ABSTRAK
Komunikasi terapeutik adalah suatu proses dimana perawat pelaksana secara sadar memengaruhi pasien atau menolong pasien untuk lebih mengerti tentang kesehatannya. Ini adalah salah satu faktor yang memengaruhi indikator kinerja pelayanan kesehatan. Berdasarkan survey pendahuluan di Rumah Sakit Umum Herna Medan ditemukan bahwa komunikasi terapeutik belum dilaksanakan sepenuhnya oleh perawat pelaksana dan ini berdampak kepada indikator kinerja Rumah Sakit Umum Herna Medan yang rendah dimana BOR (Bed Occupancy Rate) untuk tahun 2011 hanya 52.71% yang berarti belum mencapai standard nasional yaitu 80%.
Tujuan penelitian ini adalah untuk menganalisis pengaruh faktor personal dan faktor situasional terhadap komunikasi terapeutik antara perawat pelaksana dengan pasien di Rumah Sakit Umum Herna Medan. Jenis penelitian adalah survey explanatory. Populasi dalam penelitian ini adalah seluruh perawat pelaksana yang berjumlah 241 orang dan yang menjadi sampel adalah perawat pelaksana yang bertugas diruang rawat inap dengan masa kerja lebih dari lima tahun yang berjumlah 91 orang. Data diperoleh dengan wawancara langsung dan observasi melalui kuesioner yang sudah disiapkan yang kemudian dilakukan analisis regresi linier berganda pada a = 0,05.
Hasil penelitian korelasi bivariat menunjukkan bahwa secara statistik ada pengaruh faktor personal terhadap komunikasi terapeutik antara perawat pelaksana dengan pasien di Rumah Sakit Umum Herna Medan dengan korelasi r = 0,489 dengan p = 0.000. Korelasi antara faktor situasional terhadap komunikasi terapeutik melalui korelasi bivariat diketahui tidak signifikan, tetapi dalam analisis regresi linier berganda (secara serempak) dengan faktor personal maka terlihat ada pengaruh faktor situasional terhadap komunikasi terapeutik dengan p = 0,045. Faktor personal ternyata mempunyai pengaruh lebih besar dari faktor situasional terhadap komunikasi terapeutik antara perawatan pelaksana dengan pasien di Rumah Sakit Umum Herna Medan.
Disarankan kepada Manajemen Rumah Sakit Umum Herna Medan supaya mengalokasikan dana untuk meningkatkan mutu Sumber Daya Manusia dengan memberikan pendidikan lebih baik dibidang komunikasi terapeutik terutama bagi perawat pelaksana yang bertugas diruang rawat inap. Juga dicari cara-cara yang dapat diterapkan dalam praktek keperawatan untuk meningkatkan mutu komunikasi terapeutik.

ABSTRACT
Therapeutic communication is a process in which the working nurses consciously influence the patients or help the patients to understand more about their health. This is one of the factors influencing the indicators of performance of health service. Based on the result of the preliminary survey conducted at Herna General Hospital Medan, it was found out that therapeutic communication has not yet fully implemented by the nurses and this brought an impact to the indicator of performance of Herna General Hospital Medan where its BOR (Bed Occupancy Rate) in 2011 was only 52,71%. This BOR is still far from the national standard of 80%.
The purpose of this explanatory survey study was to analyze the influence of personal and situational factors on the therapeutic communication between the patients and the nurses working at Herna General Hospital Medan. The population of this study was all of the 241 nurses working at this hospital and the samples for this study were 91 nurses working in the in-patient wards with more than 5-year length of service. The data for this study were obtained through observation and questionnaire-based interviews. The data obtained were then analyzed through multiple linear regression tests at a = 0,05.
The result of bivariate correlation showed that statistically personal factor had influence on the therapeutic communication between the nurses and the patients at Herna General Hospital Medan with r = 0,489 andp = 0,000. The correlation between situational factor and therapeutic communication through bivariate correlation was insignificant, but, simultaneously, in the multiple linear regression analysis with personal factor, the influence of situational factor on therapeutic communication was seen with p = 0,045. Personal factor in fact had a bigger influence than situational factor in terms of therapeutic communication between the nurses and the patients at Herna General Hospital Medan.
The management of Herna General Hospital Medan is suggested to allocate the fund to improve the quality of its human resources by providing a better education in the field of therapeutic communication especially for the nurses working in the in-patient wards and to find the ways that can be applied in nursing practice to improve the quality of therapeutic communication.
