GENERAL TERMS AND CONDITIONS FOR CONTRACT
2. RUANG LINGKUP PEKERJAAN SCOPE OF WORK
2.1. DESKRIPSI LINGKUP PEKERJAAN SCOPE OF WORK DESCRIPTION Kontrak ini merupakan proyek yang meliputi
pekerjaan sipil yang berkaitan dengan konstruksi lahan basah, dan pekerjaan lainnya. Pekerjaan utama yang diberikan kepada Kontraktor adalah untuk menyelesaikan pekerjaan pada lokasi proyek tersebut dengan menyediakan tenaga kerja, peralatan konstruksi, perlengkapan keselamatan, armada operasional, alat berat, dan berbagai peralatan pendukung lainnya selama proyek berjalan dari mulai persiapan sampai selesai serah terima proyek.
This contract is a project that includes civil works related to the construction of wetlands and other works. The main work given to the Contractor is to complete work at both project sites by providing manpower, construction equipment, safety equipment, operational fleet and various other supporting equipment during the project from preparation to completion of project handover.
Pada prinsipnya aktivitas utama dalam kontrak ini adalah untuk melakukan kegiatan pengadaan barang, fabrikasi, konstruksi dan instalasi sesuai dengan gambar yang telah mendapatkan cap issued for construction. Jika ada pekerjaan yang tidak dijelaskan cukup rinci dikontrak, maka Kontraktor wajib mengacu pada gambar Issued for Construction (IFC) sebagai acuan pekerjaan yang harus dilakukan dan mendiskusikannya dengan perwakilan perusahaan.
In principle, the main activity in this contract is to engage in goods procurement, fabrication, construction, and installation activities which have been issued for construction. If any works that is not describe/mentioned elsewhere in the contract, then Contractor must refer to the Issued for Construction (IFC) drawing as reference of the work to be done and discuss it with company representatives..
Kontraktor harus menghitung volume pekerjaan berdasarkan berdasarkan gambar IFC, lalu di ajukan dalam dokumen tender. Kontraktor bertanggungjawab penuh terhadap akurasi dari perhitungan tersebut. Perusahaan tidak akan menyetujui adanya perubahan volume pekerjaan
Contractor shall calculate the quantity of work Based on the IFC drawings, to be proposed in the tender document. Contractor is fully responsible to the accuracy of the calculation. Company will not approve change of quantity during contract period except if the change is caused by Company request
Selama Kontrak berlangsung, Kontraktor wajib untuk melengkapi segala persyaratan yang sesuai dengan regulasi pemerintah dan Perusahaan.

During Contract execution, Contractor is required to complete all the requirements in accordance with government and enterprise regulations General scope of work that to be provided by Contractor for this Contract:
Perencanaan, persiapan, manajemen proyek, dan teknik yang diperlukan dan gambar seperti drawing fabrikasi, sesuai keputusan Perusahaan.
Planning, preparation, project management, and necessary engineering and drawings such as shop drawing (fabrication drawing), as per Company’s judgment.
Menyediakan semua tenaga kerja yang diperlukan untuk dukungan operasi peralatan, termasuk namun tidak terbatas pada pekerja kasar, rigger, flag-man, pengawasan, keamanan, dan lain-lain sebagaimana permintaan Perusahaan.
Provide all necessary manpower for equipment operation support, including but not limited to helper, rigger, flag man, supervision, safety, and others as Company request.
Menyediakan Alat Pelindung Diri (APD) dan valid Medical Check-Up (MCU) untuk personil sebelum bekerja di fasilitas Perusahaan.
Provide Personnel Protecting Equipment (PPE) for personnel and provide valid personnel Medical Check-Up (MCU) prior working on site.
Menyediakan sertifikasi sesuai peraturan Pemerintah yang diperlukan untuk peralatan dan operator termasuk riggers, Juru Las (Welder) dan sebagainya oleh EBTKE atau lembaga pemerintahan lainnya (MIGAS, DEPNAKER) sesuai degan peraturan pemerintah yang berlaku.
Provide required Government certification for equipment and operators including riggers, welders and others by EBTKE or other government agencies (MIGAS, DEPNAKER) in accordance with government regulations.
Khusus untuk pesawat angkat dan angkut beserta operatornya, jika telah memiliki sertifikasi (SILO, SIO, SKPP) dari lembaga pemerintahan lainnya selain EBTKE (MIGAS, DEPNAKER), Kontraktor wajib untuk meyerahkan bukti Sertifikat tersebut ke Dirjen EBTKE Direktorat Panas Bumi untuk mendapatkan endorsement yang dibuktikan dengan surat pendukung dari EBTKE sebelum alat tersebut dan operatornya melakukan pekerjaan.
Especially for lifting and moving edquipment along with the operator. If it has been certified (SILO, SIO, SKPP) from a government agency other than EBTKE (MIGAS, DEPNAKER), the Contractor is obliged to to submission approved certificate to Dirjen EBTKE Direktorat Panas Bumi in order to get the endorsement letter from EBTKE before the lifting and moving equipment and the operator is doing the work.
Menyediakan semua izin Pemerintah yang diperlukan untuk transportasi selama mobilisasi dan demobilisasi.
Provide all required Government permits for transportation during mobilization and demobilization
Menyediakan dokumen konstruksi yang diperlukan termasuk namun tidak terbatas pada hal berikut:
Provide applicable construction documents including but not limited to the following:
a. JSA/HIRA, Rencana Pengangkatan (Lifting Plan), metode kerja, Rencana Perjalanan, dan Izin untuk Kerja (PTW). Tingkat persetujuan dokumen tersebut harus ditetapkan pada kick
a. JSA/HIRA, Lifting Plan, work method, Travel Plan, and Permit to Work (PTW).

c. Equipment routine inspection and maintenance record.
d. Catatan pemeliharaan peralatan dari Dealer Resmi atau Pusat Layanan untuk truk ringan, dump truck, truk kargo.
d. Equipment maintenance record from Authorized Dealer for light trucks, dump trucks, cargo trucks.
e. Laporan jumlah peralatan dan jam pemakaian per hari. Laporan tersebut harus disetujui oleh Pengawas Konstruksi Perusahaan dan Project Manager Perusahaan.
e. Report of actual daily equipment quantity and hours. The report shall be approved by Company Construction Supervisor and Company Project Manager.
f. Mempertahankan kinerja keselamatan sesuai dengan Peraturan Keselamatan Perusahaan dan Praktek Kerja Selamat yang berlaku.
f. Maintain the safety performance as applicable Company Safety Regulations and Safe Work Practices.
Semua bahan bakar, operator, pemeliharaan dan suku cadang merupakan biaya yang ditanggung Kontraktor.
All Fuel, operators, maintenance, and spare parts shall be at Contractor cost.
Penggunaan bahan bakar harus sesuai dengan peraturan pemerintah Indonesia mengenai penggunaan industri dan bahan bakar komersial yang berlaku.
Fuel usage shall comply with prevailing Indonesian Government regulations regarding the use of Industrial and Commercial fuel.
Kontraktor harus memastikan bahwa semua alat dan / atau peralatan yang digunakan untuk layanan harus dalam kondisi baik bebas dari cacat dan memenuhi praktik industri, standar, dan peraturan/perundangan, untuk melakukan pekerjaannya
The contractor shall ensure that all tools and/or equipment utilized for the services must be in a good condition free from defect and fulfil the Industrial practices standard and codes to perform the services
Kontraktor harus menyediakan transportasi dengan biaya Kontraktor dari titik Penginapan personil ke lokasi kerja, di dalam lokasi kerja, dan dari lokasi bekerja kembali ke titik penginapan, dan juga untuk mendukung staf untuk melakukan peran mereka.
Keselamatan transportasi mengacu pada prosedur perusahaan.
Contractor shall provide transportation at Contractor cost from personnel lodging point to working location, inside working location, and from working location back to lodging point, and also for supporting staff to perform their role.
Transportation safety shall refer to Company Procedure.
Sopir-sopir harus merupakan professional drivers, tidak merangkap dengan pekerjaan lain selain mengemudi, dan tidak ada pekerja lain yang diizinkan untuk mengemudi kendaraan atau mengoperasikan peralatan. Sopir harus memiliki
Drivers shall be Professional drivers. No driver shall do other work than driving. and no worker shall be allowed to drive vehicles or operate equipment. Driver shall have valid permit from Company and driving license as per government regulation.
