Assessment scales
Functional Oral Intake Scale (FOIS): 1 = nothing by mouth; 2 = tube dependent with minimal attempts of food or liquids; 3 = tube dependent with consistent oral intake of food or liquid; 4 = total oral diet of a single consistency; 5 = total oral diet with multiple consistencies but requiring special preparation or compensations; 6 = total oral diet with multiple consistencies without special preparations, but with specific food limitations; 7 = total oral diet with no restrictions.
penetration and aspiration scale (PAS): 1 = material does not enter the airway; 2 = material enters the airway, remains above the vocal folds, and is ejected from the airway; 3 = material enters the airway, remains above the vocal folds, and is not ejected from the airway; 4 = material enters the airway, contacts the vocal folds, and is ejected from the airway; 5 = material enters the airway, contacts the vocal folds, and is not ejected from the airway; 6 = material enters the airway, passes below the vocal folds, and is ejected into the larynx or out of the airway; 7 = material enters the airway, passes below the vocal folds, and is not ejected from the trachea despite effort; 8 = material enters the airway, passes below the vocal folds, and no effort is made to eject it.
Murray secretion severity rating scale: 0 = the most normal rating; 1 = any secretions evident upon entry or following a dry swallow in the channels surrounding the laryngeal vestibule that were bilaterally represented or deeply pooled. This rating
would include cases where there is a transition in the accumulation of secretions during the observation segment. A subject could start with no visible secretions but accumulate secretions in an amount great enough to be bilaterally represented or deeply pooled. Likewise, a subject would be rated as a level 1 if initially presenting with deeply pooled bilateral secretions and ending the observation segment with no visible secretions; 2 = any secretions that changed from a level 1 rating to a level 3 rating or from a level 3 rating to a level 1 rating during the observation period; 3 = the most severe rating. Any secretions seen in the area are defined as the laryngeal vestibule. Pulmonary secretions were included if they were not cleared by swallowing or coughing at the close of the segment.
The vallecular residue scale has five levels: 1 = none: no residue; 2 = trace: trace coating of the mucosa; 3 = mild: epiglottic ligament visible; 4 = moderate: epiglottic ligament covered; 5 = severe: filled to epiglottic rim
The pyriform sinus residue scale has five levels: 1 = none: no residue; 2 = trace:
trace coating of the mucosa; 3 = mild: up wall to quarter full; 4 = moderate: up wall to half full; 5 = severe: filled to aryepiglottic fold)
The degree of the Pharyngoesophageal Segment Opening (PESO): 0 = normal open; 1 = partially open; 2 = slightly open; 3 = not open
The degree of the Laryngeal Elevation (LE): 0 = superior movement of thyroid cartilage that results in complete approximation of the arytenoids to the epiglottic petiole; 1 = superior movement of thyroid cartilage that results in partial approximation of the arytenoids to the epiglottic petiole; 2 = superior movement of thyroid cartilage that results in slightly approximation of the arytenoids to the epiglottic petiole; 3 = indicates no superior movement of thyroid
The degree of the Hyoid Motion (HM): 0 = complete anterior hyoid movement; 1 = incomplete anterior hyoid movement; 2 = no anterior movement of hyoid
