peranan proses apoptosis sangat penting dalam menJaga homeostasis
jaringan dengan menyeimbangkan perbandingan antara sel-sel yang hidup dengan
sel-sel yang mati.
Berbeda dengan kematian sel akibat nekrosis, apoptosis dikendalikan oleh
suatu kontrol genetik yang ketat, berlangsung melalui proteolisis, kondensasi
kromatin,
fragmentasi
disusul pengerutan
sel. Proses ini berakhir
dengan
difagositnya sel tersebut, baik oleh makrofaga ataupun sel parenkim di sekitarnya
tanpa kebocoran komponen-komponen
sitosol sehingga tidak terjadi respon
inflamasi.
Terdapat beberapa bukti bahwa pengendalian apoptosis dilakukan oleh
gen-gen yang mengatur siklus sel, seperti gen-gen p53, Rb, myc, dan lain-lain. Sedangkan
keluarga bel2 dan beberapa jenis virus onkogen dikenal sebagai penghambat
apoptosis, yang berpotensi mengakibatkan tranformasi sel menjadi ganas. Untuk
merangsang proses apoptosis tidak diperlukan ekspresi gen baru, melainkan
inhibisi ekspresi gen yang dapat menghambat apoptosis. lnhibisi sintesa protein
mengakibatkan berkurangnya protein inhibitor dengan cepat sehingga terjadi
induksi apoptosis.
Defek pada mekanisme apoptosis dapat meningkatkan ketahanan hidup sel,
menambah kemungkinan
ekspansi sel ganas tanpa bergantung pada proses
pembelahan sel. Oleh karena itu penting untuk diketahui peranan mekanisme
apoptosis terhadap proses timbulnya suatu keganasan dan kegunaannya sebagai

4.1. Kesimpulan
Apoptosis merupakan bentuk kematian sel yang diperlukan, baik untuk
perkembangan
sel
normal
maupun
homeostasis
jaringan.
Peristiwa
ini
dikendalikan secara ketat oleh berbagai gen, baik yang berslfat proapoptotik
maupun antiapoptotik. Apoptosis terjadi melalui tiga fase berturut-turut, yaitu fase
inisiasi, fase efektor, dan fase eksekusi atau degradasi. Kanker diketahui sebagai
akibat mutasi genetik, diantaranya mutasi gen yang terlibat dalam siklus sel dan
mekanisme apoptosis. Pengetahuan mengenai mekanisme apoptosis pada keadaan
normal maupun pada kanker penting untuk menentukan respons penderita
terhadap terapi, bahkan dapat digunakan sebagai landasan suicide gene therapy di
masa depan.
4.2. Saran
Melihat besamya manfaat apoptosis ini dalam menghambat timbulnya kanker, maka pengetahuan mengenai mekanisme apoptosis sebagai tambahan pengetahuan sangat dianjurkan. Sehingga pengetahuan tersebut dapat digunakan sebagai landasan pencegahan kanker. Dengan demikian apoptosis dapat meningkatkan kualitas hidup dan menekan angka kematian penderita kanker.
