Introduction: Diabetes is one of the most common health problems in healthy people. One of the essential care strategies based on 5A self-management is to increase the potential of chronic patients. Therefore, this study was conducted in 2019s to impact the program's self-management based on the 5A on empowering salamanders with diabetes in the Clinic of Razi Hospital in Qaemshahr city.
Methods: This semi-experimental experimental study was performed on 90 older adults with diabetes by simple random sampling in two test and control groups. In the test group, 12 sessions of 30-40 minutes of training were performed based on the 5A self-management. The instrument of this study is the Patient Empowerment Questionnaire.
Results: The independent t-test did not show a significant difference between the two groups of test and control before the intervention (P < 0.93), but after the intervention, it showed a significant difference (P < 0.03). The ANOVA test showed a significant difference after eliminating the pre-test effect (P < 0.032 and Eta = 0.01) so that 10% of the post-test changes are due to the self-management.
Conclusions: According to the results of this study, it can be said that the implementation of the 5A self-management model is one of the most important appropriate approaches to reduce the costs of treatment and care for the elderly with diabetes and improve their empowerment.

Extended Abstract
OBJECTIVE
Diabetes is a chronic disease and one of the significant public health problems around the world. Among the models and programs used in self-management and empowerment, the 5A self-management, or behavior change based on five stages of review, guidance, agreement, assistance, and follow-up, is considered the most common healthcare model. 5A self-management program training is an evidence-based program that first assesses patients 'behavioral levels, therapeutic beliefs, and motivations and describes patients' status. Based on
the clinical trial study and educational experiences, the researcher decided to conduct the present research to investigate a self-management program based on the 5A model on empowering the elderly with diabetes.
MATERIALS AND METHODS
This study was a semi-experimentalexperimental study using a simple random sampling method on 90 older people with diabetes in Qaemshahr city located in the Mazandaran province, located in northern Iranin 2019.

The research environment was the diabetes clinic of Razi Hospital in Qaemshahr city Criteria for entering the study of the elderly over 60 years of age with a diagnosis of diabetes by a physician who has been diagnosed with the disease for the past one year. They had minimal literacy, were aware of time and place, and lacked mental illness or other chronic illnesses such as high blood pressure and dialysis. Criteria for elimination from this study were patients with diabetes who missed more than one session. In this research, the Empowerment Questionnaire was designed by the University of Michigan Education and Research Center in 2000 in English. The questionnaire was graded from eight questions based on a 5-point Likert scale from an entirely agreeable score of one to an opposite score of five. The score ranges from 40 to 8, with a lower score indicating higher empowerment. This experimental study of 90 diabetic salamanders was performed randomly in a simple randomized trial in two groups. In the test group, 12 sessions were conducted with 40-30 minutes of training based on the model 5A. This research tool can empower patients after getting the test SPSS21 descriptive statistical software (tables, mean
and standard deviation) and inferential statistics (paired t-test and analysis of covariance) were analyzed at the significant level 0/05.
RESULTS
This study showed the empowerment of the elderly with diabetes in the experimental group before the intervention of 22.56 + 4.98 and the control group of 22.4 + 5.7. The independent t-test did not show a significant difference between the two groups (P <
0.92(. However, after the intervention in the test group20.17 + 4.89 and the control group23.15 + 5.32, it was obtained that the independent t-test did not show a significant difference (P < 0.03). The paired t-test in the test group showed a significant difference before and after the intervention (P < 0.03), but the paired t-test control group did not show a significant difference before and after the intervention (P < 0.32). The incubation test showed a significant difference after eliminating the pre-test effect (P < 0.03, Eta = 0.1) so that 10% of the post-test changes were due to the 5A self-management model.

CONCLUSION
This study shows the effectiveness of implementing the 5A model self-management program on increasing the empowerment of the elderly with diabetes. Given the weakness of traditional education in considering the active role of patients in controlling their disease and clarifying the effectiveness of short-term structured training programs due to patients' active participation in managing the disease symptoms, we can expect increased independence in patients. This method can be used as a cheap way to improve the condition of the elderly with diabetes. He also used these programs in future research to improve the state of other chronic diseases.
