Pengaruh Persepsi Mutu Pelayanan Kesehatan Terhadap Kepuasan Pasien

di Puskesmas Simpang Limun Tahun 2017

Bapak/ibu/saudara yang terhormat,

Saya adalah mahasiswa Fakultas Kesehatan Masyarakat Universitas Sumatera

Utara. Saat ini saya sedang menyelesaikan tugas akhir Sarjana Kesehatan

Masyarakat.

Dalam rangka mengumpulkan data, saya memohon atas kesediaan serta bantuan

bapak/ibu/saudara dapat meluangkan waktu untuk pengisian kuesioner ini. Hasil

dari kuesioner ini sepenuhnya akan digunakan untuk kepentingan penelitian saya

yaitu tentang Pengaruh Persepsi Mutu Pelayanan Kesehatan Terhadap Kepuasan

Pasien di Puskesmas Simpang Limun Tahun 2017 dan saya menjamin kerahasiaan

dari jawaban kuesioner ini.

Mengingat keberhasilan penelitian ini akan sangat bergantung pada kelengkapan

jawaban, dimohon dengan sangat agar kiranya jawaban dapat diberikan selengkap
