KET LUAN VA KIEN NGHj

Sd luang bao cao SAE gCrt ve BDGOD trong giai jan tu nam 2006 di'n het nam 2013 la 189 bao cao Dng 185 TNLS dUOc tien h i n h v i eac bao cao nay cd I hudng tang dan theo thai gian.

Trung vj thdi gian gCfi bao cao la 13 ngay (thdi gian 10 cao nhanh nha't la 0 va cham nhal la 671 ngay).

Bao cao co day du cac thong tin can thie't chiem ty le cao 82,01% trong tdng sd 189 bao cao SAE.

HLA-B'1502 ALLEl£ IN EPILEPTIC PATIENTS AND ASSOCIATED LAMOTRIGINE-RELATED CUTANEOUS ADVERSE REACTIONS - A PILOT STUDY IN VIETNAM

Background: An^epileptie dnigs are among the most common causes of cutaneous adverse reasons, such as mild maeulopapular eruption, hypersensitivity syndrome, Stevens-Johnson syndrome (SJS) and toxic epidermal necrc^ysis (TEN). Some recent studies have reported that the human leukocyte antigen (HLA)- ff1502 allele is associated with an increased risk of lamobigine (LTG)-reiated cutaneous rea(^ons.

Objective: We investigated the frequency of HLA- B'1502 allele and the relation between HLA-B'1502 allele and LTG-related cutaneous reactions in Vietnamese epileptic patients.

Methods: This was a cross-sectional study, including 63 epileptic patients. Thirty-eight subjects were treated with lamotrigine. The presence of HLA- B'1502 was confirmed by real-time polymerase chain reacMon. Patients who had taken LTG for at least 3 months without developing cutaneous adverse reactions were considered to be LTG-tolerant.

Results: Among 63 sulyects (28 men, 35 women, mean age at diagnosis 18.4 ±12.8 years), the frequency of HLA-ff-1502 allele was 25.4% (16 patients). The HLA-^1^2 allele was present in 80%

(4 of 5) of LTG-related cutaneous reac^ons cases but in only 9.1% (3 of 33) of LTG-tolerant groups. The occurrence of LTG-related cutaneous reactions associated with the presence of the HLA-ff'1502 allele (odds ratio (OR) = 40; 95% confidence interval (Cl) = 3.3-483.4; p = 0.004).

Conclusions: This study showed a high rate of HLA-B'1502 allele in epileptic patients in Vietnam.

TTiere was statistical relation between HLA-B'1502 and lamotrigine-related cutaneous adverse reactions.

Future studies with larger sample sizes are suggested to verify the results.

Trong thdi gian tU 1/2014 den 10/2014, chung toi khao sat dupe 63 trUdng hpp ddng kinh cd ket qua xet nghiem alen HLA-B'1502, trong dd cd 38 ddi tupng sd dung lamotrigine.

Theo cac nghien cUu trudc d i y , ti le hien hanh ciia alen HLA-B'1502 k h i cao trong nhieu cong ddng d i n cU C h i u A, T I l i nay thay ddi tU 12-15,7% d ngUdi Malay sdng tai Malaysia v i Singapore; 5,7-14,5% 6 ngudi Han sdng tai Dai Loan, Hong Kong, Malaysia, Singapore; cho den tU 8,5-27,5% d ngudi T h i i Lan.

Tuy nhien ti le n i y lai thap d Sri Lanka, Nhit Ban, H i n Qudc cung nhu 6 Chau Au, Nam My v i dan da trang eOng nhu dan ban dia d Bac My [5].

Nghiin cUu cua chung tdi thuc hien tai Benh vien Chuyen khoa Ngoai than kinh Qudc td tren nhdm ddi tudng la benh nhan dong kinh, Trong 63 benh nhin, 16 trudng hpp cd alen tHLA-B'1502, chidm tj- l i 25,4%, trong do cd 7 trudng hpp alen HLA-B'1502 5 dang ddng hpp tLT (11,1%) va 9 trudng hpp d dang d; hpp tijr (14,3%) (bang 1). Ti le dUdng tinh cao tren nhdm binh nhin nay gian tiep the hien ti le hien hanh cCia alen HLA-B'1502 cd kha nang kha cao d dan sd Viet Nam ndi chung. Vdi 25,4% binh nhin cho ket qua duong tinh, chdng tdi nhan thay ti le nay cao hdn so vdi nhieu nghiin cUu dupe thUe hiin tai cac quoc gia khac, va g i n tuwng dUdng nghien cUu tai Thai Lan vdi ket qua l i 27.5% [5]. Tuy tl le mang alen nay rat dao dong d cac nghiin cUu khic nhau, nhung ket qua dUdng tinh cao cOa trong nghien cUu cfia chUng tdi cho thay day that su la mdt van dd can duac quan tam nham nang cao hieu qua quan ly benh nhan ddng kinh 5 Viet Nam.

Tiep sau dd, nhieu nghien eUu duac tien hanh nham khao s i t mdi lien he giiia tinh trang dj Ung da ciia benh n h i n cd sir dung cae thudc chdng ddng kinh vdi alen HLA-B*1502. Tai An Dp, carbamazepine dupc xem l i mdt trong nhCftig nguyen n h i n chi'nh gay nen hdi chUng SJS/TEN, tai Singapore va Malaysia cung cho rang 27,7 den 35,7% trudng hpp SJS/TEN cd lien quan den Carbamazepine [3]. Nghien cUu phan tich tong hpp thuc hien nam 2014 cho kdt qua tren 12 benh nhan bj hoi ehUng SJS/TEN gay ra bdi lamotrigine cd den 33,3% mang alen HLA- B'1502 so vdi ehi 9,4% trong sd 128 benh nhin d nhdm dung nap thude cd alen nay [7]. Mot nghien cUu khac cung cho thay d nhdm ed mang alen HLA- B'1502, ti l i benh n h i n bi hpi c^Ung SJS/TEN do sit dung lamotrigine cao 4,51 lan so vdi binh nhan khdng phit hien HLA-B1502 [4]. Nghien cUu cOa chdng toi cd 38 benh nhin dpng kinh si> dung lamotrigine, trong dd ghi nhin 5 benh n h i n cd phan Ung phu tren da v i da dupc t u van ngUng thudc. Trong nhdm benh nhin di Cmg da cd den 80% (4/5 benh nhan) mang alen HLA- B'1502 trong khi nhdm dung nap vdi lamotngine chi ed 9,1% (3/33 benh nhin) mang alen nay, SU khac biet giiJa 2 nhdm cd y nghTa thdng ke (OR = 40; 95% Cl = 3,3 - 483,4; p = 0,004) (bang 2).

KET LUAN

TJ l i cd alen HLA-B'1502 tren benh nhin dong kinh tai Viet Nam l i 25,4%.

Cd mdi lien quan cd y nghTa thdng ke giija alen HLA-B'1502 va nguy cd phan Ung phu tren da cOa lamotrigine.

Trong tuong lai can cd cac nghien eUu vdi ed m i u Idn hdn de lam rd ket qua.

SUMMARY

DERMAL GRAFT FOR SEVERE CHORDEE WITH HYPOSPADIAS

Purpose: We present the technique of using a small dermal graft to augment the ventral tunica albuginea in the patient with severe chordee.

Materials & methods: We reviewed the records of 44 patients who underwent severe chordee correction between March 2011 and September 2013. The age ranged from 1 to 14. Dermis was used in 44 patients We grouped cases according to the etiology of chordee and we treated in the standard and a straight phallus was confirmed in all postoperatively.

Results: One dermal graff was placed in 44 patients. Second stage urethral.

reconstruction was done 6 months later with a Thiersch Duplay tube. Patient age ranged from 1 to 14 years. All patients have been followed for a minimum of 6 months and a maximum of 2 years.

Conclusions: We bilieve that the additional penile length achieved with dermal graft results In a depent phallus and cosmetic preferable to that of plication only.
