Hipertensi
Hipertensi atau penyakit darah tinggi sebenarnya adalah suatu gangguan pada pembuluh darah yang mengakibatkan suplai oksigen dan nutrisi yang dibawa oleh darah terhambat sampai ke jaringan tubuh yang membutuhkan. Hipertensi sering kali disebut sebagai pembunuh gelap (Silent Killer), karena termasuk penyakit yang mematikan tanpa disertai dengan gejala lebih dahulu (Sustrani dan Alam, 2004).
Hipertensi adalah suatu keadaan dimana tekanan darah meningkat melebihi batas normal. Batas tekanan darah normal bervariasi sesuai dengan usia. Berbagai faktor dapat memicu terjadinya hipertensi, walaupun sebagian besar (90%) penyebab hipertensi tidak diketahui (hipertensi essential).
Penyebab tekanan darah meningkat adalah peningkatan kecepatan denyut jantung, peningkatan resistensi (tahanan) dari pembuluh darah dari tepi dan peningkatan volume aliran darah (Kurniawan, 2002).

1. Prevalensi secara global
WHO mengestimasikan saat ini prevalensi hipertensi secara global sebesar 22% dari total penduduk dunia. Dari sejumlah penderita tersebut, hanya kurang dari seperlima yang
melakukan upaya pengendalian terhadap tekanan darah yang dimiliki. Prevalensi Hipertensi di dunia Menurut WHO, 2019 : dimana wilaya Afrika memiliki prevalensi Hipertensi tertinggi
sebesar 27%, posisi kedua ada mediterania Timur sebesar 26%, dan di posisi ke tiga yaitu Asia Tenggara sebesar 25%. Di Posisi ke empat ada Eropa sebesar 23%, selanjutnya bagian pasifik barat sebesar 19% dan Amerika sebesar 18%.
2. Prevalensi di Indonesia
Berdasarkan Laporan Nasional Riset Kesehatan Dasar 2018 terhadap 658.201 subjek penelitian dari seluruh provinsi di Indonesia, prevalensi hipertensi menurut diagnosis dokter pada
penduduk >18 tahun berada pada angka 8,36%. Angka ini terlampau jauh dari prevalensi hipertensi pada penduduk >18 tahun berdasarkan hasil pengukuran secara nasional sebesar 34,11%.Data tersebut menunjukkan tingginya prevalensi hipertensi yang belum terdeteksi di masyarakat Indonesia. Selain itu, kepatuhan minum obat secara rutin pada subjek yang telah didiagnosis hipertensi hanya berada pada 54,40%.
3. Prevalensi di DIY
Berdasarkan Laporan Provinsi DI Yogyakarta Riset Kesehatan Dasar 2018 terhadap 8,373 subjek penelitian dari seluruh kabupaten/kota di provinsi DIY. Prevalensi hipertensi menurut diagnosa dokter pada populasi dewasa berada pada angka 10,68%. Dan angka ini terlampau jauh dari prevalensi hasil pengukuran hipertensi yaitu berada pada angka 32,86%. Maka data tersebut menunjukan tingginya prevalensi hipertensi yang belum terdeteksi di provinsi DIY. Dan dalam kepatuhan minum obat secara rutin pada subjek yang telah didiagnosis hipertensi berada pada angka 50,29%.

Berdasarkan etiologinya hipertensi dapat diklasifikasikan menjadi 2 yaitu : 1. Hipertensi Primer/Esensial
Hipertensi primer/esensial memiliki insiden sebesar insiden 80-95% dimana pada hipertensi ini belum dapat diketahui penyebabnya, sementara penyebab sekunder dari hipertensi esensial juga tidak ditemukan. Pada hipertensi esensial tidak ditemukan penyakit renovaskular, gagal ginjal maupun penyakit lainnya, genetik serta ras menjadi bagian dari penyebab timbulnya hipertensi esensial termasuk stress, intake alkohol moderat, merokok, lingkungan dan gaya (Triyanto, 2014).
2. Hipertensi Sekunder
Hipertensi sekunder penyebabnya dapat diketahui seperti adanya suatu penyakit atau kelainan yang mendasari, seperti stenosis arteri renalis, penyakit parenkim ginjal, feokromositoma, hyperaldosteronism, dan sebagainya (James et al., 2014).

Patofisiologi terjadinya hipertensi adalah melalui terbentuknya angiotensin II dari angiotensin I oleh angiotensin converting enzyme (ACE). ACE memegang peranan penting dalam mengatur tekanan darah. Darah mengandung angiotensinogen yang diproduksi di hati. Selanjutnya oleh hormone renin yang diproduksi di ginjal akan diubah menjadi angiotensin I. Oleh ACE yang terdapat di paru-paru, angiotensin I diubah menjadi angiotensin II. Angiotensin II inilah yang memegang peranan penting dalam menaikkan tekanan darah. Angiotensin II adalah vasokonstriktor yang sangat kuat dan memiliki efek lain yang juga mempengaruhi sirkulasi. Selama angiotensin II ada dalam darah, maka angiotensin II mempunyai dua pengaruh utama yang dapat meningkatkan tekanan arteri.
Pengaruh pertama yaitu vasokonstriksi, timbul dengan cepat. Vasokonstriksi terjadi terutama pada arteriol dan sedikit lemah pada vena. Cara kedua dimana angiotensin II meningkatkan tekanan arteri adalah dengan bekerja pada ginjal untuk menurunkan ekskresi garam dan air (Sylvestris, 2017).
