LIST OF GRADUATION REQUIREMENT FOR MASTER

(All documents to be submitted to Study Program)

1 Recent Black and White Photograph size 3 X 4 with dark background, matte photo paper (so that faculty stamp can be applied), upright position facing forward, it is compulsory for man to wear formal suit (with tie), for woman adjusted, both earlobes should be seen, except for those who wears veil and not wearing sunglasses, (2 pieces).

2 Recent Colour Photograph size 3 X 4 with dark background, matte photo paper (so that faculty stamp can be applied), upright position facing forward, it is compulsory for man wear formal suit (with tie), for women adjusted, both earlobes should be seen, except for those who wears veil and not wearing sunglasses, (2 pieces).

3 Valid Student Card or Terminated Student Card by BNI (original and photocopy)

4 Draft of Master Degree Certificate (2 pieces, affixed with black and white photograph, matte photo paper) signed and added with Bachelor Degree certificate (2 copies) 5 BNI Bank Recipt for graduation payment

6 Graduate’s CV Form (2 pieces, attached with colour photograph, matte photo paper) signed and acknowledged by Program Manager

7 Copy of AcEPT Certificate/TOEFL UGM/one which admitted by University (1 photocopy)*) Minimum TOEFL score > 450 / AcEPT > 209 for Students batch 2009 and after

8 Copy of PAPs/TPA which admitted by University (1 photocopy)*) Minimum TPA score /PAPs > 500 for Students batch 2009 and after

9 Paid off certification letter from Financial Directory of UGM, Central Building, 1st Floor, North Wing (2 pieces - original and photocopy)

B. Print out of thesis online submission at https://unggah.etd.ugm.ac.id, using UGM email account

C. All files are separated from thesis manuscript all in PDF format, example for the title can be seen as follows: Format: [Education Level]-[Graduation Year]-[University Student Number]-[part code].pdf
