Dislipidemi Dan Nondislipidemi.

Pendahuluan : DM tipe 2 merupakan masalah kesehatan dunia yang
prevalensinya terus meningkat. Salah satu komplikasi penyakit ini adalah Penyakit Jantung Koroner (PJK). PJK disebabkan oleh disfungsi endotel yang berlanjut menjadi aterosklerosis. Selain DM, dislipidemia dapat menyebabkan terjadinya disfungsi endotel. Diketahui bahwa 15,95% penderita DM tipe 2 disertai dengan dislipidemia. DM dan dislipidemia, dapat menyebabkan kerusakan endotel, sehingga bila dijumpai adanya dislipidemia pada penderita DM seharusnya dapat meningkatkan resiko terjadinya PJK. Salah satu marker untuk menilai resiko terjadinya PJK adalah hsCRP. Penelitian ini bertujuan mengetahui resiko terjadinya PJK pada penderita DM tipe 2 yang dislipidemia dan nondislipidemia dengan membandingkan kadar hsCRP- nya.
Metode dan cara : Penelitian inibersifat analitik observasional dengan
pendekatan potong lintang. Dilakukan di Departemen Patologi Klinik RSUP H. Adam Malik Medan, terhadap 46 sampel DM tipe 2 dengan dislipidemia sebanyak 29 orang dan nondislipidemia. sebanyak 17 orang. Dilakukan pemeriksaan darah lengkap, HbA1C, SGOT, SGPT, Lipid Profile, dan hsCRP.
Hasil : Didapati perbedaan yang signifikan untuk rata-rata kadar hsCRP pada
penderita DM tipe 2 yang dislipidemi (2,26±1,74) dan nondislipidemi (1,08±0,59) dengan p=0.01. Uji korelasi Pearson menunjukkan hubungan signifikan antara hsCRP dengan total kolesterol (r=0,326,p=0,02), hsCRP dengan LDL (r=0,327,p=0,02), trigliserida dan hsCRP (r=0,468, p=0,01). Tidak tampak hubungan yang signifikan antara hsCRP dan HDL (r=-0,093, p=0,58).
Pembahasan : Pada DM tipe 2, resiko PJK yang digambarkan dengan kadar
hsCRP, terlihat lebih dipengaruhi oleh kadar total kolesterol,HDL dan Trigliserida
Kesimpulan : Terdapat perbedaan yang bermakna kadar hsCRP antara DM tipe2
yang dislipidemia dengan yang nondislipidemia.

Introduction : DM type 2 is a global health problem which the prevalence is getting more increased. One of the complications DM type 2 is the coronary artery disease (CAD). CAD is caused by the endothelial dysfunction which later becomes atherosclerosis. Besides DM, dyslipidemia might cause the endothelial dysfunction. DM and dyslipidemia, each of them might cause the endothelium dysfunction, furthermore, if it is found that the DM patients suffer dyslipidemia, it will increase the risk of the CAD. One of the markers to evaluate the risk of CAD is hsCRP.This study aimed at knowing the risk of the cardiovascular disease in dyslipidemia and non- dyslipidemia DM type 2 patient by comparing their level of hsCRP.
Method : This study is analytical with cross sectional approach. Full blood count,
HbA1C,SGOT,SGPT, Lipid Profile, HsCRP was check to the 29 dyslipidemia samples and 17 non-dyslipidemia DM type 2 patients.
Result : Thereisa significant difference of hsCRP level in dyslipidemia
(2.26±1.74) and non-dyslipidemia (1.08±0.59) DM type 2 patient with p=0.01. The Pearson correlation coefficient shows the significant correlation between hsCRP with total cholesterol (r=0,326, p=0,02), hsCRP with LDL (r=0.327, p=0.02), hsCRP with triglyceride (r=0,468, p=0,01). However, there is no significant correlation between hsCRP with HDL (r=-0.093, p=0.58).
Discussion : In DM type 2, the risk of CAD is illustrated with hsCRP levels, its
more influenced by the total cholesterol, HDL and triglycerides levels.
Conclusion : There is a significant difference between of hsCRP level DM type 2
with dyslipidemia and nondyslipidemia. ..
