The Board of Regents, upon the recommendation of the President of the University, approved at its 862nd meeting on 26 June 1975 the following measures by way of providing medical assistance to the low-salaried em- ployees of the University:

1. Appropriation of the amount of forty thousand pesos (P40,000.00) to be used as a hospitalization fund supplementary to the Medicare benefits of low-salaried personnel.

2. Reduction of the annual phyaicul and medical exam- ination fee paid by low-salaried employees: from PSO.OO to only 1'5.00.

For the implementation of the above twin measures, the following regulations arc hereby adopted and issued:

1. "Low-salaried employees" shall mean administrative personnel whose basic salary is not more than P7,236.00 per annum and academic non-teaching personnel with basic salary not exceeding ·P7,212.00 per annum.

2. A qualified employee shall be entitled to avail him- self of the hospitalization fund as specified in these regu- lations, and likewise his dependents as defined in Bureau of Internal Revenue Form No. 1701.

3. A qualified employee and his dependents as defined above shall be entitled in each case of hospitalization to up to P250.00 worth of actual and necessary hospitaliza- tion expenses over and above the Medicare benefits;

provided that in no case shall an individual qualified to enjoy these benefits be entitled to more than f500.00 worth of actual and necessary hospitalization expenses per year.

4. Except in emergency cases or upon certification by the Director of the U .P. Health Service of the necessity for confinement elsewhere, availment of the benefits specified in these regulations shall preferably be enjoyed by qualified persons who are confined in the U.P. Health Service.

5. These regulations shall take effect immediately.

The Philippine Medical Care Commission approved un- der its Resolution No. 74-614 dated December 4, 1974, amended by Resolution No. 75-638 dated February 19, 1975, the compulsory coverage of casual, temporary, con- tractual. emergency and substitute employees in the Gov- ernment service for Medicare purposes effective January 1, 1975.

Accordingly, the required Medicare contributions pur- suant to Section 21 of Republic Act 6111 will be deducted monthly beginning January 1, 1975 from the salaries of these employees. They are, therefore, requested to fill up

the Information-for-Medicare-Membership form for vali- dation by the GSIS Medicare Department and issuance of Medicare passbooks.

Likewise, all University personnel who have previously submitted their Information-for-Medicare-Membership forms shall fill up new ones in three (3) copies for updating and validation. One validated copy will be re- turned to the employee to be presented to the hospital concerned with the passbook whenever a member avails of his Medicare benefits. This will avoid the filling up of such form every time one makes use of his Medicare membership. Those who have not been issued their Medi- . care passbooks are requested to submit their names to the Office of Administrative Personnel Service (GAPS) so that they may be given their passbooks. Information- for-Medicare-Membership forms are available at GAPS.

Please circularize this Memorandum to all personnel of your Unit.

Upon the recommendation of the President and as ap- proved by the Board of Regents at its 862nd meeting on 26 June 1975, the following units are to be referred to by the names listed hereunder:

1. U.P. College Baguio 2. U.P. College Cebu 3. U.P. College Iloilo 4. U.P. College Manila 5. U.P. College Tacloban

The University of the Philippines at Los Bailoe shall he called by that name as heretofore.

The unit in the south in process of being planned shall come by the name of the University of the Philippines in the Visaya.s.

Please be guided accordingly.
