6. POLICIES AND GUIDELINES ON THE CONDUCT OF FACE-TO-FACE EXAMINATIONS Introduction
The emergence of the COVID-19 global pandemic has increased the risks we face in our everyday activities. However, with the improvement in the situation, certain health and safety restrictions were lifted which allows for the resumption of face-to-face classes including the conduct of examinations.
Conduct of face-to-face examination will enable the College of Nursing to evaluate the knowledge of the students as well as to simulate a paper-and-pencil test in preparation for the licensure exam.
General Guidelines:
1. Evaluative activities such as Prelim, Midterm, Final, Comprehensive, Unit, Evaluation Examinations and other assessments deemed necessary shall l be administered face to face.
2. The Term and Comprehensive Examinations shall be scheduled by the Vice Dean. However, the Unit and Evaluation Examinations shall be arranged and scheduled by the respective faculty member.
3. The examination schedule shall be posted and disseminated one week before the examination.
4. The existing guidelines on the examination stipulated in the CN Student Handbook shall be implemented.
5. The following conditions prohibit the students to take the face-to-face examination 5.1.1. Manifesting COVID-19 related symptoms
5.1.2. Testing positive for COVID-19 based on an RTPCR and/or swab test.
5.1.3. Living with family members/persons who are confirmed cases of COVID-19
Students who failed to take the examinations due to the aforementioned conditions should make appropriate arrangements with their course professor and level chairs for special examination. They are required to seek consultation with Vitamin HSI

196 | P a g e 6. Honesty is expected from all CN students. Failure to declare symptoms and/or exposure to COVID-19 positive individuals shall result in a disciplinary action.
7. The following precautionary measures shall be implemented before, during and after examination:
Before the Examination
7.1.1. A suitable venue for the examination shall be enforced following the institutional guidelines
7.1.2. No mask, No Entry Policy shall be enforced.
7.1.3. Doors to examination rooms shall be left open to prevent touching of the knobs
7.1.4. Proper indoor ventilation of the classroom shall be implemented (all windows opened, if possible)
7.1.5. The students are expected to;
7.1.5.a. reserve ample time to enter the examination room by batches to avoid crowd gathering.
7.1.5.b. Practice hand hygiene before entering the classroom.
7.1.5.c. Observe social distancing
7.1.5.d. Bring their own ballpen and pencil (grade Number 2) on the day of the examination.
During the Examination:
7.2.1. Eating and drinking are not allowed inside the examination room.
7.2.2. The students are expected to wear their face mask throughout the examination period.
7.2.3. Sharing of ballpen/pencil, paper, or any personal items is not allowed.
After the Examination
7.3.1. The students are expected to vacate the premises right after the examination for disinfection purposes.

PARENTAL CONSENT
This is to acknowledge that we were given information on the Limited Face-to-Face Classes (Related Learning Experience) that will be implemented during the Second Semester of the School year 2022-2023 and a copy of the College of Nursing Safety Policy, Guidelines and Procedure on the Resumption of the Limited Face-to-Face Classes.
Together with our child/ward, we understand that the institution and its officers, faculty and staff are expected to exercise the legal diligence required for the safety and well-being of our child/ward for the duration, place, date and time of the activity as stated.

197 | P a g e This legal diligence would include oral and/or written instruction, whether given before or during the activity that, if followed, would ensure the safety and well-being of our child/ward.
If our child/ward disregards or fails to follow those instructions, we shall have no claim against the Institution, its officers, faculty, and staff-in-charge.
Likewise, our child/ward shall at all times during the institutional sanctioned activity be required to observe the minimum standard health protocols, such as, but not limited to, practicing social distancing, performing regular handwashing, wearing of prescribed PPEs and other directions which may be given by faculty and staff-in-charge.

5. Proof of negative RT-PCR test prior to the limited face-to-face skills lab/clinical rotation 6. Medical clearance for students recently recovered from COVID-19 Infection.
