Aims: To study the clinical, paraclinical features and effectiveness of the early excision and skin graft for deep bums from 10% to 20 % of the total body sudace area (TBSA) in children.
Method: This prospective study was conducted on 30 pediatric patients aged from 6 months to 7 years at the National Institute of Burns from November 2010 to July 2012 who had area deep bums from 11 to 20% TBSA.
Results: Wet necrosis takes up 96.7%. The bactena on the wound surface was main P.
eruginosa (60%) and S. aureus (23.3%). The ratio of shock, clinical and paraclinical course in the period of burn shock and infection cases increased in direct proportion to the area of deep burns Deep bum wounds treated eady excision and skin graff: the edema reduce and go away after 1-2 days; reduction in both the frequency and amount of P. eruginosa in Igram of tissue (from 53.3%

The symptoms of bacterial toxicity period reduced: mental disorder (from 83.33% to 30%); high fever (100% - 30%), digestive disorder from 56.66% - 16.66%. Inflammatory cytokine reduced, immune factor increased: when the deep bum wounds are fully covered, IL1, IL-6 and TNF-a (P < 0.05 and P < 0.01). Affer surgery, number of CD4 increased while IgM and IgG only increase with the only increase if the deep bum wound was fully covered with skin graff (P < 0.01).

4. BAN LUAN
