“KORELASI ANTARA SKOR CHILD-PUGH DENGAN GASTROPATI HIPERTENSI PORTAL PADA PENDERITA SIROSIS HATI”

Latar Belakang: Gastropati hipertensi portal (GHP) adalah kelainan patologis pada mukosa lambung yang bisa didapatkan pada penderita sirosis hati dan dapat mengakibatkan perdarahan akut maupun kronik. Patogenesis GHP masih kontroversial dan sampai saat ini belum ada kesepakatan tentang pengaruh derajat kerusakan hati terhadap GHP.
Tujuan: Untuk mengetahui korelasi antara derajat keparahan penyakit hati yang tergambar melalui skor Child-Pugh dengan GHP dan untuk mengetahui akurasi skor Child-Pugh dalam memprediksi kejadian GHP pada penderita sirosis hati.
Metode: Studi ini adalah studi potong lintang bersifat analitik. Dilakukan pencatatan data demografis dan klinis, pemeriksaan laboratorium (albumin, INR, bilirubin) untuk mengkalkulasi nilai Child-Pugh, serta dilakukan tindakan esophagogastroduodenoscopy (EGD) untuk menilai GHP. Metode statistik yang digunakan untuk menentukan korelasi adalah analisis korelasi Spearman. Nilai cut-off skor Child-Pugh ditentukan dengan analisis receiver operating characteristics (ROC) dilanjutkan dengan uji performa diagnostik.
Hasil: Dari 41 subjek penelitian, didapati berturut-turut pasien dengan skor Child-Pugh A,B, dan C adalah 8, 18, dan 15 orang. Dari hasil endoskopi didapatkan berturut-turut pasien tanpa GHP, GHP ringan, dan GHP berat adalah 10, 19 dan 12 orang. Terdapat korelasi yang positif antara skor Child-Pugh dengan derajat GHP secara endoskopi (r = 0,556; p=0,0001). Di antara komponen yang membentuk skor Child-Pugh, komponen asites dan bilirubin adalah komponen yang memiliki korelasi positif terhadap kejadian gastropati hipertensi portal. Dengan nilai cut-off ≥7, skor Child-Pugh memiliki sensitivitas 87,1%, spesifisitas 40%, PPV 81,8%, NPV 50%, LR(+) 1,45, LR(-) 0,32, akurasi diagnostik 75,6%, dan AUROC 0,65 (95% CI:0,437-0,860; p=0,163)
Kesimpulan: Skor Child-Pugh memiliki korelasi positif dengan derajat gastropati hipertensi portal, di mana semakin tinggi skor Child-Pugh maka akan semakin tinggi derajat gastropati hipertensi portal.

“CORRELATION BETWEEN CHILD-PUGH SCORE AND HYPERTENSIVE PORTAL GASTROPATHY IN LIVER CIRRHOTIC
PATIENTS”
Background: Hypertensive portal gastropathy (PHG) is a pathologic appearance on the gastric mucosa which can cause acute or chronic bleeding in liver cirrhotic patients. The pathogenesis of PHG is controversial and there is no agreement on the effect of liver dysfunction to PHG.
Aim: To evaluate correlation between liver dysfunction severity that is assessed by Child-Pugh score and PHG in liver cirrhotic patients.
Methods: This is an analytic cross sectional study. The demographics and clinical data were recorded, laboratory tests were performed (albumin, INR and bilirubin) to calculate Child-Pugh score, and patients underwent esophagogastroduodenoscopy (EGD) to assess PHG. Correlation between Child-Pugh score and PHG were tested with Spearman correlation analysis. Cut-off value of Child-Pugh score was determined with receiver operating characteristics (ROC) analysis which is followed by diagnostic performance analysis.
Results: From 41 subjects, there were 8, 18, 15 patients with Child-Pugh class A,B, and C respectively. On endoscopic examination, there were 10,19, and 12 patients without PHG, with mild PHG, and with severe PHG respectively. There is a positive correlation between Child-Pugh score and the severity of PHG (r=0,556; p=0,0001). Among Child-Pugh score components, ascites and bilirubin showed positive correlation with PHG. Using cut off point of≥7, Child Pugh score showed 87,1% sensitivity, 40% specificity, 81,8% PPV,50% NPV, 1,45 LR(+), 0,32 LR(-), accuracy diagnostic was 75,6% and the AUROC was 0,65 (95% CI:0,437-0,860; p=0,163)
Conclusion: Child-Pugh score has a positive correlation with severity of PHG, in which higher Child-Pugh score is correlated with more severe PHG.
