GENERAL TERMS AND CONDITIONS FOR CONTRACT
1. LINGKUP PEKERJAAN UMUM GENERAL SCOPE OF WORK Kecuali adanya catatan tertentu, lingkup umum
pekerjaan untuk jenis pekerjaan "Pemasokan dan Pemasangan” atau “Pemasokan, Fabrikasi dan Pemasangan" atau "Pemasokan, Fabrikasi dan Konstruksi” atau “Pemasokan Material dan Pelaksanaan Jasa apapun” akan meliputi namun tidak terbatas pada sebagai berikut:
1.1. Except otherwise noted, the general scope of work for “Supply and Install” or “Supply, Fabricate, and Install” or “Supply, Fabricate, and Construct” or “Supply Material and Perform any Services” type of work shall include but not limited to the following:
a) Perencanaan, persiapan, manajemen proyek, dan engineering dan gambar-gambar teknik yang diperlukan seperti shop drawing (gambar fabrikasi), sesuai pertimbangan Perusahaan.
a) Planning, preparation, project management, and necessary engineering and drawings such as shop drawing (fabrication drawing), as per the Company’s judgment.
b) Jika diminta oleh Perusahaan, mempersiapkan kantor di lokasi lapangan, papan buletin, tempat fabrikasi, area penyimpanan material, tempat parkir peralatan yang telah disetujui oleh Perusahaan, Perusahaan akan memberikan saran, daerah lokasi yang tersedia dan sarana yang tersedia untuk Kontraktor
b) If required by the Company, prepare the site office, bulletin board, fabrication shop, material laydown area, and parking equipment as approved by the Company, The Company will advise on the available locations, areas, and utilities available to the Contractor
c) Menyediakan sarana termasuk tetapi tidak terbatas pada air, udara, dan listrik di lokasi.
c) Provide utilities including but not limited to water, air, and electricity at the site.
d) Menyediakan, menangani, dan transportasi material, termasuk pemuatan/ pembongkaran ke lokasi pekerjaan

e) Provide all necessary manpower, including supervision, safety, project control, drivers, etc.
f) Menyediakan semua bahan habis pakai, peralatan dan perlengkapan yang diperlukan.
f) Provide all consumables, required tools, and equipment.
g) Spesifikasi alat dan perlengkapan harus mengacu pada spesifikasi "Penyewaan Alat Berat" yang diatur dalam Exhibit ini untuk semua atau baik untuk harga unit maupun instruksi pekerjaan sewa,
g) Specification of equipment shall refer to the specification of “Heavy Equipment Rental”
in this Exhibit for all/both unit rates and rental work instruction,
h) APD untuk karyawan. Mengacu kepada pedoman dan Spesifikasi APD sebagaimana telah disampaikan.
h) PPE for personnel. See PPE Guideline and PPE Specification as provided.
i) Menyediakan setidaknya satu unit system pemantau H2S yang dapat dipindahkan seperti Toxiclip pada setiap lokasi pekerjaan
i) Provide at least one unit of portable H2S monitoring system such as Toxiclip at each location of work
j) Menyediakan peralatan SCBA (Self Contained Breathing Apparatus) yang diperlukan dan
j) Provide necessary SCBA (Self Contained Breathing Apparatus) equipment and
terbatas pada hal berikut: limited to the following:
1) Membuat dan meminta persetujuan/
approval untuk JSA/HIRA. Rencana Pengangkatan/Lifting Plan, Rencana Perjalanan, dan Perijinan kerja. Tingkat persetujuan harus ditetapkan selama pertemuan kick off atau selama pelaksanaan pekerjaan.
1) Develop and obtain approvals of JSA/HIRA. Lifting Plan, Travel Plan, and Permit to Works. Approval level shall be defined during the kick-off meeting or the performance of work.
2) Laporan Mingguan untuk Insiden Keselamatan/near miss. Laporan ini harus disetujui oleh perwakilan SHE Perusahaan.
2) Weekly report of Safety Incidents/near misses. The report shall be approved by the Company SHE representative.
3) Laporan Kemajuan (progress) mingguan termasuk perbandingan antara kemajuan yang sebenarnya dengan rencana penyelesaian. Laporan ini harus disetujui oleh Pengawas Konstruksi Perusahaan dan Manajer Proyek Perusahaan.
3) Weekly progress report including the comparison of actual progress with the planned completion. The report shall be approved by Company Construction Supervisor and Company Project Manager.
4) Nilai mingguan hasil dari pekerjaan (earned value) yang telah dilakukan. Laporan ini harus disetujui oleh Pengawas Konstruksi Perusahaan dan Manajer Proyek Perusahaan.
4) Weekly earned value of the works. The report shall be approved by Company Construction Supervisor and Company Project Manager.
5) Laporan mingguan pemakaian bahan aktual.
Laporan ini harus disetujui oleh Pengawas Konstruksi Perusahaan dan Manajer Proyek Perusahaan.
5) Report of actual weekly material usage. The report shall be approved by Company Construction Supervisor and Company Project Manager.
6) Laporan mingguan jumlah tenaga kerja dan jam kerja aktual. Laporan ini harus disetujui oleh Pengawas Konstruksi Perusahaan dan Manajer Proyek Perusahaan.
6) Report of actual weekly manpower quantity and hours. The report shall be approved by Company Construction Supervisor and Company Project Manager.
7) Laporan mingguan jumlah aktual peralatan dan jam pemakaian. Laporan ini harus disetujui oleh Pengawas Konstruksi Perusahaan dan Manajer Proyek Perusahaan.
7) Report of actual weekly equipment quantity and hours. The report shall be approved by Company Construction Supervisor and Company Project Manager.
8) Laporan akhir pemakaian material aktual.
Laporan ini harus disetujui oleh Pengawas Konstruksi Perusahaan dan Manajer Proyek Perusahaan.
8) Report of actual final material usage.
The report shall be approved by Company Construction Supervisor and Company Project Manager.
9) Laporan akhir jumlah tenaga kerja dan jam kerja aktual. Laporan ini harus disetujui oleh Pengawas Konstruksi Perusahaan dan Manajer Proyek Perusahaan.

13)QA/QC inspection report. The report shall be approved by Company Construction Supervisor and Company Project Manager.
14)Laporan pengujian (testing) dan komisioning.
14)Testing and commissioning report l) Melakukan pekerjaan berdasarkan Standar yang
telah ditentukan dan/atau Spesifikasi yang telah dijelaskan dalam Paket Pekerjaaan (Work Package), termasuk inspeksi, pengujian, komisioning, dan penyelesaian punch list commissioning seperti yang dijelaskan dalam Kick-off Meeting
l) Perform the work based on the specified Standard and/or Specification described in the Work Package, including inspection, testing, commissioning, and the completion of the commissioning punch list, as described in the Kick-off Meeting
m) Menyediakan dan memasang bangunan sementara dan/atau fasilitas yang diperlukan, untuk menyelesaikan pekerjaan- (contoh Shelter).
m) Supply and install required temporary structures, and/or facilities to complete the work. (i.e. Shelter)
n) Menyediakan alat-alat dan perlengkapan dan melakukan semua pemeriksaan yang diperlukan dan pengujian untuk peralatan dan sistem yang dipasang, harus melakukan atau paling tidak disetujui oleh pengawas pihak ke-3 & disetujui oleh perwakilan Perusahaan.
n) Provide tools and equipment and perform all necessary inspection and testing for installed equipment and system, should be performed or at least approved by 3rd party inspector & approved by Company representative.
o) Melakukan perbaikan pekerjaan dengan biaya Kontraktor untuk pekerjaan yang tidak sesuai dengan spesifikasi.
o) Repair the work at Contractor’s expense for non-conformance work.
p) Melakukan perbaikan dan/atau penggantian material, peralatan, dan bagian lain yang rusak yang disediakan oleh Perusahaan yang disebabkan oleh metode Kontraktor yang tidak tepat penanganan dan cara kerjanya

1.2. Lingkup Umum pekerjaan dan kebutuhan untuk sewa/ Rental peralatan yang di atur dalam kontrak ini:

a) Menyediakan semua tenaga kerja yang diperlukan untuk mendukung operasi peralatan, termasuk namun tidak terbatas pada tenaga pembantu, rigger, flag man, pengawasan, keselamatan, dan pengontrolan proyek yang termasuk dalam biaya sendiri Kontraktor.
a) Provide all necessary manpower for equipment operation support, including but not limited to helper, rigger, flag man, supervision, safety, and project control which is included in Contractor cost.
b) Menyediakan APD untuk Karyawan. Mengacu kepada Exhibit Kontrak.

c) Menyiapkan sertifikasi yang diwajibkan oleh Pemerintah untuk peralatan dan operator termasuk riggers

e) Menyediakan perlengkapan dan peralatan yang dibutuhkan untuk tenaga operator perlengkapan termasuk tetapi tidak terbatas pada sling, penjerat, kait, bantalan rigger crane, penahan roda untuk kendaraan, tali, terpal
e) Provide equipment with tools required for equipment operator including but not limited to slings, shackles, hooks, crane rigger pads, wheel chocks for vehicles, ropes, tarpaulin
f) Spesifikasi peralatan harus seperti yang ditentukan dalam Exhibit kontrak.
f) Specification of equipment shall be as specified in the Contract exhibit.
g) Peralatan harus dalam kondisi operasi yang baik dan aman lengkap dengan operator, bahan bakar, perawatan, suku cadang.
g) Equipment shall be in good and safe operating condition complete with operators, fuels, maintenance, and spare parts.
h) Semua truk kategori ringan, truk sampah, truk kargo, harus dikelola oleh Dealer Manufaktur Resmi atau Pusat Layanan sesuai dengan Pedoman Pemeliharaan Manufaktur
h) All light trucks, dump trucks, and cargo trucks, shall be maintained by the Manufacturer's Authorized Dealer or Service Centre as per Manufacturer's Maintenance Manual

4) Catatan pemeliharaan peralatan dari dealer resmi atau Pusat layanan Servis untuk truk ringan, truk sampah, dan truk kargo.
4) Equipment maintenance record from Authorized Dealer or Service Centre for light trucks, dump trucks, and cargo trucks.
5) Laporan aktual harian jumlah dan jam pemakaian peralatan. Laporan ini harus disetujui oleh Pengawas Konstruksi Perusahaan dan Manajer Proyek Perusahaan.
5) Report of actual daily equipment quantity and hours. The report shall be approved by Company Construction Supervisor and Company Project Manager.
l) Mempertahankan kinerja keselamatan sesuai dengan Peraturan Keselamatan Perusahaan yang berlaku dan Praktik-praktik dasar Keselamatan Kerja
l) Maintain the safety performance as applicable Company Safety Regulations and Fundamental Safe Work Practice m) Kontraktor harus mematuhi peraturan pemerintah

m) The contractor needs to comply with government regulations and Company SHE requirements.
