CLINICM TEATUGES AND EEIATED FACTODS IN PATIENTS WITH CUTANEOUS SQUAMOUS CELL CAECINOMA

SirmTa-y The objective of the study is to investigate clinical features and histopattiologica! findings of patients with cutaneous squamous cell carcinoma (SCC) at National Hospital of Dermatology and Verenology (NHDV). The study's method is the restrospective and spective descriptive study on data of 162 patients with cutaneous SCC at NHDV. Results show that, the number of patients with SCC has yearly increased from 2007 to 2014 at the NHDV. The most common age group of patients with SCC is the over 60 year-old group (69.65%).

Farmers account for 60% of patients.

SCC arised from chronic cutaneous eryptions makes up 39.85%. Lesions have often dominated on the exposed skin areas, including head and neck (52.74%). Ulcerated lesions are usually infiltrated rather than non-infiltrated (70.5% compared to 29.5%); 5 out of 18 patients (27.8%) have local lymph node metastasis. The tumour evoluting on chronic lesion has 1.56 times higher risk of lymph node metastasis than that developing on normal skin [95% CI;

(1.05; 2.39)]. The infiltrated lesion has 1.63 times higher risk of lymph node metastasis compared to non-infiltrated tumours [95% CI; (1.06; 2.5)]. In conclusion, SCC has usually occurred in groups of people working under the sun and often appeared on previous skin lesions. The rate of lymph node metastasis is relatively high, and there has been a link between lymph node metastasis and previous skin lesions under the patterns of infiltration.

Theo nghien cUu cua Nguyen Thj Thai Hoa nam 2002 va cCia Trjnh Quang Di§n nam 1999 [5, 6] thi tren 80% cac trudng hop UTBM t^ bao vky d lUa tudi tr§n 40 v& tudi trung binh theo Geller A.C. nam 2003 la 68,1 6 nam va 72,7 6 nCf [3]. Dieu nay khac biet so vdi mot sd nghidn cUu khac nhu nghien cdu cua C D . Sherman [2] va nghien cdu cua Phgm Cam PhUdng [8], UTBM te bao vky phat trien chCi ydu trdn seo bdng. Cd Id do ngay nay nhu cau tham my cua con ngudi cao va phUdng tidn chan doan ngay cang tidn tien ndn sd IUdng bdnh nhdn kham va didu tn seo ngay cang sdm. 6 nhdng benh nhan cd thUdng tdn da lan tda nhu lupus ban dd, benh da do gen, UTBM te bao vky thudng cd nhieu thUdng ton ung thu, 100% cac benh nhan nay cua chung tdi cd Xii 2 thUdng ton ung thu trd len. Cd le sd bdnh nhan nghien cUu cua chung tdi nhd ndn khdng thay rd mdi lien quan gida nguy cd di can va thudng tdn da mgn tinh cung nhu thUdng ton tham nhiem. NhOng bdnh nhan cd thUdng ton da mgn tfnh chT cd nguy cd di can gap 1,56 lan so vdi UTBM te bao vay khdng cd thUdng tdn da cu, nhUng thUdng tdn tham nhilm cd nguy cd di can cao gap 1,626 lan so vdi thUdng ton khdng thdm nhiem. Ty Id di can cua UTBM te bao vay cung khdng cao, theo nghidn cUu cua Phgm Cam PhUdng [8], ty Id di can hach chiem 9,3%. Tuy nhidn tuy vao vi trf khdi u ty Id di can cung khac nhau, cd the tdi 43% d chi trdn, 60% d ban tay trong nghidn cdu nam 2000 cua Pham Hung CUdng [10].
