POLIFARMASI DAN INTERAKSI OBAT PADA PASIEN USIA LANJUT RAWAT JALAN DENGAN PENYAKIT METABOLIK DI RUMAH

ABSTRAK
Proses penuaan menyebabkan penurunan fungsi organ pada usia lanjut sehingga mudah terserang berbagai penyakit. Penyakit metabolik merupakan penyakit yang berkaitan dengan peningkatan usia seperti hipertensi, diabetes melitus, dislipidemia dan obesitas. Penyakit-penyakit tersebut ditangani dengan terapi obat yang sifatnya polifarmasi sehingga bisa menyebabkan resiko efek samping obat meningkat.
Penelitian ini bertujuan untuk mengetahui jumlah interaksi obat-obat yang terjadi, pola mekanismenya, tingkat keparahannya, hubungan antara jumlah interaksi dengan jumlah obat, hubungan antara jumlah interaksi dengan jumlah diagnosis.
Penelitian ini dilakukan secara restropektif dengan menggunakan rekam medik pasien usia lanjut dengan penyakit metabolik yang di rawat jalan di Rumah Sakit Umum Pusat Haji Adam Malik Medan.
Sebanyak 328 sampel yang termasuk kedalam kriteria inklusi diperoleh data bahwa jumlah interaksi obat-obat yang terjadi cukup tinggi sebesar 78,96%. Pola mekanisme yang terbanyak adalah farmakokinetik (63,6%) dengan tingkat keparahan yang terbanyak adalah moderat (69,8%). Penelitian ini menunjukan
adanya korelasi antara jumlah interaksi dengan jumlah obat (r = 0,728; p = 0,0001), jumlah interaksi dengan jumlah diagnosis (r = 0,264; p = 0,0001).
Penelitian ini dapat disimpulkan bahwa interaksi yang terjadi pada pasien usia lanjut dengan penyakit metabolik cukup tinggi, pola mekanisme terbanyak adalah farmakokinetik, tingkat keparahan terbanyak adalah moderat, dan ada korelasi yang signifikan antara jumlah interaksi dengan jumlah obat dan jumlah diagnosis.

ABSTRACT
The aging process causes a decrease in organ function in elderly people so prone to various diseases. Metabolic disease is a disease that is associated with increasing age such as hypertension, diabetes mellitus, dyslipidemia and obesity. Such diseases are treated with drug therapy polypharmacy nature that can cause increased risk of drug side effects.
This study aiims to determine the number of drug-drug interaction that occur, the pattern of the mechanism, its severity, the relationship between the number of interactions with a number of drugs, the relationship between the number of interactions with a number of diagnoses.
This study was conducted using a restropective medical records of elderly patients with metabolic diseases in outpatient RSUP HAM Medan.
A total of 328 samples were included in the inclusion criteria data showed that the number of drug-drug interactions that occur quite high at 78.96%. The pattern is the mechanism that most pharmacokinetic (63.6%) with the highest severity was moderate (69.8%). This study shows a correlation between the number of interactions with a number of drugs (r = 0.728; p = 0.0001), the number of interactions with a number of diagnoses (r = 0.264; p = 0.0001).
It can be concluded that the interaction that occurs in elderly patients with metabolic diseases is high enough, the pattern is most pharmacokinetic mechanisms, most are moderate severity, and no significant correlation between the number of interactions with a number of drugs and the number of diagnoses.
