The guidelines is set forth for the College of Allied Sciences to exact appropriate protocols and guidelines in the event of emerging infectious diseases breakout and force majeure during the conduct of delivering education to our students, in accordance and compliance to existing health protocols per Alert Level as mandated by the Department of Health, Inter-Agency Task Force for the Management of Emerging Infectious Diseases (IATF), Commission on Higher Education, the Local Government Unit and or other applicable requirements set forth by other governing bodies in the Academic Community.
Specifically, these guidelines will:
1. Provide the detailed response that the college will undertake in the event that the aforementioned scenario takes place;
2. Guide both the college’s clientele and personnel on the various requirements, guidelines and protocols that need to be observed inside the campus for a specific alert level, and
3. Ensure the continuous viability of the institution to provide services and continue its operation amidst an unforeseen emergency situation.
Learning Modality
Learning modality is defined within the context of these guidelines as the different methods through which learning takes place. This section will explain the delivery of quality education to students, specifically the manner of curriculum implementation and alternative methods to facilitate learning..
Alert Levels 1, 2 and 3
All year levels under Alert Levels 1, 2 and 3 shall use the HBALMs using a combination of synchronous and asynchronous sessions, both online and in-person. All students are required to submit a vaccination card and medical health insurer for this purpose and a duly accomplished and signed Parents’ Consent Form. Other specific guidelines are discussed further below.
Lecture Classes General Guidelines
1. Lecture classes shall still be conducted through HBALMs.
2. Major examinations of all courses, both core/professional and General Education subjects shall be administered inside the classrooms. Long quizzes and other activities which require physical participation of students shall be conducted face to face.

159 | P a g e 3. Online major examinations shall no longer be done via online platforms.
4. Blended learning approach shall be employed for PE classes. Some activities shall be performed face to face, and those activities shall be determined by the subject professors.
Laboratory Classes General Guidelines
1. All laboratory experiments of all courses shall be conducted inside the laboratory rooms. The laboratory room capacity shall be 100%, 75% and 50% for Alert Levels 1, 2 and 3, respectively. All students are required to attend. Virtual laboratory experiments shall no longer be practiced.
2. Major examinations of all courses, both core and professional laboratory classes shall be administered inside the classrooms. Long quizzes and other activities which require physical participation of students shall be conducted face to face.
3. Online major examinations shall no longer be done via online platforms.
4. Blended learning approach shall be employed and activities shall be performed face to face, and those activities shall be determined by the subject professors.
5. Students doing experimental research are allowed to use laboratory facilities for sample preparation, data gathering, and testing.
Specific Guidelines
1. Laboratory instructors and students who are going to attend the laboratory sessions should be fully vaccinated.
2. Students are required to wear mask at all times for Alert Levels 1, 2and 3 with mandatory temperature scanning. If a student's temperature is higher than 37.5°C, he or she should take a 5-minute break, after which, another temperature check shall be done. If the student's temperature remains high, he/she shall be asked to go home.
3. Wearing of face shields shall only be required for Alert Level 3.
4. Health declaration will not be required for Alert Level 1; however, the health declaration form shall be accomplished under Alert Levels 2 and 3 and via QR Code for Alert Levels 1 and 2 upon entry. After accomplishing the Health Declaration Form, the student shall proceed to the TSA to obtain a CLEAR PASS, then proceed to the designated laboratory class.
5. The right wing stairs of the Lourdes Campos Building will used to gain entrance to the 2nd floor laboratory rooms and the college, while the left wing stairs of the Lourdes Campos Bldg. will be used as the exit point, following a one-way traffic flow of movement in the 2nd Floor Hallway, from Vice Dean’s Office to Microbiology Room (LEC Room 5209 ➙Room 5208).

6. Each laboratory room should have a maximum of 20 students and one teacher, with 2-3 members per group, or as required by prevailing guidelines and policies.
7. Personal protective equipment such as laboratory gown, face mask, laboratory goggles and gloves should always be worn inside the laboratory.
8. Laboratory instructors are in charge of enforcing and monitoring of the minimum standard health protocols and guidelines.
Alert Levels 4 and 5
In the event of Alert Levels 4 and 5 , the conduct and delivery of academic services of the College shall adopt the full Home-Based Alternative Learning Modalities (HBALMs).
As such the College shall implement the following:
General Guidelines
1. Students and faculty members must have a device and Internet connection at home.
2. Faculty members must have knowledge on how to use the learning management system (LMS) or have undergone a series of trainings on the use of Blackboard LMS and other platforms.
3. Students must be oriented and trained on the use of Blackboard LMS and other platforms.

161 | P a g e 4. Safety measures and precautions shall be observed at all times both inside and outside
the campus.
Conduct of Classes
1. Classes are conducted using synchronous and asynchronous learning.
a. Synchronous learning is a type of learning where students are attending classes or accomplishing an activity at the same time. This is done through live online sessions.
b. Asynchronous learning is used to describe learning and instruction that do not occur at the same time or at the same place.
c. For lecture classes (professional courses, sciences and mathematics), at least 50%
of the period shall be devoted for synchronous learning while at least 30% for laboratory classes.
Online Classes
1. Lecture and laboratory classes shall be conducted online via the institution’s LMS. The following shall be employed:
Lecture Courses
a. Lecture handouts, PowerPoint presentation, podcast-style recordings, and/or pre-recorded videos shall be posted on the institution’s LMS site.
b. Live online sessions shall be held every class schedule via Cisco Webex Education and other online platforms.
Laboratory Classes
a. Laboratory experiment manuals/handouts, laboratory reports template, and pre- recorded videos on the conduct of experiments shall be posted on the institution’s LMS site.
b. Prelab lectures shall be held online. At least 30% of the laboratory experiments shall be conducted via virtual laboratory simulations.
c. Live online sessions shall be held every laboratory class schedule via Cisco Webex Education and other online platforms.
2. Attendance to live sessions is not part of a student’s overall grade. However, a student must inform the subject professor of the reason for not joining the live session. Policy on attendance as stipulated in the student manual still applies. Students are allowed 20% of the total number of school days or 14 hours of absences inclusive of tardiness.
All absences after that shall mean excessive absences, which shall merit a grade of 0.00.

162 | P a g e 3. Live sessions shall be recorded for the benefit of students experiencing connectivity
issues.
Assessment and Other Course Requirements
Student assessments and requirements shall be announced at least a week prior to schedule or deadline. The following shall be implemented:
General Education Courses
1. Short quizzes and long examinations shall be administered online via the institution’s LMS site.
2. Navigation method in answering the online quiz is sequential and set to one attempt.
3. Short quiz is to be given at least one week after the discussion.
4. A duration of one day is given to students in answering the quiz. This is for the benefit of students experiencing connectivity issues.
5. Long examination/Major exams are given during the assigned schedule. The duration depends on the type of exam or difficulty of questions. For sciences, two hours is given to accomplish the exam. The subject faculty must be online on the assigned time of examination. This is to ensure that all concerns shall be addressed properly.
6. Final output/projects shall be submitted online.
Professional Courses
1. Major exams (long exams, midterms, finals) are given during the assigned schedule.
Questions are set to free navigation method and one attempt but proctored. Students are required to have two devices during the examination, with one device being used as a camera for the proctors to monitor the students while taking the exam and the other one for the examination questions and answers.
2. Short quizzes shall be administered online via the institution’s LMS site. Navigation method in answering the online quiz is sequential and set to one attempt. Additional restrictions (such as lockdown browser) may be set by the instructor to ensure the integrity of the assessment. Short quizzes may also be administered with virtual proctoring. In that case, questions may be set to free navigation and one attempt.
Students shall need two devices for virtual proctoring, as stated in No. 1.
3. Final output projects shall be submitted online. The use of online resources, such as in silico approaches shall be employed to execute the goals of the project.
Missed Requirements/Quiz/Exam
1. Should the student fail to submit the requirement on the deadline, a percentage deduction shall be given. The deduction shall be determined by the subject professor.
Should the student still fail to comply with the requirements after the second chance, a grade of zero will be given.
2. Should the student fail to take the major exam on the schedule, he/she needs to secure a permit for a special exam, to be approved by the subject professor and by the

163 | P a g e Registrar. It must be taken on the schedule set by the subject professor. Should the
student still fail to take the exam on the set schedule, a grade of zero shall be given.
3. Should the student fail to take the short quiz on the schedule, he/she needs to inform the subject professor by writing a letter of explanation with parents’ signature and proof not later than one week after the scheduled quiz. Once the reason is considered valid, the subject professor shall set a schedule for the special quiz. Should the student still fail to take the quiz on the set schedule, a grade of zero shall be given. A grade of zero shall also be given should the student fail to inform the faculty within the one-week period
Thesis Requirements
1. In silico approach may be employed.
2. Thesis presentations and defense shall be conducted via Cisco Webex Education Practicum Requirements
1. Should deployment to companies be disallowed by CHED, the following tasks, but not limited to, shall be required of the students:
a. Analyze and process raw data provided.
b. Write Standard Operating Procedures (SOP) for various methods.
c. Write a Certificate of Analysis based on data provided.
2. Practicum coordinators from the Partner Companies shall be invited to conduct online orientation and lectures about the operations of their companies.
Instructional Materials/Books
1. Copy of the e-book (or e-book code) shall be sent to students thru email by the book supplier once the students are officially enrolled in the course.
2. For printed books/modules/handouts, they shall be delivered to students via courier once the students are enrolled in the course.
Monitoring of Students Performance
1. Mentoring program shall be implemented to monitor the progress of the students and to assist the students with their academic career. The following shall be the guidelines:
a. Mentors shall meet with their assigned students at least once per grading period.
b. Mentoring sessions shall be recorded via Cisco Webex Education.
c. Mentors shall submit a report of their meeting to the Program Director for documentation purposes.
d. Any at-risk cases shall be reported immediately to the Vice Dean and Program Director/Chair.

164 | P a g e Monitoring of Online Classes
1. To ensure the quality of teaching, instructors shall be required to submit a weekly report of online activity.
Lecture classes shall be facilitated via the institutional Learning Management System (LMS). In some occasions where the need arises due to scheduling, personnel and physical facilities limitations, some lectures may be held on campus. Short quizzes as well as submission of assignments and activities are facilitated thru the LMS while major examinations are administered on campus.
Laboratory classes, on the other hand, shall be held on campus due to hands- on experience necessary. The college/department may also opt to schedule online laboratory sessions by using the LMS and/or other means.
Minimum Health Standard
This section gives emphasis on the requirements, procedures and conduct of transactions inside the campus in different alert levels as stipulated and in compliance with the Commission on Higher Education and other governing agencies.
Alert Levels 1, 2 and 3
The use of foot bath flooded with sanitizers, as well as handwashing, is mandatory and will be placed on all entry and exit points of the building. Upon entry, temperature check must be done, together with hands disinfection using the disinfection booth beside the temperature scanner. Wearing of mask is mandatory while the use of face shield is optional. Personnel as well as students need to be fully vaccinated.
Alert Levels 4 and 5
The foregoing disinfection protocols are not applicable as the campus is closed under this condition.
Spatial Protocols in the Campus Alert Levels 1, 2, and 3
The use of acrylic barriers shall not be required under Alert Level 1 for both lectures and laboratory classes, while this shall be optional under Alert Level 2 and mandatory under Alert Level 3. Social distancing with a distance of 1.5 meters shall be optional under Alert Level 2 and mandatory under Alert Level 3. Single entry and exit policy to the campus, unidirectional pathway and separate ingress and egress points shall be optional under Alert Level 2 and mandatory under Alert Level 3.
Alert Level 4 and 5

165 | P a g e Under Alert Levels 4 and 5, the implementation of the restrictions is suspended and not
applicable since no activities inside the campus are allowed.
Fundamental Controls While in Campus
This section explains the different measures and controls that are put into place inside the campus under different alert levels.
Alert Level 1, 2 and 3
A memorandum of undertaking and the creation of appropriate guidelines shall be created in response to the needs of the prevailing circumstances with the conduct of an orientation program before the undertaking takes effect.. Parent’s consent/Conforme is mandatory for the activities requiring the students to enter the campus. Vaccination cards and health insurance are required for both students and personnel. In the absence of vaccination card, RT-PCR test or any pertinent test will be asked upon entry to school under Alert Level 3 while this will be optional under Alert Level 2. Health declaration via scanning a QR code is mandatory under Alert Levels 2 and 3, and optional under Alert Level 1. Contact tracing is mandatory. Signages and other infographics about the health protocols shall be posted on walls, doorways, and other strategic places while floor decals will only be mandatory under Alert Level 3 to guide students and personnel on the foot traffic. Classrooms for both lectures and laboratory classes should be disinfected after each class with the installation of HEPA filters, and complying with the required airflow under Alert Level 3. The college, in cooperation with the institution, shall set up an isolation room.
Alert Levels 4 and 5
Under the Alert Level 4 and 5, the implementation of the restrictions is suspended and not applicable since no activities inside the campus are allowed.
