Latar Belakang : Karbamazepin dan asam valproat telah diketahui sejak lama mempunyai efek samping metabolik serta perubahan ringan pada enzim hati yang biasanya bersifat sementara. Karbamazepin mengandung zat yang dapat merangsang enzim yang dapat meningkatkan metabolisme glukoronide dari hormon tiroid. Karbamazepin dapat membentuk epoksid stabil dan metabolit reaktif yang berperan dalam patogenesis hepatotoksik. Asam valproat menyebabkan peningkatan GABA yang berpengaruh pada kelenjar pituitari dalam menghambat sekresi TSH. Reduksi CoA interselluler merupakan jalur sentral utama asam valproat dalam menyebabkan hepatotoksisitas.
Metode : Penelitian ini merupakan kasus potong lintang dengan 13 subjek yang memakai karbamazepin dan 13 subjek yang memakai asam valproat di Rumah Sakit Haji Adam Malik Medan. Subjek merupakan pasien yang didiagnosis epilepsi berdasarkan anamnesa, pemeriksaan neurologi dan pemeriksaan penunjang. Fungsi tiroid dan fungsi hati dihitung dengan mengambil darah vena.
Hasil : Penelitian ini terdiri dari 13 subjek yang memakai monoterapi karbamazepin dan 13 subjek yang memakai monoterapi asam valproat. Didapatkan perbedaan yang tidak bermakna antara pemakaian monoterapi karbamazepin dan asam valproat terhadap T3(p=0,420), T4(p=0,645), TSH(p=0,202) serta terhadap Bilirubin (p=0,593), ALP(p=1,00), AST(p=1,00), ALT(p=0,673). Namun terdapat perbedaan yang bermakna antara durasi pemakaian monoterapi asam valproat terhadap ALP (p=0,038). Besar risiko dosis pemakaian monoterapi karbamazepin terhadap T3, T4, TSH (RP=1,88; 1,25; 0,60) dan besar risiko durasi pemakaian monoterapi karbamazepin terhadap T3, T4, TSH (RP=0). Besar risiko dosis pemakaian monoterapi karbamazepin terhadap Bilirubin, ALP, AST, ALT (RP=0) dan besar risiko durasi pemakaian monoterapi karbamazepin terhadap Bilirubin, ALP, AST, ALT (RP=0). Besar risiko dosis pemakaian monoterapi asam valproat terhadap T3, T4, TSH (RP=0,44; 0,44; 0) dan besar risiko durasi pemakaian monoterapi asam valproat terhadap T3, T4, TSH (RP=1,25; 1,60; 0,25). Besar risiko dosis pemakaian monoterapi asam valproat terhadap Bilirubin, ALP, AST, ALT (RP=0,89) dan besar risiko durasi pemakaian monoterapi asam valproat terhadap Bilirubin, ALP, AST, ALT (RP=1,25).
Kesimpulan : Dijumpai perbedaan yang tidak bermakna antara pemakaian monoterapi karbamazepin dan asam valproat terhadap T3(p=0,420), T4(p=0,645), TSH(p=0,202) serta terhadap Bilirubin (p=0,593), ALP(p=1,00), AST(p=1,00), ALT(p=0,673). Namun terdapat perbedaan yang bermakna antara durasi pemakaian monoterapi asam valproat terhadap ALP (p=0,038).

xxv ABSTRACT
Background : Carbamazepine and valproic acid has been known for a long time to have metabolic side effects and mild changes in liver enzymes temporarilly. Carbamazepine contain substances that can stimulate enzymes to increase glukoronide metabolism of thyroid hormones. Carbamazepine can form stable epoksid and reactive metabolites that play a important role in the pathogenesis of hepatotoxicity. Valproic acid causes an increase GABA that effect on the pituitary gland to inhibit the secretion of TSH. Interselluler CoA reduction is the major central line of valproic acid in hepatotoxicity.
Methods: This study is a cross-sectional study with 13 subjects taking carbamazepine and 13 subjects taking valproic acid in Haji Adam Malik Hospital in Medan. Subjects were epilepsy patients that diagnosed by anamnesis, neurological examination and diagnostics tools. Examination of thyroid function and liver function were taking from venous blood.
Results: This study consisted of 13 subjects taking carbamazepine monotherapy and 13 subjects taking valrpoac acid monotherapy. There were significant difference between carbamazepine and valproic acid monotherapy to T3(p=0.420), T4(p=0.645), TSH(p=0.202) and to Bilirubin (p=0.593), ALP(p=1.00), AST(p=1.00), ALT(p=0.673). However there was a significant difference between the duration of valproic acid monotherapy to ALP (p = 0.038). Prevalence ratio of carbamazepine monotherapy doses to T3, T4, TSH (PR = 1.88; 1.25; 0.60) and prevalence ratio of carbamazepine monotherapy duration to T3, T4, TSH (PR = 0). Prevalence ratio of carbamazepine monotherapy doses to Bilirubin, ALP, AST, ALT (PR = 0) and prevalence ratio of carbamazepine monotherapy duration to Bilirubin, ALP, AST, ALT (PR = 0). Prevalence ratio of valproic acid monotherapy doses to T3, T4, TSH (PR = 0.44; 0.44; 0) and prevalence ratio of valproic acid monotherapy duration to T3, T4, TSH (PR = 1.25; 1.60 ; 0.25). Prevalence ratio valproic acid monotherapy doses to bilirubin, ALP, AST, ALT (PR = 0.89) and prevalence ratio of valproic acid monotherapy duration to Bilirubin, ALP, AST, ALT (PR = 1.25).
Conclusion: There were significant difference between carbamazepine and valproic acid monotherapy to T3(p=0.420), T4(p=0.645), TSH(p=0.202) and to Bilirubin (p=0.593), ALP(p=1.00), AST(p=1.00), ALT(p=0.673). However there was a significant difference between the duration of valproic acid monotherapy to ALP (p = 0.038).
