TOM TAT

Muc tieu: nhdn xet dac diem 1dm sang, nguyen nhdn viem ket mac cap vd danh gid ket qua dieu tri.

Doi tuong va phuong phap nghien cuu: nghien ci'nt md ta cat ngang tren 91 benh nhdn (167 mat), 100% benh nhdn duac lam xet nghiem soi nhuom, nudi cdy vi khudn va te bao hoc ket mac.

Ket qua: ty le viem ket mac cd)) do vi khudn la 82,4% vd do virut Id 17,6%; 20,9% benh nhdn nudi cdy phdn lap duac vi khudn; ty le khoi benh 100%>.

Ket luan: nguyen nhdn viem ket mac thuang gap la do vi khudn. benh canh 1dm sdng do vi khudn thudng ndng hern do virut. Viem ket mac thudng duac dieu tri khdi hdu nlur khdng de lai di chimg.

I. DAT VAN DE

Viem ket mac cap la benh thuong gap, do nhieu nguyen nhan: vi khuan (VK), virut va di ung... Nhung bien doi ciia moi truong khi hau, sir xuat hien va sir dung nhieu loai khang sinh moi, cung nhu viec lam dung khang sinh trong dieu tri...

Sir phan bo cac loai vi khuan trong viem ket mac cap d Viet Nam da co nhung thay doi. Chiing toi thirc hien nghien ciru nay nham: danh gid ddc diem 1dm sdng, nguyen nhdn vd ket qua dieu tri viem ket mac cap.

II. DOI Tl/ONG VA PHUONG PHAP NGHIEN CUU

Nghien ciiu mo ta cat ngang, tren 91 benh nhan (167 mat) viem ket mac cap den kham va dieu tri tai phong kham Benh vien Mat Trung uang tii'

1/2009 den 9/2009. Tat ca cac benh nhan duoc lam cac xet nghiem soi nhuom phat hien vi khuan va nuoi cay va lam khang sinh do de xac dinh loai vi khuan va khang sinh thich hop. Cluing toi lam xet nghiem te bao hoc ket mac cho tat ca 91 BN de chan doan do vi khuan hay do virut, dira theo hinh anh te bao hoc ket mac: do vi khuan neu co chii yeu la bach cau da nhan, do virut neu co chii yeu te bao nhieu nhan va te bao thoai hoa nhan truong.

Cluing toi tien hanh dieu tri theo phac do: giai doan dau (trong 3-5 ngay) rua mat bang nuoc muoi sinh ly loai bo tiet to ket mac. Tra mat dung dich khang sinh (Cebemycin)l 0-15 lan/ngay, ma khang sinh (Oflovid) 2 lan/ngay; boc mang ket mac ngay I lan neu co. Giai doan tiep theo tiep tuc dieu tri nlur tren, neu nhiem triing giam thi giam so lan tra thuoc khang sinh (3 - 4 lan/ngay), diing them thuoc tra

mat co corticoid. Khi co kit qua khang sinh do thi diing khang sinh dac hieu theo khang sinh do.

Kham lai sau 3. 7 va 14 ngay. Ket qua dieu tri dugc coi la khoi khi BN het cac trieu chung lam sang.

IH. KET QUA NGHIEN CUTJ I. Dac diem lam sang

- Tuoi: benh xuat hien a moi lira tuoi nhung tap trung chii yeu d tuoi tir 17 den 40 tuoi (do VK la 58,7%. do virut la 62,5%).

- Gioi tinh: trong ca 2 nhom do VK va do virut, ty le benh d nam gidi (54,7% do VK va 56,3% do virut) ludn cao hon d nCr gidi (45,3% do VK va

43.8% do virut).

- Mat bi benh: ty le bi benh ca 2 mat cao han mdt mat, trong ca 2 nhdm do VK va do virut vdi ty le tuan tu la 81,3% va 93,7% so vdi 18,7%

va 6,3%.

- Thi lire trudc dieu tri: trong tdng sd 167 mat, sd mat cd thi lire >7/10 chiem ty le cao trong ca 2 nhdm nguyen nhan (VK 89,0% va virut 93,5%), duy nhat 1 mat cd thi lire <3/l 0 (0,7%).

- Thai gian tir khi bi benh den khi kham benh: den kham benh mudn > 5 ngay, chiem ty le cao nhat (53,8%). BN den kham ngay trong ngay dau bj benh la thap nhat (4,4%).

- Tilt to mil trong viem ket mac do VK chiem ty le cao: 98,7% (74 BN), khdng cd tiet to mil trong nguyen nhan do virut.

- Tiet to nhay chiem ty le cao trong viem ket mac do virut: 100% (16 BN), trong khi do VK cd 1 BN (1,4%).

- Sung ne mi chiem ty le cao, 76% do vi khuan va 75% do virut.

Sir dung thudc tnrdc khi den kham benh: hau het BN deu diing thuoc khang sinh tra tai mat: 88 BN (96,7%). 15 BN (16,5%) diing thudc cd corticoid tra tai mat.

2. Ket qua dieu tri

Thj Urc sau dilu tri (bang 4): trong nhom \ iem ket mac cap do VK co 1 mat thi lire <3 10 (0.7%). 2 mat (1.5%) thi lire tir 3 10-" 10 \ a da s6 (133 mat. Q".8%) co thi lire binh thuong >7 10. Viem ket mac cap do

- Ket qua dieu trj chung: toan bo 91 BN (100%) deu khoi sau dieu tri.

- Thdi gian dieu tri: ngay dieu trj trung binh cua viem ket mac cap do vi khuan la 10,6 ngay, do virut la 9,1 ngay. Ngay dieu trj ngan nhat la 6 ngay, dai nhat la 23 ngay (1 trudng hop viem ket mac do vi khuan cd bien chimg viem giac mac.

IV. BAN LUAN

Dac diim vi thi lire (ngay dau kham va dieu tri): trong tdng sd 167 mat bj benh thi nhdm thj lire

>7/10 chiem da sd: 121 mat (89%) d nhdm do VK va 29 mat (93,5%) d nhdm do virut. Duy nhat mdt mat (bien chiing viem loet giac mac do lau cau) cd thi lire thap nhat: BBT 0.3m (0.7%). Nhdm thi lire tir 3/10-7/10 cd 14 mat do VK (10,3%) va 2 mat do virut (6,5%). Ket qua ve thj lire trong nghien ciru ciia chung tdi phu bop vdi ket qua ciia cac nghien ciru khac: viem ket mac cap thudng khdng anh hirdng den thi lire [7].

Thai gian den kham benh: BN den kham benh trong ngay dau bi benh it nhat (4 BN, 4,4%). Theo chime tdi cd le BN tir mua thudc dieu tri hoac kham benh \ te co sd va khi benh nang len mdi den

benh vien. sd BN den kham benh >5 ngay

nhieu nhat (49 BN, 53,8%) (p<0,001).

Trieu chung ca ndng: trieu chirng dd mat va cam giac com rat mat cd ty le 100% trong nguyen nhan do VK cung nhu virut. Cac trieu chirng ca nang khac nhu ngira mat, chdi, chay nude mat cd ty le tuong duong nhau giiia 2 nhdm. Theo nghien ciin ciia Ting Huang, Yujuan Wang[12] thi chay nude mat thudng gap hon trong viem ket mac do virut.

Tiet td ket mac dang mil chii yeu gap trong viem ket mac cap do vi khuan chiem 98,7% (74 BN) va khdng gap trong nguyen nhan virut. Tiet td ket mac dang nhay xuat hien chii yeu trong viem ket mac do virut (16 BN - 100%) va chi cd d 1 BN (1,4%) trong viem ket mac do vi khuan (p<0,001).

Dau hieu thuc the: cuang tu ket mac va nhu gai xuat hien 100% cac BN, cac dau hieu xuat huyet duoi ket mac, phii ket mac va gia mac xuat hien trong nguyen nhan do VK (ty le 100%), khong thay co trong nguyen nhan virut. Dau hieu sung ne mi mat co ty le tuong duong nhau giua 2 nhom do VK (76%) va do virut (75%).

Trieu chirng toan than: khong co truong hop nao viem duang ho hap tren. dieu nay khac biet vai nghien ciiu ciia Hazel Everitt [5]: viem ket mac do virut thuong do adenovirus hoac picornavirus va thuong kem theo viem dudng hd hap tren.

Ket qua xdc dinh vi sinh vat gay benh:

Sd BN cd mdt loai vi khuan la cao nhat: 63/91 BN (69,2%). Trong sd cd mdt loai vi khuan thi 37 BN (40,7%) cd can khuan gram(+). Trong sd benh nhan cd phdi hop 2 loai VK thi cao nhat la nhdm phdi hop cau khuan grani(+) vdi true khuan gram(+) 24 BN (26,4%).

Cd 19/91 BN (20.9%) xac dinh dugc VK gay benh, thap hon so vdi mdt sd tac gia khac. VK trudc day thudng gap trong viem ket mac cap (Staphylo- coccus, Streptococcus pneumoniae va Haemophi- lus influenzae) thi ket qua nudi cay trong nghien ciiu ciia chimg tdi khdng moc. Ket qua trong nghien ciru ciia chting tdi cung phii hop vdi nghien cuu ciia Malhotra (2005-An Dd) vdi ty le nudi cay (+) la 20,4% [7], nhung thap ban nghien cii'u ciia Remco Rietveld (2004-Ha Lan) ty le phan lap dugc la 32%

[10], Hazel Everitt (2006-Anh) la 50% [4].

Loai VK cd ty le cao nhat la Staphylococcus saprophyticus 5,5% (5 BN), Corynebacterium 4,4%

(4 BN), Staphylococcus epidermidis va Flavimonas oryzihabitans la nhtr nhau: 3,3% (3 BN). Nghien ciru ciia Nguyen Hien [I]: cd 43,9% viem ket mac phan lap dirge VK, tu cau ty le cao nhat 51%, lien

cau 32,3%, Moraxella 7.4%. Ohnsman va Ritter- band (2007-My) thi ty le S. epidermidis 19,7% cao hon so vdi ket qua nghien ciru ciia chting tdi [8].

Theo Pham Thi Thu Lan (BV Mat Trung uong) tii- nam 1991-1996. khi nudi cay VK trong viem ket mac, tu cau cd ty le 10.6%, lien cau 1.3%, Moraxella 1,9%, true khuan mil xanh 0,4% va lau cau 17,9% va da ket luan tu cau la tac nhan gay benh hay gap [2]. Theo chiing tdi cd the tinh hinh VK gay benh khac nhau d tirng viing dia du, ciing cd the do each lay benh pham va dimg mdi trudng nudi cay chua chuan hoac do da sd BN da diing khang sinh dieu tri trudc khi den vien, nen ket qua nudi cay va phan lap VK cdn d ty le thap.

Xet nghiem te bao hgc ket mac (TBHKM):

cluing tdi lira chgn xet nghiem TBHKM de chan doan viem ket mac do virut hay do VK, vi xet nghiem nay thirc hien dugc tai BV Mat Trung uong, ky thuat khdng pluic tap, chi phi xet nghiem khdng cao va BN de dang chap nhan. Thirc te ly tuong nhat de xac dinh virut la nudi cay va phan lap bang xet nghiem mien dich huynh quang hoac xet nghiem PCR. Do thai gian nghien ciru va trang thiet bj khoa Xet nghiem benh vien, nen chimg tdi lam xet nghiem TBHKM de sa bd khang dinh nguyen nhan viem ket mac do virut: ggi y viem ket mac do VK khi cd nhieu bach cau da nhan trung tinh va virut khi cd nhieu lympho bao, te bao bieu md thoai hda nhan truang va te bao bieu md nhieu nhan.

Theo ket qua nghien ciru thi 75 BN cd ty le bach cau da nhan trung tinh cao 98,9% dugc chan doan la viem ket mac do vi khuan. Te bao bieu md thoai hda nhan truong va te bao bieu md nhieu nhan chiem ty le 100% d 16 BN viem kit mac do virut, nhirng te bao nay khdng gap trong nhdm viem ket mac do vi khuan.

Kit qua dilu trj chung: 91 BN (100%) dlu khdi benh. Kit qua nay cung phii hgp vdi ket qua nghien

ciru cua cac tac gia Poonam Shasma va Gian Singh (An Do): viem ket mac rat it khi co bien chirng. ty le khoi cao [9J.

Thdi gian dieu tri: ngay dieu tri dai nhat la 23 ngay va ngan nhat: 5 ngay. Ngay dieu tri trung binh viem ket mac cap do VK (10,6 ngay), do virut (9.1 ngay). Ket qua nay phii hop voi mot so nghien ciiu cho rang: viem ket mac thuong khoi trong 10 den

14 ngay [3]. [6]. [11].

V. KET LUAN

1. Dac diem lam sang va xet nghiem

- Benh viem ket mac cap la benh thuong gap.

nguyen nhan do VK, chiem 82,4% va do virut 17,6%.

- Benh gap nhieu nhat d lira tuoi trung nien I 7-40 tudi (48,3%).

- Benh nhan thudng den kham benh mudn (sau 5 ngay: 53,8%).

- Tiet td mu gap trong nguyen nhan do vi khuan:

98.7%.

- Tiet td nhay gap trong nguyen nhan do virut:

100%.

- Vi khuln thudng gap nhat la VK gr(+). Staphylo- coccus saprophyticus la loai vi khuan gay benh cd ty le cao nhat (5,5%).

- 75/91 BN (82,4%) viem ket mac cap do VK.

ket qua xet nghiem te bao hoc ket mac chii yeu la bach cau da nhan trung tinh va 16/91 BN (17.6%) viem ket mac cap do virut, ket qua xet nghiem te bao hoc ket mac chu yeu la lympho bao. te bao bieu md thoai hda nhan truong va te bao bieu md nhieu nhan.

2. Ket qua dieu tri

- 100% BN viem ket mac cap dieu tri khdi bang phuong phap ndi khoa.

- Ngay dieu tri trung binh viem ket mac cap do VK la 10,6 ngay, do virut la 9,1 ngay.

- Ngay dieu tri trung binh ciia nhdm benh nhan den kham benh ngay trong ngay dau la ngan nhat (9 ngay), nhdm den kham benh mudn > 5 ngay la dai nhat (10,6 ngay).

- Bien chirng viem giac mac cd lien quan vdi viec sir dung thudc cd corticoid tra mat chiem ty le cao (5 mat, 33,3%).

SUMMARY

STUDY OF CHARACTERISTIC. AETIOLOGY AND TREATMENT OF ACUTE CONJUNCTIVITIS IN NATIONAL INSTITUTE OF OPHTHALMOLOGY

Purpose: to describe the characteristics, aetiology and evaluate the treatment result of acute conjunctivitis.

Methods: 91 patients (16' eyes), aged 17-40. were diagnosed acute conjunctivitis. Conjunctival cultures and scrapings were performed.

Results: 82.4% of patients were diagnosed bacterial conjunctivitis and 17.4% were viral conjunctivitis.

Conjunctival cultures were positive in 20.9%. 100% of patients were treated successfully.

Conclusion: common cause of conjunctivitis is bacteria. The clinical features of bacterial conjunctivitis were more serious than these of viral conjunctivitis.
