Nghien cim dm?c thuc hien nhdm ddnh gid tdc dying ciia vien nang "Gidng chi tieu khat linh "

(GCTKL) tren cdc chi sd glucose vd trgng thdi chdng oxy hod trong mdu d chuot cdng trdng gay ddi thdo duang typ 2 thuc nghidm. Kit qud nghien cuu da chung minh GCTKL co tdc dung- (1) o chudt sau khi hi rdi logn lipid mdu gdy ddi thdo dudng typ 2, sau 60 ngdy diiu tri GCTKL liiu 840mg vd 1260mg/kg/ngdy dudng udng co tdc dung gidm cdc chi sd glucose mdu con 79,8% & 67,8% (2) Tdc dung tren trgng thai chdng oxy hod mdu: Hogt do GPx mdu a chugt ddi thdo dudng dugc diiu tri a lieu 840mg va 1260mg/kg/ngdy gidm it hem so vai cdc chudt khdng dugc diiu tri, s\t khdc biet vdi p

Summary

STUDY ON T H E EFFECTS O F GCTKL (GIANG CHI TIEU KHAT LINH) CAPSULES ON S O M E INDICATORS OF GLUCOSE AND ANTIOXIDANT SYSTEM IN BLOOD IN EXPERIMENTAL DIABETIC WHITE RAT TYPE 2 Objective of the study was to evaluate Peatment effects of GCTKL capsules on some indicators of glucose and antioxidant system in ~blood in experimental diabetic white rat type 2. The results showed that the GCTKL at doses of 840 mg/kg. 1260 mg/kg via oral after 60 days reduced concenPationof glucose at 79,8% & 67,8%: GCTKL at dose of 1260mg/kg/dqy is more effective than dose of 840 mg/kg/day (p<0,05). Effects on anti-oxidation: blood GPx activity in dyslipidemia and diabetic rats treated at dose of840mg and 1260 mg/k^day reduced less than untreated rats, the difference was statistic significant with p<0.05 andp<0.01.

Su thieu tuong dfii hay tuy6t doi insulin trong benh dai thao duong (DTD) la nguyen nhan gay thay doi cac con duong chuyen hoa ma truoe tien la roi loan chuyen hoa glucid.

Benh nhan DTD thudng duoc cho la chiu tac dong cua cac stress oxy hoa.

Cac nghien cuu dich te hpc da cho thay rang su tang glucose mau man tinh la yeu to nguy co cho su phat trien cac bien chung cua DTD. Khi nong do glucose trong te bao cao co the gay doc bang cac co che nhu tang chuyen hoa glucose theo con ducmg polyol, glucosamin, tao thanh. cac san pham tan (advanced glycation end- products AGE), hien tugng glycosyl boa khong enzym, tao thanh va boat hoa protem kinase C trong hang chuoi cac stress oxy hoa. S\r tao thanh AGE la khong thuan nghich, xay ra trong giai doan hang thang den hang nam va co the gay t6n thuong mo. Su tich luy AGE trong collagen co the lam giam tinh dan hoi cua mo lien ket (ton hai chuc nang mach mau) va lam ton thuong mang day ciia than. CJ nhung b?nh nhan DTD, AGE con co mat trong ca LDL ciia mau va cac ton thuong VXDM [1][2]. Theo cac thdng ke, bien chiing tim mach la nguyen

nhan hang diu gay tii vong 6 b?nh nhan DTD, vgly nen dieu tri RLLPM, kiSm soat duong mau, cai thiSn trang thai chfing oxy hoa mau la vin de thiet thuc trong dieu tri DTD [2][3]. Vi$c sii dung cac che pham YHCT co ngufin g6c thuc vat, CO tac dyng ch6ng oxy hoa dang la xu huong trong thoi dai hiSn nay. Giang chi tieu khat linh (GCTKL) tu bai thuoc nghiem phuang

"Giang chi thang" gia vj. Huong den ting dung tren ldm sang, chiing toi tien hanh tren thuc nghiem nham cac muc tieu:

1. Bdnh gid tdc dung ha glucose mdu cua vien nang Gidng chi tiiu khdt linh & chugt cdng trang gay BTB typ 2 thuc nghidm.

2. Bdnh gid tdc dung cua vien nang Gidng chi tieu khdt linh trin su thay doi trang thdi chSng oxy hoa trong mdu & chupt BTB typ 2 th^

nghiem

II. GHAT LIEU, DOI TU'QfNG VA PHirONG PHAP NGHIEN ClTU [4]

2.1. Chat lifu nghien curu: Vien nang "Giang chi tieu khat linh"

500mg.

- Noi san xuat: Cong ty CP Dugc phSm Khang Minh - TP. HCM

- Bai thufic: Dan sam, Sinh hoang ky, Ha thu 6 do, Trach ta, Nguu tat.

chi, Thien hoa phan, Hoang lien (tong 72g).

- Lieu thuoc du ki6n dung tren ngucri (50kg): 18 vien nang 500mg/ngay tuong duong 01 thang thuoc 72g, nghien ciiu tham do them lieu 2 bang 2/3 lieu tren.

- Lieu dimg tren chupt (he so 1:7):

Liau 1260mg va liSu 840mg /IkgTT/

ngay.

2.2. Doi tuQTig nghidn cmi: Chupt eong trang, chung Ratus norvegicus 2 thang tuoi, ca hai gioi, khoe manh, can nang 120 ± 20 g, nuoi trong dieu kien tieu chuan thuc nghiem tai khu chan nuoi dong vat thi nghiem benh vien Viet Nam Thuy Dien, Uong bi, Quang Ninh.

2.3. Phuong phap nghien cihi + Mo hinh gdy roi loan lipid mau:

Cho chupt an che do an giau lipid 40%

calo la ch4t beo (theo bang thanh phan dinh duong thuc pham Viet Nam, Vien Dinh duong, Bp Y ta, Nha xu4t ban Y hpc 2000) trong suot qua trinh thuc nghiem.

+ Mo hinh gay dai thao duong thuc nghiem: Ngay thii 31 cua roi loan lipid mau, chupt dupe tiam mang bung STZ voi nau 50mg/kg (mot liau duy nhat). Sau 48 dan 72 h, dinh lupng

nong dp Glucose mau dat > 11 mmol/l la dat yau cau chupt dai thao duong.

Chupt dupe chia ngau nhien thanh 516

- Lo chirng: n = 21, chupt dupe nuoi vdi cha dp binh thuong 12%

calo khau phan an la lipid (theo bang thanh phan dinh duong thuc pham Viet Nam, Vien Dinh duong, Bp Y ta, Nha xu4t ban Y hpc 2000) va uong dung dich NaCl 9%o lieu 1 ml/1 OOgam/ngay.

- Cac 16 1, 2, 3, 4 : Chupt duoc gay roi loan lipid mau; trong do cac 16 2, 3, 4: Sau khi gay roi loan lipid mau, ngay thii 31 chupt dupe gay DTD bang phuang phap tiem mang bung STZ voi lieu 50mg/kg (mpt liau duy nhat). Sau 48 dan 72 h, dinh lupng n6ng dp Glucose mau dat > 11 mmol/1 la dat yau cau chupt dai thao duong.Trong suot qua trinh thuc nghiem, cung v6i che dp an giau lipid 40% calo la chat beo, chupt cac 16 nay dupe uong them:

+ L6 1 (chiing duong gay RLLPM) .. = 21, u6ng NaCl 9%o liau 1 ml/1 OOgam/ngay.

+ Lo 2: n =14, Sau khi gay DTD, u6ng NaCl 9%o liau Iml/lOOgam/ngay.

+ Lo 3: n =14, Sau khi gay

Tai cac thai diem giet chuot dinh lupng glucose mau, TC, TG, LDL-C; HDL-C, TAS huySt tuong, hogt dp SOD hong cau, GPx mau toan phjn, THb (Total Hb).

Dinh lupng Glucose mau (tinh theo don vj mmol/1), hoat do SOD

h6ng cSu (tinh theo don vi U/ml), GPx mau toan phSn (tinh theo dcm vj U/1) va THb (Total Hb tinh theo don V! g/dl).

Cac xet nghiem sinh hoa dupe th\rc hien tren may phan tich sinh hoa tu dpng OLUMPUS - AU 400 (Nhat) vdri cac kit h6a chat hang Olympus cung cap tai Khoa Hoa sinh, benh vien Viet Nam - Th\iy Dien, Uong Bi - Quang Ninh.

Sau khi dinh lupng SOD, GPx, Hb tren may, tinh ket qua SOD va GPx theo don vi U/gHb theo cong thiic sau:

Thuc nghipm gay r6i loan lipid mau a chupt voi ch6 dp an 40% calo khau phan an la ch§t beo trong nghien cuu da dat yeu cau. Ngay 6 ngay thii 30 hau het cac chi s6 ve lipid mau 6 nhom chupt an theo cha dp giau ch4t beo da tang nhiau (tru HDL- C). Dac biet chi so LDL- C tang cao va ty so HDL- C/LDL- C giam, sir khac biet co y nghia th6ng ka so voi 16 chung, cho thay da xuat hien yeu to nguy co gay vija xo d0ng mach 6 chupt.

Mpt so nghien cuu cua cac tac gia trong va ngoai khac dau co nhSn xet chung la cha dp an giau chat beo gay tang ro r^t nong dp TG, TC va LDL-C trong mau chupt luc doi. Chupt duoc nuoi boi cha dp an giau lipid co su giam nhay cam voi insulin va chi can mpt lieu thap STZ ciing dii gay DTD.

Sau 90 ngay nghian cuu, n6ng dp TAS huyat tuang, hoat dp SOD va GPx h6ng cau giam so vai 16 chiing thi chi CO sir khac biet d boat dp GPx giam hpn 16 chiing voi p<0,05.

Nhu v|iy mo hinh gay DTD typ 2 da thanh c6ng, dap iing yeu cau cho nghien cuu

4.1 Tac dung diau tri tren chi so glucose mau ciia Giang chi tieu khat linh sau 30 ngay va 60 ngay uong thuoc

Bang 3.1 cho thay nghien cuu da thanh c6ng trong viec gay DTD typ 2 cho nhiing 16 chupt bi RLLPM bSng each tiam mang bung mot liau thap STZ 50mg/kg. Trong nghien ciiu nay cho ket qua sau 30 ngay va 60 ngay u6ng thuoc, n6ng dp glucose huyet thanh ciia chupt 16 3 va 16 4 (hai 16 chupt DTD duac u6ng GCTKL) giam CO y nghia th6ng ke so vai !6 2 (chupt DTD kh6ng dupe u6ng GCTKL) (p<0.01); Nong dp glucose mau thay d6i chiau huong t6t hon a liau GCTKL 1260mg (p<0,05). Ket qua nay tuong duong voi ket qua cua Nguyan Quang Trung nghien ciiu 2008, bpt chiat la dau CO tac dung giam n6ng dp glucose mau cua chupt gay DTD typ 2 sau 30 ngay va 60 ngay la 71% va 77%.

GCTKL CO tac dung giam n6ng dp glucose mau 6 chupt gay RLLPM va DID thuc nghiem, nhung kh6ng lam giam glucose mau o chupt khong bi DTD. Dieu nay rat co y nghia trong y hpc va day cung la mpt uu tha ciia thuoc y hpc co truyen, thuoc co chi co tac dung voi cac truong hop benh ly va khong anh huong toi cac trucmg hpp binh thucmg. Tian tai nghien ciiu tran lam sang, Giang chi tieu khat linh co the dung ha glucose mau cho benh nhan co duong mau cao.

V. KET LUAN

Giang chi tieu khat linh lieu 840mg va 1260mg/kg/ngay duong u6ng sau 60 ngay diau tri co tac dung:

1. Lam giam nong dp glucoza mau tran chupt DTB typ 2 (p<0,01)

2. Lam boat dp GPx tren chupt DTD typ 2 giam it ban so vai 16 kh6ng dugc dieu tri (p<0,01).
