Saya yang bernama Irawati Susilo (137032045) adalah mahasiswi Program Studi S2
Ilmu Kesehatan Masyarakat Fakultas Kesehatan Masyarakat Universitas Sumatera
Utara. Saat ini sedang melakukan penelitian tentang “Peran Teman Sebaya Terhadap
Sikap Remaja tentang Kesehatan Reproduksi di SMA HARAPAN 3 Medan Tahun
2015” penelitian ini merupakan salah satu kegiatan dalam menyelesaikan tugas akhir
di Program Studi S2 Ilmu Kesehatan Masyarakat Fakultas Kesehatan Masyarakat
Universitas Sumatera Utara.
Untuk keperluan tersebut saya mohon kesediaan siswa SMA HARAPAN 3
untuk menjadi responden dalam penelitian ini. Selanjutnya saya mohon kesediaanya
untuk mengisi kuesioner ini dengan jujur. Jika bersedia silahkan menandatangani
lembar persetujuan ini sebagai bukti kesediaan anda.
Partisipasi anda dalam penelitian ini bersifat sukarela, sehingga anda bebas
