SURAT PERMOHONAN KESEDIAAN BERPARTISIPASI DALAM PENELITIAN

Bersama ini kami mohon kesediaan Ibu/Saudara untuk berpartisipasi sebagai subyek penelitian kami yang berjudul:
Tingkat Akumulasi Plak Gigi, Kalkulus Gigi, dan Keradangan Gingiva pada Perempuan Paskamenopause
dengan tujuan untuk: Menganalisis tingkat akumulasi plak gigi, kalkulus gigi, dan keradangan gingiva pada perempuan pascamenopause di wilayah Bekasi.
Dalam penelitian tersebut kepada Ibu/Saudara akan dilakukan: 1. Wawancara mengenai kebiasaan membersihkan gigi dan mulut 2. Pemeriksaan gigi-gigi rahang atas dan bawah
Adapun ketidaknyamanan yang akan dialami selama prosedur penelitian tersebut adalah: Saat pemeriksaan gigi dan pengisian kuesioner.
Semua tindakan tersebut membutuhkan waktu sekitar 15-20 menit.
Namun keuntungan menjadi subjek penelitian juga ada yaitu: Untuk menambah pengetahuan tentang gejala-gejala serta perubahan-perubahan yang terjadi pada periode pascamenopause dan untuk mengetahui status kebersihan mulutnya agar subjek termotivitasi untuk menjaga kebersihan gigi dan mulutnya.
Penelitian ini tidak dipungut biaya (gratis).
Jika Ibu/Saudara bersedia, surat Pernyataan Kesediaan Menjadi Subjek Penelitian terlampir harap ditandatangani dan diserahkan kembali.
Perlu Ibu/Saudara ketahui bahwa surat kesediaan tersedia tidak mengikat dan Ibu/Saudara dapat mengundurkan diri dari penelitian ini kapan saja selama penelitian berlangsung.
