B ID D ATA S HEET
The name of the contract is construction of two (2) units of drinking water system - Bulanao Campus. The Government of the Philippines (GoP) through General Budget Act (GAA) Capital Outlay CY 2017 duly approved by the Board of Regents through Board Resolution No. The name of the project is Design and Construction of Two-Unit Drinking Water System at KSU Bulanao Campus.
5.4(b) For this purpose, similar contracts shall refer to contracts which have the same main categories of work as the design and construction of water systems or the design and construction or installation of water facilities. Note: All key personnel must have General and Relevant Experience applicable and defined in their line of specializations. Amount of not less than Php e ABC), if the bid security is in cash, cashier's/manager's check, bank draft/guarantee or irrevocable letter of credit;.
NOTE: The parent envelope must be wrapped in a black plastic bag when dropped off at the KSU Bid Box located in front of the BAC Office, 2/F Admin Bldg., Main Campus.
G ENERAL C ONDITIONS OF C ONTRACT
All materials and equipment on site are considered the property of the client. The contractor will follow all the instructions of the client's representative, which are in accordance with the applicable legislation where the location is located. The performance bond remains valid until the client issues a certificate of final acceptance.
All drawings prepared by the contractor for the performance of the temporary works are subject to prior approval by the contracting authority's representative before being used. The approval of the contracting authority's representative does not diminish the responsibility of the contractor for the design of the temporary work. The Contractor shall obtain third party approval for the design of the Agency Work, if required by the Contracting Authority.
Each completed form must be verified and signed by the contracting entity's representative within two days of completion of the work. The approval of the work program by the contracting entity's representative does not change the contractor's obligations. Such request for payment must be verified and certified by the procuring entity's representative/project engineer.
The Contractor shall submit monthly statements of the estimated value of the work performed less the cumulative amount previously certified to the Procuring Entity's Representative.
S PECIAL C ONDITIONS OF C ONTRACT
To provide signed and sealed structural analysis, structural details, drawings, plans and specifications relating to the construction of two units drinking water system;. v). Integrate solar panel system or windmill as a form of alternative electrification for the operation of this proposed two-unit drinking water system; And. VII). To build two units of drinking water system at Bulanao Campus that will provide an uninterrupted supply of water, sufficient water pressure, a water quality free of unpleasant taste, turbidity, odor or chemical attack and water that is bacteriologically safe to drink and easily accessible to Kalinga State University end users.
NOTE: Names of key personnel and their appointment to be completed by the winner before signing the contract. 7.4(c) The performance bond covers the period from the signing of the contract to the final acceptance of the project by the government, subject to the warranty period as prescribed in Sec. For this reason, the performance bond cover must include a one (1) year defect liability period and will only be released when the client issues a final acceptance certificate.
The amount to be withheld for late submission of an updated Work Program is at least equal to one-tenth of one percent (0.001) of the actual cost of contract for each day of delay. The date on which "as built" drawings are required is upon approval of the variation orders.
S PECIFICATIONS
The architectural drawings and details to be approved shall be the general reference in the structural, electrical and sanitary design of the construction of the two unit potable water system. The design development drawings for the proposed two-unit water system at KSU Bulanao Campus shall be included in the bidder's technical proposal. All communications about this project should be addressed to the President of Kalinga State University.
Master's or Ph.D in Civil/Structural Engineering or recognized as a Civil Engineer by PICE or Civil Engineers Association of the Philippines. This must refer to the natural plane or the identified final floor lines of adjacent existing buildings. The proposed building must also be designed using a wind importance factor of 1.15, particularly in the design of the roof system and building supports.
Soil drilling and survey (at least three drilling sites) should be performed to estimate the bearing capacity of the soil that will be used in the design of the building's foundation. Reference: Revised Implementing Rules and Regulations of the National Building Code of the Philippines (PD 1096).

If the funding source for the project has been withheld or reduced through no fault of the contracting entity. The Completion Date is the date of completion of the Works as certified by the Contracting Entity's representative in accordance with GCC Clause 49. The Contractor shall submit to the Employer for consent the name and details of the person authorized to receive instructions on behalf of the Contractor.
The performance security provided in favor of the client shall be forfeited if the contractor is found to be in breach of any of its obligations under the contract. The contracting authority's representative may require the contractor to provide an estimate of the expected effect of the future event or condition on the contract price and completion date. The first installment payment may be made by the contracting authority to the contractor, provided that at least twenty percent (20%) of the work has been completed, as certified by the contracting authority's representative.
