TELAAH MASALAH TERAPI OBAT PADA PASIEN PENDERITA DIABETES MELITUS KOMPLIKASI HIPERTENSI DI RUANG RAWAT
INAP RSUD KOTA LANGSA TAHUN 2014
ABSTRAK
Diabetes melitus dan hipertensi merupakan salah satu penyakit yang paling sering dijumpai pada pasien geriatri. Keduanya seringkali hadir bersamaan sehingga apabila tidak ditangani dengan tepat akan menimbulkanrisiko makrovaskuler dan mikrovaskuler dan risiko kejadian kardiovaskuler juga akan semakin tinggi.
Pemberian kombinasi obat untuk mengontrol kadar glukosa darah dan tekanan darah akan mengarah pada polifarmasi yang merupakan salah satu penyebab utama terjadinya Masalah Terapi Obat (MTO). Penelitian ini bertujuan untuk mengetahui ada atau tidaknya MTO serta tingkat kejadiannya pada pasien penderita diabetes melitus komplikasi hipertensi di ruang rawat inap RSUD Kota Langsa pada tahun 2014.
Penelitian ini merupakan penelitian deskriptif retrospektif menggunakancase control sebagai metode penelitian. Penelitian ini menggunakan klasifikasi MTO berdasarkan Pharmaceutical Care Network Europe (PCNE) V5.01 sebagai dasar evaluasi sehingga didapatkanlah pemilihan obat yang kurang tepat, indikasi tidak terobati, obat dengan mekanisme kerja mirip, kontraindikasi pemakaian obat dan potensial interaksi obat sebagai MTO yang ditelaah dalam penelitian ini.
Berdasarkan penelitian yang telah dilakukan dari 57 kasus yang masuk kedalam kriteria inklusi, telah terjadi MTO sebanyak 56kasus dan 1 kasus tidak mengalami MTO. Besaran MTO yang terjadi sekitar38,61% kasus pemilihan obat yang kurang tepat ditemukan. Berikutnya ada 1,98% kasus indikasi tidak terobati, 3,96% kasus obat dengan mekanisme kerja mirip, 6,93% kasus kontraindikasi pemakaian obat, dan 48,51% kasus potensial interaksi obat. Dengan demikian dapat disimpulkan bahwa telah terjadi masalah terapi obat pada pasien penderita diabetes melitus komplikasi hipertensi di ruang rawat inap RSUD Kota Langsa tahun 2014.

ANALYSIS OF DRUG RELATED PROBLEMS IN DIABETES MELLITUS WITH HYPERTENSION PATIENTS IN LANGSA GENERAL HOSPITAL
INPATIENT CHAMBER IN 2014
ABSTRACT
Diabetes mellitus with hypertension is one of the most common diseases which are found in geriatric patient. Both usually occur at the same time so if there are not any appropriate medications, it will enhance the possibility of macrovascular and microvascular risks and also cardiovascular risk event will be increased.
The use of drug combinations to maintain blood glucose level and blood pressure will lead to a polypharmacy which is one of the main causes of Drug Related Problems (DRPs). The purposes of this research are to analyze whether there are or there are not any DRPs and the number of it which occur in diabetes mellitus with hypertension patiens in Langsa General Hospital Inpatient Chamber in 2014.
This is a descriptive-retrospective research, using ‘case control’ as its research method. This research uses Pharmaceutical Care Network Europe (PCNE) V5.01 classification as its basic evaluation, so improper drug selection, untreated indication, inappropriate duplication of therapeutic group of active ingredient, drug’s contraindication, and potential drug interaction will be the DRPs that are analyzed.
According to the research that has been analyzed from 57 cases that are classified into the inclusion criteria, about 56 cases shows that there are DRPs happened and 1 case show there aren’t any DRP happened. About 38.61% data are found out as improper drug selection. Thereafter, there are 1.98% cases of untreated indication, 3.96% of inappropriate duplication of therapeutic group of active ingredient, 6.93% of drug’s contraindication, and 48.51% of potential drug interaction. Therefore, it can be concluded that there are DRPs in diabetes mellitus with hypertension in Langsa General Hospital inpatient chamber in 2014.
