IDENTIFIKASI DRUG RELATED PROBLEMS PADA PENYAKIT GINJAL KRONIK DENGAN PENDERITA ANEMIA DI INSTALASI
RAWAT INAP RSUP H. ADAM MALIK MEDAN
ABSTRAK
Drug Related Problems(DRPs) adalah kejadian yang tidak diinginkan pasien terkait terapi obat yang secara nyata maupun potensial berpengaruh terhadap outcome yang diinginkan pasien. Penyakit ginjal kronik (PGK) kini telah menjadi masalah kesehatan serius di dunia, karena menyebabkan berbagai komplikasi diantaranya pasien memerlukan polifarmasi yang dapat mengakibatkan DRPs sehingga menurunkan kualitas hidup pasien.
Tujuan penelitian ini untuk mengetahui angka kejadian DRPs pada terapi pengobatan PGK (n=20). Penelitian ini menggunakan metode Strand, et al dengan kategori DRPs yaitu indikasi tanpa obat, obat tanpa indikasi, obat salah, dosis obat kurang, dosis obat berlebih, reaksi obat merugikan, dan interaksi obat. Pengambilan data dilakukan secara prospektif terhadap rekam medis pasien. Kriteria subjek penelitian meliputi pasien diagnosis PGK dengan penyakit anemia pada rawat inap di Haji Adam Malik Medan periode bulan Desember 2015 - Februari 2016.
Hasil penelitian terhadap 20 pasien kriteria inklusi di peroleh 19 pasien (95%) mengalami DRPs. Jenis DRPs yang paling banyak terjadi adalah indikasi tanpa obat 26 kasus (61,91%), interaksi obat 15 kasus (35,71%) dan obat tanpa indikasi 1 kasus (2,38%).

IDENTIFICATION OF DRUG RELATED PROBLEMS IN
MANAGEMENT OF PATIENTS CHRONIC KIDNEY DISEASE WITH ANEMIA IN HAJI ADAM MALIK HOSPITAL MEDAN
ABSTRACT
Drug Related Problems (DRPs) are undesirable events related to the patients drug therapy that actually and potentially affect the patients desired outcomes. Chronic Kidney Disease (CKD) remains a serious health problem in the world because it causes various complications that require polypharmacy which turn result in DRPs and decrease quality of patients life.
Objective of this prospective study was to determine the incidence of DRPs (n=20). This research used Strand method that categorizes into indications without drugs, drugs without indications, wrong drug, too low dose, too high dose, adverse drug reactions, drug interactions. Data collection was prospectively done by studied from medical records of the patients. Inclusion criteria for this research patients diagnosed with chronic renal failure along with anemia hospitalized at Haji Adam Malik Medan in December 2015 – Februari 2016.
The results showed that there were 19 patients (95%) experienced DRPs. The DRPs the most commonly occured DRPs was indicated without medication was 26 cases (61,91%), drug interactions wass 15 cases (35.71%) and drug without indication was 1 case (2.38%).
